A Case Report of General Anesthesia for Abdominal Surgery in Severe Cavitary Tuberculosis: Strategies to Minimize Barotrauma.

A 37-year-old man with suspected tuberculosis and severe cavitary lung disease presented with an acute abdomen with pneumoperitoneum requiring emergent exploratory laparotomy. The patient presented with dysphagia, recurrent regurgitation, pancytopenia, treatment refractory elevated International Normalized Ratio (INR). His compromised respiratory status, likely to worsen under positive pressure ventilation, along with a high risk of aspiration of gastric content and coagulopathy during emergent abdominal surgery, required general anesthesia. In patients with extensive pulmonary tuberculosis an

Artificial Intelligence in Anesthesia: Enhancing Precision, Safety, and Global Access Through Data-Driven Systems.

Artificial intelligence (AI) enhances anesthesiology by introducing adaptive systems that improve clinical precision, safety, and responsiveness. This review examines the integration of AI in anesthetic practice, with a focus on closed-loop systems that exemplify autonomous control. These platforms integrate continuous physiologic inputs, such as BIS, EEG, heart rate, and blood pressure, to titrate anesthetic agents in real time, providing more consistent and responsive management than manual methods. Predictive algorithms reduce intraoperative hypotension by up to 40%, and systems such as McS

The effects of ciprofol and propofol on hypoxemia during gastrointestinal endoscopy in overweight and obese patients: a randomized controlled trial.

BACKGROUND: Overweight and obesity are prevalent modern health issues. It has been suggested that individuals who are overweight or obese are more likely to experience hypoxemia during gastrointestinal endoscopy. Ciprofol has demonstrated advantages over propofol in endoscopic procedures, including lower rates of respiratory depression among normal-weight populations. This study aimed to compare the incidence of hypoxemia between ciprofol and propofol in overweight and obese patients undergoing gastrointestinal endoscopy. METHODS: A total of 176 patients were randomly assigned to receive eith

Efficacy and safety of ciprofol for sedation/anesthesia in patients undergoing hysteroscopy: a prospective, randomized, non-inferiority trial.

INTRODUCTION: Propofol is the preferred sedative for painless hysteroscopy and other procedures due to its fast onset and short duration. However, its limitations, including injection pain, respiratory depression, blood pressure decline, and bradycardia, cannot be disregarded. Ciprofol, a new short-acting gamma-aminobutyric acid receptor agonist, has shown effectiveness and safety in painless gastrointestinal endoscopy. This study aims to demonstrate that ciprofol is not inferior to propofol in terms of sedation efficacy for painless hysteroscopy. PATIENTS AND METHODS: A randomized study was

Vatinoxan increases microcirculation in dogs anaesthetised with medetomidine-methadone-propofol-isoflurane for prescrotal castration - A randomized, clinical study.

We investigated the impact of vatinoxan, a peripherally selective alpha2-adrenoceptor antagonist, on oral mucosal and testicular surface microcirculation in dogs anaesthetised for prescrotal castration. Seventeen healthy, privately-owned dogs were randomly allocated to receive one of two intramuscular treatments: medetomidine 0.3 mg/m2 + methadone 0.3 mg/kg (MM) or MM + vatinoxan 6 mg/m2 (ZM). The dogs were then anaesthetised with propofol and isoflurane. Heart rates (HR) and mean arterial pressure (MAP) were recorded and microcirculatory variables quantified with real-time hybrid technology c

Ultrasound-guided caudal quadratus lumborum block combined with the greater ischiatic notch plane block in a goat with femoral head fracture.

This report describes the case of a brown-black coloured mountain goat (Caprinae), aged 13 months and weighing 46 kg, which was presented for anaesthesia to facilitate surgical repair of a femoral head fracture in the left pelvic limb. Clinical evaluation was unremarkable except for marked lameness (5/5). After sedation, general anaesthesia was induced for open reduction and internal fixation of the fracture using pins. After aseptic preparation, a 'GIN & TONIC' block, comprising the combination of an ultrasound-guided greater ischiatic notch (GIN) plane block (bupivacaine 0.25%; 0.25 mL kg-1)

Management of maxillary sinus aspergillosis followed by implant-prosthetic rehabilitation: A retrospective study of 11 consecutively treated patients with a follow-up period of 3 to 10 years.

