Abnormal findings associated with undescended testis in Nigerian children.

Children between the ages of 6 years and 12 years numbering 2200, were examined for general physical development and external abnormality to correlate these anomalies with frequency of undescended testis. Whereas the overall frequency of undescended testis in this population was 0.5%, this condition was present in 1% of boys with peri-umbilical hernia. Undescended testis should be looked for in children with external congenital anomalies, particularly of the anterior abdominal wall.

The United States and the international control of AIDS.

Before the end of 1988, 300,000 cases of acquired immune deficiency syndrome (AIDS) are expected to have been reported worldwide since 1981. Three epidemiological patterns have emerged. One, in the Americas and Europe, is where transmission is primarily among homosexual men and intravenous drug users. In Africa, the virus primarily is spread heterosexually. A third pattern, in Asia, the Middle East, and Eastern Europe, is manifested by low prevalence but transmission through homosexual and heterosexual contact and contaminated needles. AIDS cases will increase in number dramatically over the n

[Diphtheria, an imported disease].

A comparative analysis of 4256 persons living in the Federal Republic of Germany and 4751 applicants for asylum in the FRG allows the conclusion that in the FRG diphtheria is caused by imported toxinogenic strains, originating mostly from developing countries. Since 1975 a total of 109 cases have been observed, 22 ending in death. Such severe cases can be avoided by systematic revaccination even of adults.

Cross-cultural aspects of the somatization trait.

Studies using a variety of diagnostic rating scales have identified much higher levels of somatic symptoms in Hispanic psychiatric patients, particularly those with a diagnosis of schizophrenia or depression, than in their Anglo counterparts. However, this cultural difference cannot be generally demonstrated by using DSM-III somatoform disorders criteria. Many patients with "unfounded" somatic symptoms have another DSM-III primary diagnosis that is often assumed to cause or explain the somatic symptoms. In addition, the true prevalence of unfounded somatic symptoms is hidden because of the str

[Health advice before travel to tropical countries].

Some 8% of Swiss citizens travel in the developing countries annually. Advising these travellers is an important task for the physician, though the most frequent infections are the less serious travellers' diarrhea and common colds. To some extent the more serious tropical diseases can be avoided by appropriate measures, e.g. vaccination (against yellow fever, tetanus, polio etc). and by chemoprophylaxis (malaria). Individual fitness for travel should be checked prior to any journey abroad, and special advice must be given to travellers with special risks (e.g. pregnant women, infants, persons…

The popularity of injections in the Third World: origins and consequences for poliomyelitis.

Paralysis from poliomyelitis may follow injections yet injections are extremely popular in the Third World. Some injections are given by hospital doctors and nurses but the majority are given by traditional healers, pharmacists and paramedical workers who have acquired syringes. Many injections may be given to a sick child. I suggest that the early use of vaccines did not persuade people of the mystic of injections and that the mystic predated the use of penicillin. The earliest mystical result would have been the injection of quinine for malaria and antrypal for sleeping sickness. The words b

Pigbel in Papua New Guinea: an ancient disease rediscovered.

A hypothesis suggested in this paper is that pigbel, or enteritis necroticans was a common disease in mediaeval Europe when human habitats, food hygiene, protein deficiency and periodic meat feasting formed the basics of village life as they do in many Third World cultures today. Based on the Papua New Guinea experience with pigbel, it is suggested that health authorities should look closely at the epidemiology of the acute surgical abdomen in such communities. Enteritis necroticans may be the important predisposing lesion to mid-gut volvulus, jejunal and ileal ileus and other forms of small b

Selective primary health care: strategies for control of disease in the developing world. VIII. African trypanosomiasis.

African trypanosomiasis in humans is caused by trypanosomes, parasitic protozoa that inhabit the blood and tissue spaces. Humans are infected by the bite of the tsetse (Glossina species), which itself acquires the parasites from human or animal hosts. The vector and the disease are confined to sub-saharan Africa where the disease occurs in endemic foci from which epidemics arise. Two forms of African trypanosomiasis in humans are recognized and both are fatal if left untreated. Control strategy is dependent on the capacity of the public health services to diagnose and treat cases, and the main

Xerophthalmia in the state of Paraiba, northeast of Brazil: clinical findings.

An epidemiological survey of the prevalence of xerophthalmia was conducted in the state of Paraiba through 10,922 clinical examinations of 0 to 12 yr old children from six localities representative of the three different ecological regions of Paraiba. The regional distribution of clinical signs of vitamin A deficiency was markedly different, and in the semiarid region (Sertão) the prevalence reached levels suggesting that it was a public health problem in that area. Seasonal variations were observed and were specially significant in the "Sertão," with xerophthalmia signs appearing more frequen

[Diseases transmitted by mosquitoes and urbanization. Examples of urban vectors of dengue and filariasis].

Once located in time and place the beginning of urbanization process, the mechanisms of adaptation to urban ecosystem and of dissemination by human conveyances are studied for two domestic mosquitoes: Aedes aegypti and Culex pipiens fatigans (= quinque-fasciatus). Then epidemiological consequences are discussed. Ae. aegypti pullulation constitutes a potential risk of urban yellow fever outbreak and the main factor of dengue haemorrhagic fever appearance; the increase of C. p. fatigans populations involves a slow rise of bancroftian filariasis. Future prospects for these diseases are discussed

The neurology of poverty.

An intellectual deficit is known to exist in populations where extreme poverty is rife and is thus seen extensively in the lower socio-economic strata of underdeveloped nations. Poverty is a complex entity whose sociological and economic indicators often bear little relevance to the biological agents which can affect the central nervous system. An attempt is made to express poverty in terms of identifiable defects, physiological in nature. Thus adverse socio-economic factors are converted into specific biological entities which, though necessary for adequate development of the brain, are restr
