Left cardiac sympathetic denervation: case series and technical report.

BACKGROUND: Left cardiac sympathetic denervation (LCSD) is a surgical procedure that has been shown to have an antiarrhythmic and antifibrillatory effect. Evidence indicating its antiarrhythmic effect has been available for over 100 years. It involves the removal of the lower half of the stellate ganglion and T2-T4 of the sympathetic ganglia and is carried out as either a unilateral or bilateral procedure. With advancements in thoracic surgery, it can be safely performed via a minimally invasive Video-Assisted Thoracoscopic Surgery (VATS) approach resulting in significantly less morbidity and

Thoracoscopic pulmonary wedge resection without post-operative chest drain: an observational study.

OBJECTIVE: Chest drains are used routinely after wedge resection by video-assisted thoracoscopic surgery (VATS), although this practice is based largely on tradition rather than evidence. Chest drains may furthermore cause pain, infections, and prolonged length of stay. The aim of this prospective observational study was to assess the feasibility of avoiding chest drains following VATS wedge resection for pulmonary nodules. METHODS: Between 1 February and 25 August 2015 166 consecutive patients planned for VATS wedge resection of pulmonary nodules were screened for inclusion using the followi

Thoracoscopic patch insulation to correct phrenic nerve stimulation secondary to cardiac resynchronization therapy.

AIMS: Cardiac resynchronization therapy is an established therapy for heart failure, improving quality of life and prognosis. Despite advances in technique, available leads and delivery systems, trans-venous left ventricular (LV) lead positioning remains dependent on the patient's underlying venous anatomy. The left phrenic nerve courses over the surface of the pericardium laterally and may be stimulated by the LV pacing lead, causing uncomfortable diaphragmatic twitch. This paper describes a video-assisted thoracoscopic (VATS) procedure to correct phrenic nerve stimulation secondary to cardia

Consequences of persistent pain after lung cancer surgery: a nationwide questionnaire study.

BACKGROUND: post-thoracotomy pain syndrome (PTPS) and its social consequences have been inconsistently investigated as most studies were either small sized, focused on a limited number of risk factors or included heterogeneous surgical procedures. The current objectives were to obtain detailed information on the consequences of PTPS after thoracotomy and video-assisted thoracic surgery (VATS) from homogenous unselected nationwide data, and to suggest mechanisms for the development of PTPS. METHODS: data from 1327 patients were collected using a prospective national database and combined with

Surgery for pulmonary coccidioidomycosis: a 10-year experience.

BACKGROUND: Coccidioidomycosis results from infection with Coccidioides species endemic to the southwestern United States. The mobile US population has resulted in incremental cases being found throughout the world. The fungal infection can result in pulmonary sequelae, including nodules, cavities, and complications requiring treatment by the thoracic surgeon. METHODS: A retrospective chart review was conducted of 1,496 patients with coccidioidomycosis treated at our institution (January 1998 to December 2008) to identify those requiring surgery. RESULTS: Of the 1,496 patients, 86 (6%; mean

Surgical treatment of organising pneumonia mimicking lung cancer: experience of 27 patients.

OBJECTIVES: Organising pneumonia as a localised process has sometimes been managed by surgery. This poorly defined entity has been mistaken clinically, roentgenographically and usually at exploratory thoracotomy for lung cancer. This series details our experiences with surgical resections in 27 patients during the past 13 years. METHODS: Surgical lung resection was performed through wedge resection or lobectomy with or without video-assisted thoracoscopic surgery. Resected specimens were swabbed for aerobic and anaerobic bacterial, fungal and mycobacterial cultures. Clinical presentation, roe

Defining the learning curve for video-assisted thyroidectomy.

BACKGROUND AND AIM: Several studies have demonstrated that endoscopic thyroidectomy is a safe technique. Aim of the study is to evaluate the feasibility of video-assisted thyroidectomy (VAT) performed by a junior surgeon. MATERIALS AND METHODS: We consider 67 consecutive standard VAT gasless approaches. VAT was performed by an under 35-years-old surgeon trained in basic laparoscopy tutored by an experienced surgeon. Outcome measures were operative and hospitalization times, incision length, and complications. Conversion to open surgery was defined as the need to perform a longer incision. To

Indications for mediastinoscopy and comparison of lymph node dissections in candidates for lung cancer surgery.