PURPOSE: Aspergillosis is a common cause of fungal maxillary sinusitis that is often asymptomatic and detected only incidentally on imaging. It may be triggered by spores entering the maxillary sinus through the natural ostium, often following the dislocation of foreign bodies, for example root canal filling or augmentation materials. This retrospective study aimed to evaluate a standardised treatment for maxillary sinus aspergillosis followed by implant-prosthetic rehabilitation. MATERIALS AND METHODS: Eleven immunocompetent patients with maxillary sinus aspergillosis were included in the pr

Myoepithelial carcinoma of submaxillary gland: A case report.

Myoepithelial carcinoma (MEC) is a malignant tumour composed almost entirely of cells differentiated from the myoepithelium. It is rare, most commonly occurring in the parotid gland. In the current study, a rare case of MEC in the submandibular gland was presented. An 82-year-old male patient was admitted with a swelling in the upper left neck that had been present for 60 years. Magnetic resonance imaging indicated a tumour in the region of the left submandibular gland, which was not easy to clearly distinguish from the gland itself and an enhancement scan was recommended but not performed. Anesthesia management for percutaneous mitral valve repair in a patient with mitochondrial cardiomyopathy and low cardiac function: a case report.

BACKGROUND: Mitochondrial cardiomyopathy occurs when impaired mitochondrial energy production leads to myocardial dysfunction. Anesthetic management in such cases is challenging due to risks of circulatory depression associated with anesthesia and mitochondrial dysfunction induced by anesthetics. Although there are reports of anesthetic management for patients with mitochondrial diseases, there are few reports specifically addressing cardiac anesthesia for patients with mitochondrial cardiomyopathy. We present a case where percutaneous mitral valve repair with MitraClip™ was successfully perfo

High-Dose Remifentanil Anesthesia With Minimal Sedation in Neonates: A Single-Center Retrospective Study.

Introduction Remifentanil is an opioid with rapid onset and elimination. Theoretically, reducing sedation using high-dose remifentanil may contribute to early emergence and prevention of postanesthetic complications related to residual anesthesia. However, there have been few reports of high-dose remifentanil anesthesia in neonatal surgery. This study aims to describe the techniques of high-dose remifentanil anesthesia in neonates and their safety outcomes. Methods This is a single-center, retrospective observational study from January 2016 to February 2022. Medical records from neonatal surgi

Intraspinal Pressure is Not Elevated After Traumatic Spinal Cord Injury in a Porcine Model Sham-Controlled Trial.

BACKGROUND: It has been suggested that intraspinal pressure (ISP) below the dura is elevated following traumatic spinal cord injury (TSCI). The dura can maintain the pressure, and hence a subdural compartment syndrome has been hypothesized to develop regardless of bony decompression. This study aimed to evaluate whether a such intradural compartment syndrome develops during the first 72 h in a porcine TSCI model. METHODS: First, in a randomized sham controlled-trial design, longitudinal ISP measurements were performed over a period of 72 h from onset of TSCI. TSCI was inflicted by a weight-dr

Functional ultrasound imaging of stroke in awake rats.

Anesthesia is a major confounding factor in preclinical stroke research as stroke rarely occurs in sedated patients. Moreover, anesthesia affects both brain functions and the stroke outcome acting as neurotoxic or protective agents. So far, no approaches were well suited to induce stroke while imaging hemodynamics along with simultaneous large-scale recording of brain functions in awake animals. For this reason, the first critical hours following the stroke insult and associated functional alteration remain poorly understood. Here, we present a strategy to investigate both stroke hemodynamics

Evidence-based analysis of multi-pronged approaches for education and behavior management of autistic patients in a dental setting.

BACKGROUND: Autism spectrum disorder is a neurodevelopmental condition which has posed a challenge to carers, pediatricians and dentists through the years. Over the last four decades, the disorder has been better understood and management techniques have evolved. In the field of dentistry, the autistic child presents difficulty in management, even for the simplest dental procedures. A comprehensive understanding of other psychoeducational and behavioral approaches to manage autism is very important. This article aims to introduce various important key techniques such as Applied Behavior Analys
