Mortality prediction in the elderly patients with coronary artery disease and atrial fibrillation: a retrospective machine learning approach.

BACKGROUND: Elderly patients with coexisting coronary artery disease (CAD) and atrial fibrillation (AF) are at significantly increased risk of mortality. Accurate risk stratification is crucial for improving clinical management, yet a dedicated predictive tool for this specific population is lacking. The widely used CHA2DS2-VASc score demonstrates limited performance in predicting all-cause mortality in this complex comorbid group. METHODS: A cohort of elderly inpatients (≥65 years) diagnosed with CAD and AF were retrospectively enrolled from the Department of Cardiology at the Chinese PLA Ge

Cumulative risks for reoperation due to bleeding after carotid endarterectomy and the associated clinical impact of bleeding events.

OBJECTIVE: The purpose of this study was to identify all preoperative and intraoperative variables in the Vascular Quality Initiative (VQI) carotid endarterectomy (CEA) module that have a statistically significant association with reoperation for bleeding. A weighted risk score was developed and validated to predict this event, with assessment of its impact on 30-day mortality and other adverse perioperative events. METHODS: The VQI CEA module was queried between January 2003 and October 2023. Overall, 192,547 CEA procedures met study inclusion. An internal VQI validation cohort was created w

Preoperative Evaluation and Midterm Outcomes after the Surgical Correction of Anomalous Origin of the Left Coronary Artery from the Pulmonary Artery in 50 Infants and Children.

BACKGROUND: Anomalous origin of the left coronary artery (LCA) from the pulmonary artery (ALCAPA) is a rather rare congenital anomaly that has a profound effect on heart function. This study aimed to retrospectively illustrate the perioperative clinical features, therapy experience, and midterm outcomes after surgical correction, and to determine the value of left ventricular ejection fraction (LVEF) and myocardial viability in differentiating critically ill patients among infants and children with ALCAPA. METHODS: From April 1999 to March 2013, infants and children patients diagnosed with AL

[The single-stage surgery of colorectal neoplastic occlusion. The experience of 133 cases].

BACKGROUND AND AIM: 133 cases of occluded colorectal neoplasms (14% of the entire series): 30 (23%) of the right colon, 103 (77%) of the left colon-rectum; 69 males (52%) and 64 females (48%); mean age 67.5 years old, range 33-91 years. pTNM: stage II, 28 cases (21%); stage III: 43 cases (32%); stage IV, 62 cases (47%). The aim of this study was to resolve the occlusive symptoms and to treat the neoplasm in a single operation. METHODS: In the 62 cases at stage IV, surgery was solely palliative: 49 (79%) derivations, 13 (21%) entero-enterostomies and 36 (58%) preternatural anus; 11 (18%) stand

Risk Assessment of Acute Myocardial Infarction and Stroke Associated with Long-Acting Muscarinic Antagonists, Alone or in Combination, versus Long-Acting beta2-Agonists.

BACKGROUND: The long-acting muscarinic antagonist (LAMA) aclidinium was approved in Europe in 2012 to relieve symptoms in adult patients with chronic obstructive pulmonary disease (COPD). A post-authorization safety study was initiated to assess potential cardiovascular risks associated with LAMAs versus long-acting beta2-agonists. PURPOSE: To estimate incidence rates and adjusted incidence rate ratios (IRRs) for acute myocardial infarction (AMI), stroke, and major adverse cardiac events (MACE) in new users of aclidinium, aclidinium/formoterol, tiotropium, other LAMA, long-acting beta-agonist

[Rib fixation for flail chest after resuscitation].

BACKGROUND: Rib fractures are a common complication caused by chest compressions during resuscitation. Flail chest may occur as a consequence, leading to respiratory failure. CASE DESCRIPTION: We present two cases in which surgical rib fixation was performed to treat flail chest after resuscitation. CONCLUSION: Based on a literature search, surgical rib fixation may be considered for flail chest after resuscitation in carefully selected patients.

A new intercostal artery management strategy for thoracoabdominal aortic aneurysm repair.

OBJECTIVE: The purpose of this study is to describe a new approach for addressing the intraoperative management of intercostal arteries during thoracoabdominal aortic aneurysm (TAAA) repair, using preoperative spinal MRA for detection of intercostal arteries supplying the anterior spinal artery. METHODS: Patients undergoing TAAA repair from August 2005 to September 2007 were included. Spinal artery MRA was performed to identify the anterior spinal artery, the artery of Adamkiewicz, and its major intercostal source artery (SA-AAK). Intraoperative spinal cord protection was carried out using st

Brain abscess and Rendu-Osler-Weber disease. Case report and review of the literature.

The case of a 51-year-old man presented with intermittent diplopia, headache and family history of Rendu-Olser-Weber disease , also defined heredirary hemorrhagic telangiectasia (HHT), is described. Magnetic resonance imaging of the brain revealed an enhancing right occipital mass with surrounding edema. Computerized tomography of the chest identified pulmonary arteriovenous fistlae. The physiopathology of HHT and the treatment of the cerebral lesions are discussed.

Randomised controlled comparison of continuous positive airways pressure, bilevel non-invasive ventilation, and standard treatment in emergency department patients with acute cardiogenic pulmonary oedema.

BACKGROUND: Continuous positive airways pressure (CPAP) and bilevel non-invasive ventilation may have beneficial effects in the treatment of patients with acute cardiogenic pulmonary oedema. The efficacy of both treatments was assessed in the UK emergency department setting, in a randomised comparison with standard oxygen therapy. METHODS: Sixty patients presenting with acidotic (pH<7.35) acute, cardiogenic pulmonary oedema, were randomly assigned conventional oxygen therapy, CPAP (10 cm H(2)O), or bilevel ventilation (IPAP 15 cm H(2)O, EPAP 5 cm H(2)O) provided by a standard ventilator throu

Predictors of severe morbidity and death after elective abdominal aortic aneurysmectomy in patients with chronic obstructive pulmonary disease.

OBJECTIVE: This study sought to identify risk factors associated with an unfavorable outcome after elective abdominal aortic aneurysm (AAA) repair in patients with chronic obstructive pulmonary disease (COPD). METHODS: The clinical records of 158 patients who underwent elective open AAA repair with COPD determined from preadmission International Classification of Diseases-ninth revision codes during a 12-year period at the University of Michigan were reviewed. Patients with uncomplicated outcomes (group I) were compared with those with unfavorable postoperative outcomes (group II). The unfavo

Decreased glucose in RA-cell-positive pleural effusion: correlation of pleural glucose, lactic dehydrogenase and protein concentration to the presence of RA-cells.

A significant correlation between RA-cells in the pleural fluid and lowered concentration of glucose, increased LDH activity and protein content was demonstrated in pleural effusions of diverse etiology. The finding might suggest a common metabolic reaction of the pleura, the mechanism of which remains unexplained at present.