A prospective phase II study of indications for surgery, using video-assisted mediastinoscopy (VAM) to detect mediastinal lymph node metastasis was conducted in patients with resectable primary lung cancer of clinical stages I-IIIA. According to the indication criteria for VAM, Group A patients had primary tumor resection and lymph node sampling without VAM. Patients without detected metastasis by VAM underwent thoracotomy and systematic lymph node dissection (Group B). Cases with mediastinal lymph node involvement confirmed by VAM were treated with chemotherapy followed by radiotherapy (Group

Pericardial cysts of the mediastinum.

Pericardial cysts are an uncommon benign disease. Their treatment, in the past, was based on excision by thoracotomy or, in selected cases, on percutaneous aspiration. The progress of video-assisted thoracoscopy gave new possibilities, but most surgeons still consider the thoracotomic approach as the treatment of choice. The aim of this study is to report our experience and to discuss the role of different therapeutic procedures in the management of pericardial cysts. Between 1970 and 1996, 24 patients with pericardial cysts were treated at the first Department of Surgery of the University of

Computer Tomography-Guided Pulmonary Microcoil Insertion for the Localisation of Small Pulmonary Nodules at the Time of Pulmonary Resection.

Background Small and ground glass pulmonary nodules pose a surgical and diagnostic challenge, with an increase in identification expected with the roll-out of lung cancer screening in Australia. Accurate preoperative localisation of small pulmonary nodules can be a useful adjunct to lung resection in nodules that are not palpable. Various methods have been trialled including methylene blue dye injection and hook-wire placement; however, these are limited by dye diffusion, wire dislodgement, and patient discomfort. CT-guided insertion of a microcoil is a technique of localisation that provides

3D digital exoscope is safe tool in the surgery of olfactory groove meningiomas.

BACKGROUND: Digital 3D exoscopes have been introduced as alternatives for operating microscopes in microneurosurgery. It has been hypothesized that exoscope may provide benefits especially at the most anterior skull base, where surgical trajectories often require heavy tilting of the magnification device. We evaluated the safety and practicality of the digital 3D exoscopes in surgery of olfactory groove meningiomas (OGM) during the transition from using a microscope to an exoscope. METHODS: In this retrospective cohort study, we included all consecutive adult patients who underwent surgery fo

Subpleural pulmonary nodule marking with patent blue V dye prior to surgical resection.

BACKGROUND AND OBJECTIVE: Subpleural located pulmonary nodules are perioperatively invisible to the surgeon. Their precise identification is conventionally possible by palpation, but often at the cost of performing a thoracotomy. The aim of the study was to evaluate the success rate and feasibility of the pre-operative CT-guided marking subpleural localized nodule using a mixture of Patent Blue V and an iodine contrast agent prior to the extra-anatomical video-assisted thoracoscopic surgery (VATS) resection in patients for whom the primary anatomical resection in terms of segmentectomy or lobe

Efficacy of Transcutaneous Electrical Acupoint Stimulation on Chronic Postsurgical Pain After Video-Assisted Thoracoscopic Lobectomy: Study Protocol for a Prospective Randomized Controlled Trial.

INTRODUCTION: Patients undergoing video-assisted thoracoscopic lobectomy (VATL) often experience chronic postsurgical pain (CPSP). Postoperative pain can affect the recovery of postoperative lung function, prolong postoperative recovery time, and increase patient hospitalization expenses. Transcutaneous electrical acupoint stimulation (TEAS) is an alternative therapy based on acupuncture that has shown promise in postoperative recovery and pain management across various medical fields. However, research specifically focused on the improvement of CPSP after VATL is currently lacking. The purpos

Thromboembolic complications after spinal surgery in trauma patients.

