Transanal total mesorectal excision (taTME) for rectal cancer: a training pathway.

BACKGROUND: With increasing interest in natural orifice surgery, there has been a dramatic evolution of transanal and endoluminal surgical techniques. These techniques began with transanal endoluminal surgical removal of rectal masses and have progressed to transanal radical proctectomy for rectal cancer. The first transanal total mesorectal excision (taTME) was performed in 2009 by Sylla, Rattner, Delgado, and Lacy. The improved visibility and working space associated with the taTME technique is intriguing. This video manuscript outlines the training pathway followed by pioneers in the taTME

Robotic partial nephrectomy for complex renal tumors: surgical technique.

OBJECTIVES: Laparoscopic partial nephrectomy requires advanced training to accomplish tumor resection and renal reconstruction while minimizing warm ischemia times. Complex renal tumors add an additional challenge to a minimally invasive approach to nephron-sparing surgery. We describe our technique, illustrated with video, of robotic partial nephrectomy for complex renal tumors, including hilar, endophytic, and multiple tumors. METHODS: Robotic assistance was used to resect 14 tumors in eight patients (mean age: 50.3 yr; range: 30-68 yr). Three patients had hereditary kidney cancer. All pati

Focus on pleural empyema: etiology, epidemiology, infectiology, treatment, and impact of the COVID-19 pandemic-a single-center analysis.

BACKGROUND: Pleural empyema is a potentially life-threatening condition requiring timely interdisciplinary management. Based on the underlying pathophysiology, empyema is classified into three stages: exudative (stage I), fibrinopurulent (stage II), and chronic-organizing (stage III). Despite advances in surgical techniques and perioperative care, pleural empyema remains associated with considerable morbidity. Data on changes in incidence and management during the coronavirus disease 2019 (COVID-19) pandemic remain limited. This study aims to analyze etiology, epidemiology, infectiology, and s

Minimally Invasive Resection of Intralobar Pulmonary Sequestration Following Same-Day Interventional Radiological Embolisation of the Feeding Vessel: A Case Series From a Single Tertiary Centre.

Pulmonary sequestration (PS) is a congenital abnormality of the lung where a region of pulmonary parenchymal tissue is supplied by an aberrant systemic artery, which may arise from the thoracic aorta, abdominal aorta, celiac trunk, or intercostal arteries. They are further sub-classified as intralobar or extralobar, depending on whether the sequestered lung has an independent visceral pleural covering. Hybrid minimally invasive treatment of intralobar PS with preoperative embolisation of feeding vessels followed by video-assisted thoracoscopic surgery (VATS) resection has become more widesprea

Trends in surgical approach and pediatric pulmonology follow-up for operated congenital thoracic malformations.

INTRODUCTION: Congenital thoracic malformations (CTM) including congenital pulmonary airway malformation (CPAM), pulmonary sequestration (PS), bronchogenic cysts (BC), congenital lobar overinflation (CLO), and bronchial atresia (BA) are increasingly detected antenatally. Resection is generally undertaken in symptomatic patients, whereas the management of asymptomatic lesions remains controversial, with no consensus on whether or when to operate. We compared children who underwent surgery for CTMs after antenatal versus postnatal diagnosis. METHODS: Pediatric patients who underwent resection o

Pulmonary metastasectomy and survival in osteosarcoma: a systematic review and meta-analysis of surgery-related prognostic factors.

BACKGROUND: Pulmonary metastasis is a major cause of disease progression and mortality in patients with osteosarcoma. The survival impact of pulmonary metastasectomy and the prognostic relevance of surgery-related factors remain incompletely defined. This systematic review and meta-analysis aimed to evaluate the association between pulmonary metastasectomy and survival outcomes and to identify key prognostic determinants. METHODS: PubMed, Embase, Web of Science, and the Cochrane Library were systematically searched from inception to December 2025. Studies enrolling patients with histologically

Perioperative intranasal dexmedetomidine for the prevention of chronic postsurgical pain following thoracoscopic surgery: protocol for a multicentre randomised controlled trial.

INTRODUCTION: Chronic postsurgical pain (CPSP) is common among patients undergoing thoracic surgery, with an incidence of up to 43.99% even with video-assisted thoracoscopic surgery (VATS). CPSP is associated with prolonged opioid use and significantly reduced quality of life. Currently, there are no consistently effective preventive measures. Poor perioperative sleep quality has been increasingly recognised as a key contributing factor to CPSP. Therefore, improving perioperative sleep quality is a potentially modifiable factor in the prevention of CPSP. Dexmedetomidine, an α2-adrenergic recep

Descending necrotizing mediastinitis: key points to reduce the high associated mortality in a consecutive case series.

BACKGROUND: Descending necrotizing mediastinitis (DNM) is an acute life-threatening infection that originates in the oropharyngeal region. It is an uncommon disease with a mortality rate of about 20-40%. This high mortality is mainly attributed to delays in diagnosis and treatment and poor drainage of the mediastinum. We highlight key points that may help reduce mortality. CASE DESCRIPTION: We analyze a retrospective case series of seven patients diagnosed with DNM between March 2019 and July 2022 at Hospital de la Santa Creu i Sant Pau. The primary oropharyngeal infection was peritonsillar a

Indocyanine green fluorescence-navigated thoracoscopy versus traditional inflation-deflation approach in precise uniportal segmentectomy: a short-term outcome comparative study.

BACKGROUND: Uniportal video-assisted thoracoscopic surgery (VATS) segmentectomy is widely used in the field of thoracic surgery. However, anatomical variations in the bronchi and lung vessels may be critical obstacles during precise pulmonary segmentectomy. Thus, it is necessary to optimize uniportal VATS segmentectomy and to accurately identify the plane between lung segments by precisely transecting the bronchi and blood vessels of the lung segments. The indocyanine green fluorescence (ICGF)-based method has the potential to be a feasible and effective technique to facilitate the uniportal V

Robotic-Assisted Surgery Training (RAST) Program: An Educational Research Protocol.

Technology has had a dramatic impact on how diseases are diagnosed and treated. Although cut, sew, and tie remain the staples of surgical craft, new technical skills are required. While there is no replacement for live operative experience, training outside the operating room offers structured educational opportunities and stress modulation. A stepwise program for acquiring new technical skills required in robotic surgery involves three modules: ergonomic, psychomotor, and procedural. This is a prospective, educational research protocol aiming at evaluating the responsiveness of general surger

Current status and research progress of minimally invasive surgery for flail chest.

Chest trauma accounts for ~13.5% of all traumas, and direct death from chest trauma accounts for 20-25% of all traumatic deaths. Chest trauma is the second cause of death from trauma. Frequent rib fractures, especially in patients with flail chest, often cause severe pain, chest wall softening, abnormal breathing and severe lung contusion and laceration, usually requiring thoracic surgery. In recent years, the open reduction and internal fixation treatment of rib fractures with flail chest has achieved satisfactory results, and some surgical indications have reached consensus. A number of scho

Minimally invasive mediastinal surgery.

In the past, mediastinal surgery was associated with the necessity of a maximum exposure, which was accomplished through various approaches. In the early 1990s, many surgical fields, including thoracic surgery, observed the development of minimally invasive techniques. These included video-assisted thoracic surgery (VATS), which confers clear advantages over an open approach, such as less trauma, short hospital stay, increased cosmetic results and preservation of lung function. However, VATS is associated with several disadvantages. For this reason, it is not routinely performed for resection
