Effectiveness of interventions targeting frequent users of emergency departments: a systematic review.

STUDY OBJECTIVE: Frequent users of emergency departments (EDs) are a relatively small group of vulnerable patients accounting for a disproportionally high number of ED visits. Our objective is to perform a systematic review of the type and effectiveness of interventions to reduce the number of ED visits by frequent users. METHODS: We searched MEDLINE, EMBASE, CINAHL, PsychINFO, the Cochrane Library, and ISI Web of Science for randomized controlled trials, nonrandomized controlled trials, interrupted time series, and controlled and noncontrolled before-and-after studies describing intervention

Simulating human space physiology with bed rest.

In a recent review on bed rest studies of the past 20 years, it was concluded that head-down bed rest has proved its usefulness as a reliable simulation model for most physiological effects of spaceflight. Much of this research has been conducted to find countermeasures against the negative effects, which are associated with gravitational unloading. There have been partial successes in the prevention of, for example muscle wasting, cardio-vascular deconditioning, adverse metabolic changes, and bone demineralization. Reviews refer to bone-related measurements of the U.S. and Russian space progr

Risk of recurrence after successful surgery for Cushing's disease and association with USP8 genotype and tumour size: an international, retrospective, longitudinal cohort study.

BACKGROUND: Recurrence after successful pituitary surgery remains a challenge in the management of patients with Cushing's disease, with no reliable predictors of long-term outcome. Pathogenic somatic USP8 variants are found in one third of cases and their association with recurrence is unclear. The aim of this study was to determine the association between USP8 status and postoperative outcome. METHODS: This international, retrospective, longitudinal study was done in eight tertiary centres in east Asia, Europe, and North America. We reviewed clinical records and genetic information of patie

Integration of HIV/AIDS services with maternal, neonatal and child health, nutrition, and family planning services.

BACKGROUND: The integration of HIV/AIDS and maternal, neonatal, child health and nutrition services (MNCHN), including family planning (FP) is recognized as a key strategy to reduce maternal and child mortality and control the HIV/AIDS epidemic. However, limited evidence exists on the effectiveness of service integration. OBJECTIVES: To evaluate the impact of integrating MNCHN-FP and HIV/AIDS services on health, behavioral, and economic outcomes and to identify research gaps. SEARCH METHODS: Using the Cochrane Collaboration's validated search strategies for identifying reports of HIV interve

Interventions for preventing dengue: a mapping review.

BACKGROUND: Dengue remains a pressing global health challenge, particularly in low- and middle-income countries (LMIC). Despite a growing volume of research on prevention strategies, there is a notable lack of consolidated, high-level research capturing the full scope of existing knowledge. The current evidence landscape is fragmented, with studies varying widely in design, focus, and quality. A structured map that includes both primary studies and systematic reviews is urgently needed. Such evidence and gap maps (EGMs) serve as a decision-support tool for funders, policymakers, and researcher

Tradeoffs between pace and reach of patient decision aids: a case study adapting a breast reconstruction decision aid from clinical implementation to social media dissemination.

BACKGROUND: Patient decision aids (PtDAs) support shared decision making and can improve patient outcomes. Nevertheless, PtDAs have not been widely adopted in healthcare settings because of numerous patient-, clinician-, and organizational-level factors. To address the slow pace of implementation and limited reach of PtDAs, we adapted a PtDA from implementation through the electronic health record in clinical settings for dissemination on social media and online communities. METHODS: We describe the adaptation of BREASTChoice, a PtDA about breast reconstruction after mastectomy, originally us

Roll-Out Implementation Optimization (ROIO) trial to develop and test the implementation of a food security screening and referral process: a study protocol.

BACKGROUND: Food insecurity (FI) is associated with poorer physical and mental health outcomes, exacerbation of chronic diseases, and decreased access to healthcare. Children experiencing FI face additional risks, including lower psychosocial functioning and reduced academic achievement. Although national initiatives call for integrating nutrition support services into healthcare, clinical workflows for FI screening and referral remain inconsistently implemented and difficult to sustain. The goal of this study is to refine, adapt, and optimize a comprehensive FI screening and referral program,

De-implementation of unnecessary antibiotic use for upper respiratory tract infections in ambulatory HIV care in Mozambique: a two-arm parallel cluster-randomized controlled hybrid type II trial.

BACKGROUND: Antibiotics are commonly overused to treat upper respiratory tract infections (URTIs) in HIV-infected adults in primary healthcare settings, even though viruses cause most URTIs. Therefore, a de-implementation of unnecessary antibiotic prescribing for URTIs was implemented in these settings. Thus, this study aimed to evaluate a strategy to de-implement unnecessary antibiotic prescriptions among ambulatory HIV-infected adults with acute URTI symptoms using the reach-effectiveness-adoption-implementation-maintenance (RE-AIM) framework. METHODS: We conducted a three-phase (pre-implem

Specifying pathway requirements for mobile outreach cancer testing before rollout: an implementation-aware Target Product Profile method.

