Novel technique to improve vessel mismatch when using saphenous vein bypass grafts for intracranial revascularization procedures.

Cerebral bypass procedures in the posterior circulation are difficult to perform and are considered to be high-risk surgery. Venous grafts, like that formed using the saphenous vein (SV), are simple to obtain without posing a high risk of morbidity. The main disadvantage of these high-flow grafts is the mismatch in vessel diameter between donor and recipient vessels in the posterior circulation. The authors performed a retrospective case study based of data from intraoperative video, patient charts, axial images, and cerebral angiograms. They treated a 66-year-old man who presented with a gian

The clinical features of adult unilateral moyamoya disease: does it have the same clinical characteristics as typical moyamoya disease?

BACKGROUND: The cerebral hemodynamics in unilateral moyamoya disease have not been clarified. The present study was done to clarify the clinical characteristics of patients with unilateral moyamoya disease compared to those with typical moyamoya disease or occlusive lesions of the internal carotid artery, terminal portion or middle cerebral artery. METHODS: We retrospectively analyzed patients who had unilateral or bilateral steno-occlusive lesions at the internal carotid artery terminal portion or proximal middle cerebral artery. We divided the patients into 3 groups: typical moyamoya (n = 2

Moyamoya syndrome after cranial irradiation for bone marrow transplantation in a patient with acute leukemia.

It is well known that radiation-induced vasculopathy and arteritis are two of the complications of whole brain radiation therapy. Moyamoya syndromes after cranial irradiation among patients with brain tumors were previously reported. However, we could find only three cases of prophylactic cranial irradiation for hematological disorders and no case of cranial irradiation before bone marrow transplantation in patients with acute leukemia. We recently treated a boy who developed moyamoya vessels 1.5 years after cranial irradiation for bone marrow transplantation for acute leukemia. This is the fi

Breath-hold 3D STAR MR angiography of the renal arteries using segmented echo planar imaging.

Three-dimensional (3D), cardiac triggered renal MR angiograms were acquired with excellent background suppression within a single breath-hold of 18 to 30 s using signal targeting with alternating radiofrequency (STAR), a subtraction time-of-flight MR angiography technique, and a 3D scheme combining echo planar imaging (EPI) readouts and k-space segmentation. The 3D STAR sequence was evaluated on 17 healthy individuals, 3 potential renal donors, and 2 patients with suspected renovascular hypertension. An inversion tag through the aorta was applied to produce the vascular contrast. After a suita

Cerebral No-Reflow after Thrombectomy: International Delphi Consensus on Definition and Imaging Assessment.

BACKGROUND: The operational definition and imaging assessment of the no-reflow phenomenon, a potential contributor to poor clinical outcome despite successful thrombectomy, remains debated. We aimed to develop expert-based international consensus on the definition, assessment, and reporting standard of cerebral no-reflow post-thrombectomy. METHODS: Three-rounds of a modified Delphi web-based survey was conducted among corresponding authors of peer-reviewed clinical studies on cerebral no-reflow between 2020 and 2025. Responses at each round were used to develop structured questionnaires for s

Parametric color-coding-derived microvascular transit time may predict infarction and reveals microcirculatory benefits of Tenecteplase in acute ischemic stroke.

BACKGROUND: Despite successful macrovascular recanalization a subset of patients with a large vessel occlusion (LVO) have persisting microvascular occlusion, known as the" no-reflow-phenomenon." In this study, we evaluated whether microvascular transit time (mTT), derived from parametric color-coding (PCC) of digital subtraction angiography (DSA) images, can serve as a tool to quantify impaired cerebral microcirculation and to assess its association with infarction. In a second step, we evaluated whether Tenecteplase (TNK), the new primary thrombolytic agent, improves microcirculatory flow com

Donation After Brain Death: Are There Any Organizational Difficulties?

OBJECTIVES: In the past 3 years, nearly 90 % of organ transplants performed in our country have been from living donors. Low rates of organ donation and family consent in cases of brain death reduce the source of deceased donors. Here, we reported the challenges of determination of brain death and donor care processes in our hospital and offered our recommendations to strengthen these services. MATERIALS AND METHODS: We retrospectively analyzed adult patients (n = 5845 )treated in adult intensive care units of Manisa City Hospital from June 1, 2024, through November 30, 2025. Demographic info

Leptomeningeal Collaterals Constitute an Immune-Responsive Vascular Niche in Ischemic Stroke.

BACKGROUND: Leptomeningeal collaterals form a critical vascular network that enlarges to support retrograde reperfusion after ischemic stroke, yet the cellular mechanisms governing their structural plasticity remain poorly defined. Here we identify an immune-responsive vascular niche and uncover a central role for bone marrow-derived monocytes in regulating collateral remodeling. METHODS: GFP (green fluorescent protein)+ bone marrow chimeric mice and inducible monocyte-specific Ccr2-CreERT2/EphA4f/f and Ccr2-CreERT2/EphA4f/f/Tie2f/f mice underwent permanent middle cerebral artery occlusion to

Combined interventional and craniotomy surgery for meningioma apoplexy complicated by acute subdural hematoma: a case report and literature review.

BACKGROUND: Spontaneous hemorrhage from meningiomas causing intracranial hematomas is rare (1%-2% of cases), and concurrent acute subdural hematoma (ASDH) is even rarer, which is easily misdiagnosed and may lead to inadequate preoperative planning. Combined endovascular-craniotomy hybrid surgery enables precise diagnosis and treatment, serving as an effective approach for complex intracranial lesions. CASE PRESENTATION: A 34-year-old married female was admitted with a 4-day history of sudden headache. Cranial CT showed left temporal lobe hemorrhage and left frontoparietotemporal-occipital ASD

Ruptured A3 segment aneurysm in a patient with neurofibromatosis type 1: a case report and literature review.

BACKGROUND: Neurofibromatosis type 1 (NF1) is a genetic disorder occasionally complicated by vasculopathy, but the concurrence of NF1, a ruptured distal anterior cerebral artery (ACA) aneurysm, and moyamoya syndrome is exceedingly rare. Literature review revealed that fewer than 20 such cases have been reported in English publications to date. The management of such complex, multifocal cerebrovascular pathology-including the choice of therapy, the risk of procedural complications, and the need for lifelong surveillance-poses significant challenges. CASE DESCRIPTION: A 44-year-old man with cli

Subtonsillar Approach to a Polylobulated Posterior Inferior Cerebellar Artery Aneurysm: A Case Illustration.

BACKGROUND AND IMPORTANCE: Dysplastic and polylobulated aneurysms of the posterior inferior cerebellar artery (PICA) present significant technical challenges because of their irregular geometry and proximity to the lower cranial nerves and brainstem perforators. When endovascular options carry elevated risks, microsurgical clip reconstruction remains a definitive treatment to restore the vascular architecture. CLINICAL PRESENTATION: A 46-year-old woman presented for the elective surgical management of an unruptured, polylobulated left PICA aneurysm. She had a history of subarachnoid hemorrhag

Maximizing safety in the treatment of a large choroidal fissure arteriovenous malformation: The essential role of anatomy, staged embolization, and microsurgical technique.

BACKGROUND: Arteriovenous malformations (AVMs) are complex congenital pathologies that require a deep understanding of their angioarchitecture and the involved brain anatomy. The ones within the temporal horn of the lateral ventricle present significant challenges for microsurgical treatment due to their deep location and proximity to critical neurovascular structures. These rare AVMs typically manifest with seizures or hemorrhage. They have afferents from the choroidal arteries, with arterial supply often arising from both the anterior and posterior circulations through the choroidal arteries
