INTRODUCTION: Heterotopic pregnancy is the rare coexistence of intrauterine and extrauterine gestation. The incidence may be as high as 1 in 3,889 pregnancies. The diagnosis is difficult to make, but an important one to consider in the presence of acute abdominal pain, hemorrhagic shock, and intrauterine pregnancy.

Tubo-ovarian abscess as a complication of acute salpingitis or salpingo-oophoritis is very uncommon in pre-menarchal and/or sexually inactive girls. It is generally the result of a blood-borne or genitourinary infection. Early diagnosis and treatment are essential to prevent future sequelae causing infertility. Laparoscopic surgery which minimises postoperative complications should be the first option in the treatment of TOA.

The transvaginal approach to tubo-ovarian surgery allows easy and direct access to the tubo-ovarian structures and the fossa ovarica without additional manipulation. In the absence of a panoramic view, the proximity of the tubo-ovarian structures allows operative procedures for treatment of superficial and cystic ovarian endometriosis, drilling of the ovarian capsule, adhesiolysis and salpingostomy. Pelvic abscess is a rare entity in pregnancy. The true etiology is uncertain but a flare-up of an old pelvic inflammatory disease is most likely. Usually, these patients undergo laparotomy and these abscesses are discovered incidentally, as described in our series. Surgical drainage and conservative surgical procedures under antibiotics are recommended during pregnancy albeit there is no consensus on patient management. Through this report, we aim to discuss this clinical entity and to underline the importance of patient management in case of its diagnosis.

In selected cases, medical treatment of ectopic pregnancy with methotrexate is as effective as laparoscopic surgery. However, medical treatment might have a more negative impact on the health-related quality of life than surgical treatment. This is partly because of the long resolution time after medical treatment. New evidence suggests that combining methotrexate and mifepristone can shorten this resolution time.

We have reported a very interesting, rare case of endometrial adenocarcinoma arising from pelvic endometriosis thirteen years after hysterectomy with bilateral salpingo-oophorectomy, followed with hormone replacement therapy. Our patient presented with history of abnormal vaginal bleeding, which was investigated with abdominal scans, examination under anaesthesia (EUA), and biopsy. Histology of which confirmed malignant transformation. We then proceeded to primary excision of all macroscopic tumour and palpable lymph nodes followed with radiotherapy.