BACKGROUND: Mobile cancer-testing services can widen access by taking testing to community or workplace sites. But the test is only one part of the pathway: eligibility explanation, informed choice, privacy, documentation, result communication, referral, and follow-up move outside clinics. Unless specified before rollout, these tasks can make a service easier to attend but harder to govern. In diagnostic-test development, a Target Product Profile is an advance brief defining a test's intended users, use setting, and minimum and preferred features. To create an equivalent brief for outreach ser

Mechanisms of practice facilitation identified using system dynamics diagramming in a tailored implementation study of unhealthy alcohol screening and treatment in primary care.

BACKGROUND: Practice facilitation is a valuable strategy for enhancing the adoption, implementation, and sustainability of evidence-based practices in primary care. It is important to understand how practice facilitation works to develop implementation strategies that are tailored to the needs of the practice. Research is needed to understand the mechanisms by which practice facilitation strategies impact clinic dynamics. METHODS: We used a diagramming approach from qualitative system dynamics to model the mechanisms by which practice facilitators supported implementation in a pragmatic study

Implementation of a hybrid lung health program for Northeast Texas: study protocol.

BACKGROUND: Lung cancer is the leading cause of cancer-related mortality in the United States. Lung cancer screening is recommended to individuals with a history of heavy cigarette smoking; it is proven to be effective in reducing lung cancer-related mortality, but the uptake remains low. In this project, we employ a hybrid model to ensure consistent screening for tobacco use and lung cancer screening eligibility, provision of tobacco cessation care, shared decision making for lung cancer screening, subsequent completion of screening, and navigation to follow-up care as needed, in Northeast Te

A practice-based implementation strategy bundle for integrating behavioral health in primary care: a matrixed multiple case study.

BACKGROUND: Integrated behavioral health (IBH) is an evidence-based approach to addressing mental health in primary care settings. It typically involves augmenting a primary care team with a behavioral health professional (e.g., psychologist, licensed clinical social worker) who sees patients for brief, focused concerns; can function as a bridge to specialty care; and provides medical providers with support and education about patient behavioral health concerns. IBH has slowly begun to disseminate across the United States, but practices encounter significant implementation barriers. How IBH is

Culture eats evidence for breakfast: how culture influences implementation of evidence-based practices.

BACKGROUND: Research has produced a substantial and expanding body of evidence-based practices (EBPs), encompassing interventions, programmes, clinical guidelines, protocols, care pathways and models of care supported by the best available evidence. Despite this, healthcare delivery is still frequently characterised as insufficiently evidence-based, reflecting a persistent gap between what is known to be effective and what is routinely implemented in practice. Traditional explanations only partially account for this gap, often overlooking culture as a critical yet under-theorized influence. Cu

Advancing digital equity: a qualitative case study using the Implementation Research Logic Model.

BACKGROUND: Older adults without digital access, who are attempting to manage their lives within a digital landscape often struggle to maintain their independence. Living on the periphery of their communities, older people without digital access often feel alone. Technology is transforming the world around them, but they are excluded from participating. The lack of digital connection to family and friends and limited availability of essential online products and services can adversely impact their well-being. The purpose of the research was to explore the implementation of the iPad Project, an

Community health worker implementation of transdiagnostic evidence-based treatment in Spanish: protocol for refinement, implementation, and a feasibility pilot RCT.

BACKGROUND: Latine people in the U.S. face unequal access to mental healthcare, particularly families and parents with limited English proficiency. Community health workers (CHWs) are well-positioned to increase access to care if integrated into mental health services. The Common Elements Treatment Approach (CETA) is a transdiagnostic evidence-based treatment (EBT) designed to be delivered by lay providers with little to no prior mental health training (i.e., CHWs). CHWs have demonstrated effectiveness in delivering CETA and other EBTs globally; however, there is limited evidence to guide dome

Implementing a primary-tertiary shared care model for enhancing cascade testing for familial hypercholesterolaemia: an effectiveness-implementation hybrid type III pre-post study.

BACKGROUND: Familial hypercholesterolaemia (FH) is a common but underdiagnosed genetic condition that leads to high low-density lipoprotein cholesterol and premature cardiovascular disease. Cascade genetic testing remains underutilised as a screening approach. Implementation strategies can increase the diffusion of innovations such as genetic medicine into non-genetic specialty settings more rapidly through dissemination networks. This study aimed to evaluate the success of a multifaceted implementation strategy to increase cascade testing for relatives of a person diagnosed with FH using a pr
