A case report of a premature infant with coxsackie B1 meningitis.

This case report describes a 36-week gestational age infant diagnosed with coxsackie B1 meningitis at 20 days of age. A head ultrasound 5 days after diagnosis was consistent with cystic periventricular leukomalacia. The scientific literature does not clearly elucidate differences between bacterial and viral infections in infants. When difficulties arise, it is pertinent to consider a viral etiology for the underlying illness and obtain a detailed maternal and infant history focusing on clinical symptoms, seasonality, geographic location, exposure, and incubation period. Comparison of perforation risk among physicians, other medical professionals and general adults with acute appendicitis in Taiwan.

BACKGROUND: This study aimed to compare the risk of appendiceal perforation among physicians, other medical professionals and general adults, when hospitalized for acute appendicitis. METHODS: National population-based data for 92 143 patients with acute appendicitis, aged 20 years or more, who underwent appendicectomy or drainage of an appendiceal abscess in Taiwan from 1996 to 2001 were analysed retrospectively. The outcome measure was appendiceal perforation. Patients were categorized as physicians (Western medical doctors), other medical professionals (such as dentists or nurses) or gener

[The thrombolysis of a septic portal vein thrombosis with ultrahigh-dosage streptokinase].

Eight days after an appendicectomy a 40-year-old man developed a fever of up to 40.5 degrees C. The concentration of the transaminases was elevated (GOT 61 U/l, GPT 115 U/l, gamma-GT 226 U/l). Operative revision discovered no unusual wound conditions. Ultrasonography revealed splenomegaly (15 x 7 cm) and dilatation of the portal vein (diameter of 19 mm). The portal vein contained echo-dense constituents indicating thrombosis. Colour Doppler duplex sonography recorded only minimal residual flow around the thrombus. The fever gradually subsided under antibiotic treatment (metronidazole, 500 mg t

Management of perforated appendicitis in children--revisited.

Of 522 children with acute appendicitis treated from 1978 to 1985, 170 had appendiceal perforation with peritonitis. The protocol for perforation included aggressive fluid resuscitation, preoperative triple antibiotic therapy, copious peritoneal lavage, avoidance of transperitoneal drains except those used for well-localized abscesses, delayed wound closure, and postoperative antibiotic therapy for seven to ten days. The minor complication rate was 22%; this included pleural effusion, wound infection, atelectasis, and prolonged ileus. The major complication rate was 3%; this included intra-abd

The continuing challenge of acute and perforated appendicitis.

A two year review of 216 patients with a preoperative diagnosis of appendicitis is used to help clarify the continuing points of controversy concerning appendicitis. Morbidity of perforated appendicitis by far exceeds that of nonperforated cases. Twenty-nine per cent of patients had perforation with a postoperative complication rate of 33 per cent. Rate of wound infection after perforation was 15 per cent despite antibiotics and delayed wound closure. Pediatric patients in this study did not experience a higher incidence of perforation or complication. A decision tree for management of acute a

Interval Appendicectomy after Conservative Management of Complicated Appendicitis: Balancing Recurrence, Neoplasm Risk, and Surveillance Strategies.

BACKGROUND: Non-operative management (NOM) of complicated appendicitis is increasingly accepted, but the role of interval appendicectomy (IA) remains contentious. Contemporary evidence has shifted decision-making from recurrence risk alone toward age-linked neoplasm risk and radiological features. SUMMARY: Recurrence after successful NOM is commonly reported at 12-24% and is concentrated within the first 6 months. In adults presenting with periappendiceal abscess, appendiceal tumour prevalence rises with age, reaching approximately 5-10% in patients aged 35-39 years and 14-20% in cohorts aged

Histopathological Findings of Appendix Specimens in Quiescent Ulcerative Colitis: Correlations With Clinical Outcomes in the ACCURE Trial.

BACKGROUND: The ACCURE trial demonstrated that appendicectomy reduces relapse rates within 1 year in patients with ulcerative colitis (UC) in remission. We aimed to explore appendiceal histopathology in quiescent UC and assess its association with postoperative relapse. METHODS: Appendix specimens from Dutch participants in the ACCURE trial were reassessed by a blinded gastrointestinal pathologist using the Robarts Histopathology Index (RHI; range 0-33). Active appendiceal inflammation was defined as RHI > 3. Clinical data, preoperative endoscopic findings including peri-appendiceal red patch

Neutrophil percentage-to-albumin ratio for preoperative risk stratification of complicated appendicitis in children: a retrospective study.

OBJECTIVE: Preoperative risk assessment of complicated appendicitis in children holds substantial clinical importance. This study aimed to evaluate the association between the neutrophil percentage-to-albumin ratio (NPAR) and the occurrence of complicated appendicitis in pediatric patients. METHODS: A total of 814 pediatric patients who underwent laparoscopic appendectomy in a single-centre retrospective cohort design were enrolled in this study. The primary outcome was defined as the occurrence of complicated appendicitis. Multivariate logistic regression analysis was performed to assess the…

Impact of an appendicolith and its characteristics on the severity of acute appendicitis.

BACKGROUND: Antibiotics have been reported as an efficient and safe treatment option for uncomplicated acute appendicitis without an appendicolith diagnosed using computed tomography (CT). The aim of this study was to assess the association of a CT-diagnosed appendicolith and its characteristics with appendicitis severity. METHODS: A large prospective patient cohort with suspected acute appendicitis enrolled between April 2017 and November 2018 was retrospectively reviewed. The initial analysis evaluated the association of a CT-diagnosed appendicolith with complicated acute appendicitis; then

Appendiceal hernia: an extremely rare condition.

Appendix-associated hernias are extremely rare. They have been described sporadically in the literature, mostly as inguinal hernias. Appendix-associated incisional hernias are even more unusual. High clinical awareness is needed as complications can arise if misdiagnosis or delay occurs. We present an 80-year-old man with acute appendicitis in an incisional hernia. After successful surgery, the patient made a full recovery.

Modified Antibiotic Regimen for Pediatric Complicated Appendicitis to Reduce Infectious Morbidity in Taiwan.

We compared our previous hospital-based antibiotic protocol and an optimum modified one by reviewing hospital records of children younger than 18 years with complicated appendicitis between 2010-2016. The modified protocol showed no infectious morbidity, which is significantly different from that of our previous protocol (mortality rate, 21.4%). An optimum hospital-based antibiotic protocol for complicated appendicitis can reduce the infectious morbidity rate without increasing hospital cost.

Right Iliac Fossa Pain Treatment (RIFT) Study: protocol for an international, multicentre, prospective observational study.

INTRODUCTION: Patients presenting with right iliac fossa (RIF) pain are a common challenge for acute general surgical services. Given the range of potential pathologies, RIF pain creates diagnostic uncertainty and there is subsequent variation in investigation and management. Appendicitis is a diagnosis which must be considered in all patients with RIF pain; however, over a fifth of patients undergoing appendicectomy, in the UK, have been proven to have a histologically normal appendix (negative appendicectomy). The primary aim of this study is to determine the contemporary negative appendicec

The Diagnostic Significance of a Mucocele of the Appendix.

Appendiceal mucoceles are rare lesions with a variable clinical presentation often identified incidentally on imaging or at laparotomy/laparoscopy for an unrelated diagnosis. Mucocele of the appendix may be a benign or malignant process, making early recognition based on symptoms and key radiographic characteristics of the utmost importance for optimal patient management. Here we present the case of a patient presenting with non-specific abdominal complaints suffering from appendiceal mucocele perforation due to low-grade mucinous adenocarcinoma.