BACKGROUND: Deep venous thrombosis (DVT) and pulmonary embolism (PE) may be significant complications following spinal surgery. The incidence rate ranges from 0.5% to 2.5% in patients with symptomatic thromboembolic disease and up to 15% in patients with non-symptomatic thrombotic complications. We determined the incidence of symptomatic thromboembolism after spinal surgery in patients with postoperative systemic prophylaxis and investigated general and specific risk factors for development of this disease. PATIENTS AND METHODS: We analyzed the clinical records of 978 patients who had undergo

Thoracic Segmental Spinal Anesthesia for Bullectomy in a Patient With Severe Pulmonary Compromise: A Case Report.

BACKGROUND: Thoracic surgery is conventionally performed under general anesthesia with one-lung ventilation. However, this approach may pose considerable perioperative risks in selected patients with advanced pulmonary disease and significant medical comorbidities. Thoracic segmental spinal anesthesia (TSSA), combined with adjunctive regional techniques, has emerged as a potential alternative in carefully selected high-risk patients undergoing nonintubated thoracic surgery [1-3]. Association of skill and errors with outcomes in robotic rectal cancer surgery.

BACKGROUND: Proficiency-based progression is key to analyzing and improving surgical performance. Objective assessment has demonstrated a direct link between operative performance and outcomes in laparoscopic surgery but not in robotics. There is current research to automate assessment processes with sensor data and machine learning. This requires granular, reliable annotations to train clinically implementable, trusted models, to improve patient safety. AIM: To evaluate objective skill and error tools in robotic rectal cancer surgery, to provide a granular validated dataset from which to tra

Antenatal diagnosis of congenital thoracic malformations: early surgery, late surgery, or no surgery?

Today, congenital thoracic malformations (CTMs), which comprise a spectrum of anomalies rather than separate entities, are frequently detected on routine antenatal ultrasound. However, with similar appearances of different congenital lung and nonpulmonary lesions, a definitive diagnosis cannot usually be established antenatally with absolute certainty. The natural history of CTMs is extremely variable. Large lesions may cause serious complications in the fetus, necessitating treatment in utero. Such prenatal therapy, however, is only required in a small minority of fetuses. Many lesions decrea

The use of gabapentin for post-operative and post-traumatic pain in thoracic surgery patients.

OBJECTIVE: The pain following thoracic surgery and trauma is often refractory to conventional analgesic strategies. However, it shares key characteristics with neuropathic pain which gabapentin, an anticonvulsant, has been proven to effectively treat. To our knowledge, this is the first prospective study assessing the use of gabapentin in cardiothoracic surgery patients. METHODS: Gabapentin was prescribed to 60 consecutive out-patients with refractory pain persisting at four weeks or more after thoracic surgery or trauma. Follow-up of 45 patients (75%) was performed for a median of 21 months

Surgical simulation in emergency management and communication improves performance, confidence, and patient safety in medical students.

INTRODUCTION: This study aims to enhance the confidence and operational safety of 5th-year medical students in the operating room (OR), addressing their corona pandemic gap in surgical training. METHODS: We augmented the surgical curriculum focusing on pre-, intra-, and post-operative skills, centered around a phantom operation as a pre-test-retest simulation. We measured confidence to assist in surgery on a 5-level Likert-scale and monitored surgical performance metrics (skin-to-skin time, blood loss, blood and volume transfusion, complications, fatal outcome). Half the cohort was explicitly

Three-port (one incision plus two-port) endoscopic mitral valve surgery without robotic assistance.

OBJECTIVES: Totally endoscopic minimally invasive mitral valve surgery (MIMVS) is technically demanding and often performed with robotic assistance. We hypothesized that three-port video-assisted thoracic surgery (VATS) would facilitate endoscopic MIMVS and evaluated its feasibility and safety. METHODS: From October 2010 to June 2016, we performed first-time MIMVS in 250 consecutive patients (122 male), with median age of 65 years (54-73 years, 25-75 percentile). The thoracic access ports comprised one small (3-5 cm) thoracotomy without a rib spreader plus two trocars (one for the endoscope a
