Large coronal shear fractures of the capitellum and trochlea treated with headless compression screws.

BACKGROUND: The purpose of this study is to retrospectively evaluate the clinical outcomes of 18 patients with large coronal shear fractures of the capitellum and lateral trochlea that underwent open reduction and internal fixation with headless compression screws. METHODS: Eighteen patients were identified (16 women, 2 men) with an average age of 45 years and an average follow-up of 26 months. Fractures were classified according to the Dubberley classification as 11 type-1A injuries and 7 type-2A injuries. RESULTS: All patients, with the exception of 1, had good to excellent functional resu

Soft-tissue coverage of the elbow: an outcome analysis and reconstructive algorithm.

BACKGROUND: Soft-tissue defects surrounding the elbow can be a challenging problem for the reconstructive surgeon. Multiple reconstructive options are available, but there are few published outcome studies. The authors performed an outcome analysis of soft-tissue coverage for elbow defects to determine the benefits and limitations of various reconstructive options in this problematic area. METHODS: A retrospective review was performed of all elbow defects requiring flap coverage from 1988 to 2005. Patient demographics, defect characteristics, type of flaps used, complications, and long-term o

Endoscopic Repair of Acute and Chronic Retracted Distal Biceps Ruptures.

Distal biceps tendon (DBT) ruptures are infrequent injuries that result in pain, weakness, and cosmetic deformity. Severe retraction of the ruptured DBT can occur at the time of injury, or in chronic neglected ruptures, and surgical exposure is performed using a single incision or a 2-incision technique. The technique presented here describes an endoscopic approach using 3 portals that provide access to the retracted DBT, biceps sheath, and radial tuberosity. Preoperative sonographic localization of the retracted DBT and neurovascular structures is used to guide portal placement. The parabicep

Documentation of neurovascular status in supracondylar fractures and the development of an assessment proforma.

BACKGROUND: Supracondylar fractures are associated with a high incidence of neurovascular complications. Comprehensive clinical evaluation is a necessity when children with these injuries present to the emergency department. Neurovascular assessment can be difficult due to pain, anxiety and the young age of these patients; however, it is crucial findings are well documented to identify patients requiring urgent surgical intervention, in addition to allowing the neurovascular status to be monitored over time. The aim of this study was to evaluate the preoperative neurovascular documentation in

Exploratory analysis of relationship between trunk and pelvic motion and shoulder motion during throwing in quarterbacks in American football.

BACKGROUND: American football quarterbacks (QBs) often sustain shoulder and elbow injuries from throwing. During the late cocking phase of pitching, decreased shoulder abduction and horizontal adduction angles at shoulder maximum external rotation (MER) contribute to these injuries. The relationship between incorrect shoulder positions at MER and the trunk and pelvic rotation angles at stride foot contact (SFC) during the early cocking phase has been analyzed. However, most studies focus on baseball pitchers rather than QBs. The relationship between shoulder kinematics at MER and trunk and pel…

Increased Incidence of Upper Extremity Soft Tissue Injuries and Orthopaedic Surgeries in Patients with Eating Disorders.

BACKGROUND: Despite an established increased fracture risk in eating disorder patients, no studies, to our knowledge, have investigated the association between eating disorders and upper extremity soft tissue injury or surgery incidence. Given the association of eating disorders with nutritional deficiency and musculoskeletal sequelae, we hypothesized that patients with eating disorders would have an increased risk of soft tissue injury and surgery. The aim of this study was to elucidate this link and investigate if these incidences are increased in patients with eating disorders. METHODS: Co

Clinical and Functional Outcomes After Operative and Nonoperative Treatment of Distal Biceps Brachii Tendon Ruptures in a Consecutive Case Series.

BACKGROUND: Patients with ruptures of the distal biceps brachii tendon (DBBT) have traditionally been treated via surgical repair, despite limited patient data on nonoperative management. PURPOSE/HYPOTHESIS: To determine the clinical and functional outcomes for patients with partial and complete DBBT injuries treated nonoperatively or surgically through an anatomic single-incision technique. We hypothesized that there would be no difference in outcomes in patients treated with nonoperative or operative management. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: A retrospective ch

Electrical fascicle identification as an adjunct to nerve repair.

In view of the tragic consequences of median nerve and/or ulnar nerve paralysis and the biologic problems inherent in nerve healing, it seems logical that the surgeon should make a serious effort to control those problems that lie within his or her power: to identify and align those principal sensory and motor pathways in the severed nerve ends. I find that this is feasible. For over 15 years, I have used the electrical fascicle identification method, repairing 49 major upper extremity nerves. (A single radial nerve case was lost to follow-up.) As with any new technique, problems have been ide

Does intraoperative fluoroscopic 3D imaging provide extra information for fracture surgery?

Fracture surgery of the extremities using 2D fluoroscopy frequently fails to detect the suboptimal positioning of implants and joint incongruities. The use of intraoperative 3D-rotational X-ray (3D-RX) imaging with a new X-ray device potentially reveals these failures. We compared 50 intraoperative (2D) results of surgery and certainty about the effectiveness of different aspects of fracture reduction as interpreted from conventional (2D) methods versus intraoperative 3D-RX in 42 distal extremity fractures by means of a surgery questionnaire. In addition, we investigated the need for revision

Clinical evaluation of double-plate osteosynthesis for olecranon fractures: A retrospective case-control study.

INTRODUCTION: Single, dorsal plating is a commonly used technique for treating olecranon fractures. Double-plate osteosynthesis is an alternative treatment. Aim of this study was to present the surgical technique using this novel double-plate implant for olecranon fractures and review clinical results, complication rates and revision surgeries. Results were compared to single, dorsal plating. HYPOTHESIS: Does double-plate osteosynthesis for olecranon fractures improve material's tolerance with respect to osteosynthesis by single dorsal plating? PATIENTS AND METHODS: Between February 2011 and

Closed Reduction and Percutaneous Pinning Versus Open Reduction and Internal Fixation for Type II Lateral Condyle Humerus Fractures in Children Displaced >2 mm.

BACKGROUND: The optimal treatment for pediatric lateral condyle fractures displaced >2 mm, but with little joint displacement is unclear. The purpose of this study was to assess clinical and radiographic outcomes and complication rates of patients undergoing open reduction internal fixation (ORIF) versus closed reduction and percutaneous pin fixation (CRPP) of lateral condyle fractures with >2 mm of displacement and no obvious articular surface incongruity. METHODS: A retrospective review of all children with acute lateral condyle fractures with 2.1 to 5.0 mm of displacement treated from 2006
