Machine Learning Models Using Routinely Collected Clinical Data Offer Robust and Interpretable Predictions of 90-Day Unplanned Acute Care Use for Cancer Immunotherapy Patients.

PURPOSE: Clinical management of patients receiving immune checkpoint inhibitors (ICIs) could be informed using accurate predictive tools to identify patients at risk of short-term acute care utilization (ACU). We used routinely collected data to develop and assess machine learning (ML) algorithms to predict unplanned ACU within 90 days of ICI treatment initiation. METHODS: We used aggregated electronic health record data from 7,960 patients receiving ICI treatments to train and assess eight ML algorithms. We developed the models using pre-SARS-COV-19 COVID-19 data generated between January 20

Machine learning-based modeling of acute respiratory failure following emergency general surgery operations.

BACKGROUND: Emergency general surgery (EGS) operations are associated with substantial risk of morbidity including postoperative respiratory failure (PRF). While existing risk models are not widely utilized and rely on traditional statistical methods, application of machine learning (ML) in prediction of PRF following EGS remains unexplored. OBJECTIVE: The present study aimed to develop ML-based prediction models for respiratory failure following EGS and compare their performance to traditional regression models using a nationally-representative cohort. METHODS: Non-elective hospitalizations

Validation of the GALAD Model and Establishment of GAAP Model for Diagnosis of Hepatocellular Carcinoma in Chinese Patients.

PURPOSE: GALAD is a statistical model for estimating the likelihood of having hepatocellular carcinoma (HCC) based on gender, age, AFP, AFP-L3, and PIVKA-II. We aimed to assess its performance and build new models in China, where hepatitis B virus (HBV) is the leading etiology of HCC. PATIENTS AND METHODS: We built the GALAD-C model with the same five variables in GALAD, and the GAAP model with gender, age, AFP, and PIVKA-II, using logistic regression based on 242 patients with HCC and 283 patients with chronic liver disease (CLD). We also collected 50 patients with other malignant liver tumo

Forecasting the 2017/2018 seasonal influenza epidemic in England using multiple dynamic transmission models: a case study.

BACKGROUND: Since the 2009 A/H1N1 pandemic, Public Health England have developed a suite of real-time statistical models utilising enhanced pandemic surveillance data to nowcast and forecast a future pandemic. Their ability to track seasonal influenza and predict heightened winter healthcare burden in the light of high activity in Australia in 2017 was untested. METHODS: Four transmission models were used in forecasting the 2017/2018 seasonal influenza epidemic in England: a stratified primary care model using daily, region-specific, counts and virological swab positivity of influenza-like il

Toward a Robust, Universal Predictor of Gas Hydrate Equilibria by Means of a Deep Learning Regression.

Due to offshore reservoirs being developed in ever deeper and colder waters, gas hydrates are increasingly becoming a significant factor when considering the profitability of a reservoir due to flow disruptions, equipment, and safety hazards arising from the hydrate plug formation. Due to low-dosage hydrate inhibitors such as kinetic inhibitors competing with traditional thermodynamic inhibitors such as methanol, accurate information regarding the hydrate equilibrium conditions is required to determine the optimal hydrate control strategy. Existing thermodynamic models can prove inflexible reg

Multiparametric functional magnetic resonance imaging for evaluation of hepatic warm ischemia-reperfusion injury in a rabbit model.

BACKGROUND: To assess the feasibility of noninvasive and quantitative evaluation of hepatic pathophysiological changes in rabbit hepatic warm ischemia-reperfusion injury (WIRI) models by using intravoxel incoherent motion (IVIM), diffusion tensor imaging (DTI) and blood oxygen level dependent (BOLD) MRI. METHODS: Twenty rabbits were randomly divided into hepatic WIRI model group and sham-operation group (n = 10 for each group). Hepatic WIRI was induced in rabbit by occluding hepatic inflow for 30 min and reperfusion for 6 h. The control group only underwent laparotomy and liver ligament disse

Prevention of breast cancer in postmenopausal women: approaches to estimating and reducing risk.

BACKGROUND: It is uncertain whether evidence supports routinely estimating a postmenopausal woman's risk of breast cancer and intervening to reduce risk. METHODS: We systematically reviewed prospective studies about models and sex hormone levels to assess breast cancer risk and used meta-analysis with random effects models to summarize the predictive accuracy of breast density. We also reviewed prospective studies of the effects of exercise, weight management, healthy diet, moderate alcohol consumption, and fruit and vegetable intake on breast cancer risk, and used random effects models for a

The accuracy and clinical application of predictive models for primary open-angle glaucoma in ocular hypertensive individuals.

OBJECTIVE: This report compares the accuracy of 3 prediction models for the development of primary open-angle glaucoma (POAG). The models differ primarily in their handling of these eye-specific variables: intraocular pressure (IOP), central corneal thickness (CCT), vertical cup-to-disc ratio (VCD), and visual field pattern standard deviation (PSD). The "means" model includes age and the means of right and left eyes; the "means plus asymmetry" model includes age, the means of right and left eyes as well as the absolute difference between eyes for eye-specific variables; and the "worse" eye mod

Periappendiceal fat-stranding models for discriminating between complicated and uncomplicated acute appendicitis: a diagnostic and validation study.

