Rhetorical strategies used in the reporting of implantable defibrillator primary prevention trials.

Previous studies have suggest that the authors of randomized trial reports frequently use rhetorical strategies, such as framing, the use of ratios rather than absolute values to report results, and underreporting of complications, to convince readers of treatment efficacy. The objective of this study was to determine if such rhetorical strategies were used in the publication of implantable-cardioverter defibrillator (ICD) primary prevention trials. Medline and the Cochrane Central Register of Controlled Trials were searched for all publications that described ICD primary prevention trials and

Acupuncture for treating dry eye: a systematic review.

PURPOSE: The objective of this review was to assess the effectiveness of acupuncture as a treatment option for treating the condition of dry eye. METHODS: We searched the literature using 14 databases from their inceptions to 3 December 2009, without language restrictions. We included randomized clinical trials (RCTs) comparing acupuncture with conventional treatment. Their risk of bias was assessed using Cochrane criteria. RESULTS: Six RCTs met all the inclusion criteria. Three RCTs compared the effects of acupuncture with artificial tears in patients with xerophthalmia or Sjögren syndrome.

Interventions for visual field defects in patients with stroke.

BACKGROUND: Visual field defects are estimated to affect 20% to 57% of people who have had a stroke. Visual field defects can affect functional ability in activities of daily living (commonly affecting mobility, reading and driving), quality of life, ability to participate in rehabilitation, and depression, anxiety and social isolation following stroke. There are many interventions for visual field defects, which are proposed to work by restoring the visual field (restitution); compensating for the visual field defect by changing behaviour or activity (compensation); substituting for the visua

Sutures versus staples for the management of surgical wounds: a meta-analysis of randomized controlled trials.

Surgical sutures are conventionally used in skin closure of surgical wounds. Alternative wound closure techniques include staples and adhesive strips. We aimed to evaluate sutures versus staples as methods of surgical wound closure by performing a meta-analysis. We searched PubMed, Scopus, and Cochrane Central Register of Controlled Trials for randomized controlled trials that compared sutures with staples for surgical wound closure. Trials referring to orthopedic operations were excluded. Twenty studies (involving a total of 2111 patients) were included. Five studies referred to obstetrics/gy

Surgical treatment of gastroesophageal reflux disease: total or partial fundoplication? systematic review and meta-analysis.

CONTEXT: Although the high incidence of gastroesophageal reflux disease (GERD) in the population, there is much controversy in this topic, especially in the surgical treatment. The decision to use of a total or partial fundoplication in the treatment of GERD is still a challenge to many surgeons because the few evidence found in the literature. OBJECTIVE: To bring more clear evidence in the comparison between total and partial fundoplication. DATA SOURCES: A systematic review of the literature and metaanalysis with randomized controlled trials accessed from MEDLINE, LILACS, Cochrane Controll

The efficacy and safety of rifaximin for the irritable bowel syndrome: a systematic review and meta-analysis.

OBJECTIVES: Irritable bowel syndrome (IBS) affects 10-15% of the population, and treatment options are limited. Rifaximin is a minimally absorbed antibiotic that has shown efficacy in IBS patients. The objective of our study was to perform a meta-analysis and systematic review of available randomized, placebo controlled trials evaluating the efficacy and tolerability of rifaximin in patients with IBS. METHODS: We performed a systematic literature search of multiple online electronic databases regardless of language. Inclusion criteria entailed randomized, placebo controlled trials and IBS def

Quantifying the fall in mortality associated with interventions related to hypertensive diseases of pregnancy.

BACKGROUND: In this paper we review the evidence of the effect of health interventions on mortality reduction from hypertensive diseases in pregnancy (HDP). We chose HDP because they represent a major cause of death in low income countries and evidence of effect on maternal mortality from randomised studies is available for some interventions. METHODS: We used four approaches to review the evidence of the effect of interventions to prevent or treat HDP on mortality reduction from HDP. We first reviewed the Cochrane Library to identify systematic reviews and individual trials of the efficacy o

[POEMS syndrome: a review of the literature].

POEMS syndrome is a rare multisystemic paraneoplastic disorder. That is associated with a plasma cell dyscrasia. It is characterized by peripheral neuropathy, organomegaly, endocrinopathy, M protein, skin changes and other systemic features. The pathogenesis of POEMS is not well understood, but overproduction of vascular endothelial growth factor is likely to be responsible for most of characteristic symptoms. There are no randomized controlled trials in patients with POEMS syndrome but are therapies which benefit these patients, including radiation therapy, chemotherapy, cortico-steroids and

Oral rinses, mouthwashes and sprays for improving recovery following tonsillectomy.

BACKGROUND: Tonsillectomy is the surgical removal of lymphoid tissue, the palatine tonsil, which is located at the back of the throat. It is still commonly performed for patients who have recurrent bouts of acute tonsillitis. Although tonsillectomy is considered a fairly minor procedure, pain and bleeding are two of the most common complications; either may delay recovery and can on occasion lead to hospital readmission. Postoperative tonsillectomy medication should provide an adequate reduction in morbidity while minimising side effects, therefore topical agents would seem to be an ideal, saf

Open versus laparoscopic pyloromyotomy for pyloric stenosis: a meta-analysis of randomized controlled trials.

OBJECTIVE: Aim of the study was to compare the outcomes after laparoscopic pyloromyotomy (LP) with those of open pyloromyotomy (OP) for infantile pyloric stenosis. METHOD: We conducted searches until February 2009 in multiple databases and identified randomized controlled trials comparing LP with OP for pyloric stenosis. Results were expressed using the odds ratio (OR) for categorical variables and standard weighted mean differences (SMD) for continuous outcomes. Study quality was assessed using the Cochrane Handbook 5.0.1 guidelines and statistical analysis was performed using RevMan 5.0.8 s

Interventions for hemiplegic shoulder pain: systematic review of randomised controlled trials.

PURPOSE: The primary aim of this study was to assess the effectiveness of possible interventions for hemiplegic shoulder pain. The secondary aim was to investigate whether reduction of subluxation or spasticity can decrease shoulder pain and whether a change in shoulder pain is related to change in passive shoulder external rotation. METHOD: MEDLINE, EMBASE, CINAHL and the Cochrane Central Register of Controlled Trials databases were searched to obtain the randomised, controlled trials. Two authors independently extracted data and assessed the methodological quality of studies. RESULTS: Eigh

Improving the design of maintenance studies for bipolar disorder.

BACKGROUND: In contrast to the trial design of acute mania studies, there is no standard design for bipolar maintenance studies. Over the past 15 years, the design of monotherapy maintenance studies in bipolar disorder has evolved significantly, but recent study designs continue to differ in important ways. SCOPE: We reviewed the design of recent controlled bipolar maintenance studies, using PubMed, from August 2006 to August 2009, examining the strengths and weaknesses of different study design features. FINDINGS: Design differences are sufficiently important that the disparate results acro
