Clinical subphenotypes in septic patients with new-onset atrial fibrillation: validation and parsimonious classifier model development.

BACKGROUND: The substantial heterogeneity among septic patients with new‑onset atrial fibrillation (NOAF) complicates intensive care unit (ICU) management, and the absence of a parsimonious subphenotype classifier has impeded the implementation of personalized therapeutic strategies. In this study, we aim to identify the subphenotype of septic patients with NOAF, develop a concise classifier model, and reveal the differential efficacy of heart-rate control therapies across these subphenotypes. METHODS: This retrospective study utilized data from the Medical Information Mart in Intensive Care

Cancer staging data collection using rules-based natural language processing for entity extraction from pathology notifications: the WA cancer staging project.

BACKGROUND: Cancer staging data is vital for treatment planning, outcome prediction, clinical research, and healthcare resource allocation. Collection at the population level can improve insights, however existing manual methods are resource intensive. This study aimed to develop rules-based natural language processing (NLP) systems to: (1) extract explicit tumour, node, and metastasis (TNM) entities from data reported to the Western Australian Cancer Registry; (2) extract implicit entities translatable to individual TNM values; and (3) translate these values into cancer stages for melanoma, b

Game Theory-Based Adaptive Human-Machine Joint Learning for Online MI-BCI Decoding.

Motor imagery based brain-computer interface (MI-BCI) has been extensively researched for neurorehabilitation and motor assistance, while the performance of previous MI-BCI systems for online decoding motor intentions is still not satisfactory. Therefore, a game theory-based adaptive human-machine joint (AHMJ) learning method integrating subject learning and decoder updating was proposed for MI-BCI decoding, by which multi-class MI for unilateral upper limb can be successfully decoded online. On the one hand, a novel MI training method was proposed to facilitate subjects' learning to generate

NeuroBooster: a domain-informed self-supervised learning paradigm tailored for brain MRI analysis.

Self-supervised learning (SSL) has demonstrated its potential to reduce reliance on labeled data and improve generalization to independent test sets compared to traditional supervised learning (SL). However, current SSL paradigms are predominantly designed as domain-agnostic and are often evaluated on benchmark datasets like ImageNet. As already shown in other studies, SSL paradigms tailored for the medical domain can outperform domain-agnostic SSL paradigms. This study introduces NeuroBooster, a paradigm tailored for brain MRI analysis. Its core idea is to leverage automated feature extractio

An interpretable machine learning model integrating peripheral blood lncRNAs and clinical variables for phase classification in chronic myeloid leukemia.

BACKGROUND: Accurate disease phase assessment remains clinically important in chronic myeloid leukemia (CML), as progression to accelerated or blast phase (AP/BP) is associated with therapeutic resistance and poor prognosis. We aimed to develop an interpretable machine learning (ML) framework integrating peripheral blood long non-coding RNA (lncRNA) expression and clinical variables for disease phase assessment in CML. METHODS: Using peripheral blood from 305 treatment-naïve CML patients (85 AP/BP; 220 chronic phase) and 90 healthy controls, we identified a progression-associated ten-lncRNA s

Predicting mortality risk in hospitalized ACS patients with hypertensive comorbidity: an interpretable machine learning approach.

BACKGROUND: Hypertension serves as a prevalent health issue, particularly in South Asia, where it is also a risk factor and comorbidity that affects the prognosis of Acute Coronary Syndrome (ACS) by increasing the likelihood of complications such as heart failure and arrhythmias. Conventional tools for predicting ACS mortality risk, such as the TIMI and GRACE scores, often fail to adequately represent the Asian population and do not specifically address the needs of ACS patients with comorbid hypertension. Our study focuses on developing a tailored and interpretable risk prediction model to as

ECG-MTDA: A Framework for Morphology-Temporal Decoupling and LLM-Guided Text Alignment.

Effective automated electrocardiogram (ECG) interpretation hinges on disentangling waveform morphology from rhythm dynamics, a challenge for existing multimodal models that often conflate these heterogeneous attributes and introduce semantic ambiguity. We introduce ECG-MTDA, a framework that explicitly decouples these components. It learns morphology-oriented representations via a PQRST-guided masked autoencoder, while separately modeling temporal dynamics using continuous wavelet transform. Crucially, we align the learned morphology with concise, label-conditioned textual descriptions generat

Machine learning-based prediction of disability in subacute low back pain: a primary care study on clinical and psychosocial determinants.

BACKGROUND: Subacute low back pain (LBP) is a highly prevalent condition and a major contributor to disability and health care burden. Early identification of individuals at risk of poor functional recovery is essential to support decision-making in primary care. Although prior research has identified relevant clinical and psychosocial predictors, the application of machine learning techniques for modeling disability and pain outcomes in this population remains limited. METHODS: We conducted a prospective observational study involving 92 adult patients with subacute LBP attending primary care

KidMesh: Computational Mesh Reconstruction for Pediatric Congenital Hydronephrosis Using Deep Neural Networks.

Pediatric congenital hydronephrosis (CH) is a common urinary tract disorder, primarily caused by obstruction at the renal pelvis-ureter junction. Magnetic resonance urography (MRU) can visualize hydronephrosis, including renal pelvis and calyces, by utilizing the natural contrast provided by water. Existing voxel-based segmentation approaches can extract CH regions from MRU, facilitating disease diagnosis and prognosis. However, these segmentation methods predominantly focus on morphological features, such as size, shape, and structure. To enable functional assessments, such as urodynamic simu

Multitask learning of longitudinal circulating biomarkers and clinical outcomes: identification of optimal machine-learning and deep-learning models.

BACKGROUND: Many circulating biomarkers are assessed at different time intervals during clinical studies. Despite of the success of standard joint models in predicting clinical outcomes using low-dimensional longitudinal data (1-2 biomarkers), significant computational challenges are encountered when applying these techniques to high-dimensional biomarker datasets. Modern machine- or deep-learning models show potential for multiple biomarker processes, but systematic evaluations and applications to high-dimensional data in the clinical settings have yet to be reported. We aimed to enhance the

TinnitusLLM: A Multimodal Large Language Model Framework for Tinnitus Diagnosis Through EEG-fMRI Fusion Learning.

Accurate tinnitus diagnosis is crucial for enabling timely therapeutic intervention and longitudinal treatment monitoring. While non-invasive neuroimaging modalities-particularly electroencephalography (EEG) with millisecond temporal resolution and functional magnetic resonance imaging (fMRI) with millimeter spatial resolution- provide complementary neural features, existing diagnostic approaches remain constrained to unimodal analysis of EEG or fMRI data, inherently limiting diagnostic precision and clinical generalizability. This paper introduces TinnitusLLM, the first multimodal large langu