BACKGROUND: Recent studies have reported promising outcomes of non-operative treatment for uncomplicated appendicitis; however, the preoperative prediction of complicated appendicitis is challenging. We developed models by incorporating fat stranding (FS), which is commonly observed in perforated appendicitis. MATERIAL AND METHODS: We reviewed the data of 402 consecutive patients with confirmed acute appendicitis from our prospective registry. Multivariate logistic regression was performed to select clinical and radiographic factors predicting complicated acute appendicitis in our model 1 (in

The indirect association between psychological resilience and prosocial behavior through cognitive reappraisal in Chinese college students.

INTRODUCTION: Psychological resilience is commonly defined as an individual's capacity to maintain stability and engage in constructive adaptation in the face of adversity, and its role in promoting positive developmental outcomes among college students has received increasing attention. Individuals with higher levels of resilience tend to exhibit more balanced emotional responses under stress, which may in turn facilitate their involvement in prosocial actions such as helping, comforting, cooperating, sharing, and donating. Cognitive reappraisal-the cognitive reframing of potentially stressfu

Causal effect estimation for ordinal outcomes: a performance comparison of G-computation, IPW, and TMLE-based methods.

BACKGROUND: Ordinal outcomes are frequently used in clinical and observational studies. However, because ordinal variables do not have clearly defined intervals between categories, causal effect estimation for ordinal outcomes is more challenging than that for binary or continuous outcomes. Therefore, accurate causal inference for ordinal outcomes is important, as inadequate covariate adjustment may lead to biased estimates due to confounding. In this study, we defined the average treatment effect (ATE) for ordinal outcomes and compared the performance of various causal inference methods for e

Meta-analytical federated surveillance for incubation period with interval- and right-censored data with stochastic variational inference.

Accurate estimation of incubation period is fundamental to surveillance for infectious diseases. This task faces two major challenges: (1) surveillance data are often coarsely observed, comprising interval- and right-censored observations, and (2) patient-level data are frequently fragmented across multiple facilities and cannot be pooled due to privacy constraints. While traditional meta-analysis (MA) aggregates summary statistics, its one-shot nature limits the ability to fit complex hierarchical models. We propose a novel federated Bayesian framework, Federated Stochastic Variational Infere

Powering RCTs for Marginal Effects With GLMs Using Prognostic Score Adjustment.

In randomized clinical trials (RCTs), the accurate estimation of marginal treatment effects is crucial for determining the efficacy of interventions. Enhancing the statistical power of these analyses is a key objective for statisticians. The increasing availability of historical data from registries, prior trials, and health records presents an opportunity to improve trial efficiency. However, many methods for historical borrowing compromise strict type I error rate control. Building on the work by Schuler et al. on prognostic score adjustment for linear models, this paper extends the methodol

Examining the relationship between quest for significance and smartphone addiction: does the dual passion model mediate this relationship?

BACKGROUND: In Turkey, smartphone use exceeds both European and global averages, posing potential risks to individuals' physical, social, and psychological well-being. The present research aimed to examine the relationships among smartphone addiction, significance quest, and passion, as well as to identify the mediating role of passion in the relationship between significance quest and smartphone addiction. In this regard, since there was no instrument available to assess passion within the Turkish cultural context, Study 1 adapted the Passion Scale for use in Turkish culture. Study 2 then inv

Joint Bayesian Nowcasting of Severe Acute Respiratory Illness and COVID-19 Positives in Brazil.

Surveillance of severe acute respiratory illness (SARI) in Brazil provides an early warning system for respiratory outbreaks, including COVID-19, and statistical nowcasting methods are vital for correcting long and variable reporting delays that would otherwise hinder timely decision-making. Coherent joint prediction of SARI and COVID-positive SARI in Brazil could improve outbreak response planning and hospital resource management, but most existing nowcasting methods only target one outcome at a time. Furthermore, existing approaches usually focus solely on reconstructing recent incidence rat

Probabilistic clinical target definition with nearest neighbor correlation.

Objective. The delineation of the clinical target volume (CTV) in radiotherapy is fundamentally uncertain due to the invisibility of microscopic disease on medical images. The ICRU 83 report acknowledges this by proposing a probabilistic interpretation of the CTV, but it does not define how to compute the probability of microscopic tumor presence (MTP) in tissue. This work addresses this gap by introducing a novel stochastic model that estimates the probability of MTP at the voxel level based on local spatial correlations in the voxels' neighborhood.Approach.We developed two first-principles st

Predicting intracranial hypertension as a secondary insult using the pathological peptidome of brain injured patients.

Intracranial hypertension (IHT) is a common secondary insult that worsens morbidity and death after acute brain injury from traumatic and hemorrhagic events. Suspicion of IHT is determined with a combination of neurological examination and imaging but is often hampered by sedation and limited by cost and radiation exposure for repeated assessment. Given the uncertainty, many at risk for IHT may not have intracranial pressure (ICP) monitors inserted when weighed against the risks of this invasive surgical procedure. Thus, the availability of blood-based biomarkers diagnostic of IHT would repres

Joint Frailty Mixture Cure Model for Recurrent Event Data With Dependent Censoring: An MCEM Approach.

Advancements in modern medical technology have enabled cures for a fraction of patients while extending survival times for those who are not cured. For non-cured patients, disease recurrence is influenced by observed covariates and unobserved individual heterogeneity (random effects). In biomedical studies, dependent censoring is frequently encountered, for example, in cancer patients, where right censoring can be caused by death from unrelated diseases or due to an (unobservable) cure status. This study introduces a joint frailty model for recurrent event data with a cure fraction, effectivel
