Free functioning muscle transfer in radiation-induced brachial plexopathy: case report.

Radiation-induced brachial plexopathy is a delayed complication of radiation treatment for tumors involving the neck and chest area and is progressive. A 56-year-old woman presented to us with loss of elbow flexion and weak wrist and finger extension 15 years after she received external beam radiation to the left chest, axilla, and supraclavicular region for treatment of breast cancer. She was managed with a gracilis free muscle transfer for elbow flexion and hand prehension. By 2 years after surgery she regained elbow range of motion of 40° to 110° and improved in hand function. She was able

Septic arthritis and osteomyelitis due to Bordetella petrii.

A case of Bordetella petrii septic arthritis and osteomyelitis in an elbow resulted from a dirt bike accident in Hawaii. Two months of intravenous antibiotics and repeated surgeries were required to cure this infection. Our case, and literature review, suggests that extended-spectrum penicillins, tetracycline, and trimethoprim-sulfamethoxazole are good treatment options.

A case of supracondylar humerus fracture with vascular impairment. Open it up?

Supracondylar humerus fracture is one of the most frequent fractures in childhood. A serious complication is an injury to the neurovascular structures which could potentially result in severe functional impairment. We are presenting the case of a 3-year-old girl with a supracondylar humerus fracture in our emergency department and highlight the diagnostic and therapeutic steps in managing this situation. Initially, the hand was well perfused but showed to be pulseless in the operating theater after preparation for surgery. After open reduction and internal pin fixation the neurovascular struct

The management of anterior dislocation of the shoulder with proximal humerus fracture, bony Bankart lesion, and associated glenoid labrum avulsion: a report of 2 cases.

Proximal humerus fractures are common in the older population. These fractures can be associated with dislocation of the shoulder. Four-part fracture-dislocations and head-splitting 3-part fractures are generally indicated for shoulder hemiarthroplasty in the older population. When such surgery is warranted, inspection of the glenoid during the procedure is indicated, as loss of evaluation may lead to postoperative shoulder instability. There is value in alerting orthopedic surgeons to the possibility of anterior glenoid rim fracture in association with proximal shoulder fracture-dislocation.

Joint magnetic resonance imaging: normal variants and pitfalls related to sports injury.

MR imaging abnormalities, such as increased signal within normally hypointense structures, form and attachment abnormalities, fluid collections in joints, tendon sheaths and bursa, or even tumors, such as Morton's neuromas, are common in asymptomatic volunteers. They may be explained by normal physiology, anatomic variability, MR imaging artifacts, or true abnormalities without clinical importance. Although it is not always possible to differentiate such variants or artifacts from clinically relevant findings, it is important to know their potential cause and clinical importance and not to ove

[Multitruncular block at the elbow for a major hand trauma for prehospital care].

Upper limb peripheral nerve blocks offer many advantages but they are not widely used in prehospital care. We report the case of a multitroncular block at the elbow made by the emergency department team out of the hospital after a major hand trauma. A successful axillary brachial plexus block was done two hours later, without any problem. This case report allows us to discuss about different analgesia techniques useful in prehospital care.

A surgical treatment for unstable osteochondritis dissecans lesions of the humeral capitellum in adolescent baseball players.

BACKGROUND: Various methods of surgical treatment for unstable osteochondritis dissecans lesions of the humeral capitellum have shown the potential to lead to osteoarthritis and poor functional results in throwing activities. PURPOSE: The purpose of this study was to evaluate the results of surgical treatment with pull-out wiring and bone grafting (as established by Kondo in 1989) in patients with unstable osteochondritis dissecans lesions of the humeral capitellum. STUDY DESIGN: Retrospective review. METHODS: Eleven male baseball players (average age, 14.7 years) with unstable osteochondri

Right and left traumatic fractures of a bilateral spontaneous fusion of the elbow. A case report.

Report of an exceptional case of spontaneous ankylosis of both elbows, without any family history, trauma, infection or rheumatological disease. An incomplete form of a congenital known syndrome or a thermal reflex sympathetic dystrophy were the most plausible etiologies. The traumatic fractures of both radio-ulnohumeral synostoses have been treated conservatively, within a five years interval period. The arthrodesis of both elbows at 35 and 80 degree of flexion did not impinge on the life autonomy and the work ability of the patient.

Radial head fractures associated with elbow dislocations treated by immediate stabilization and early motion.

Twenty-one elbow dislocations with an associated radial head fracture were treated with immediate joint reduction, stabilization, and early range-of-motion exercises. In all cases initial treatment involved closed reduction of the ulnohumeral joint. For those cases involving minimally displaced and a few moderately displaced radial head fractures, treatment consisted of benign neglect (4 of 21). Of the more severely displaced fractures (17 of 21), 9 were treated with open-reduction internal fixation and 8 with immediate silicone head replacement. Despite radial head treatment, 6 of these cases

Clinical and functional outcomes of rehabilitation strategies following arthroscopic repair of chronic isolated peripheral TFCC tears: A scoping review.

This scoping review aims to evaluate the existing literature regarding the types of Triangular Fibrocartilage Complex (TFCC) tears that are amenable to repair, the prevalent rehabilitation protocols, and the documented measurable outcomes across various populations. The review was conducted as per JBI methodology and reported as per PRISMA-ScR guidelines. A total of 35 studies published between 1996 and 2024 were included. Palmer Type 1B was the most frequently operated type of tear. The tears were repaired using various arthroscopic-assisted techniques like capsular repair with polydiaxone su

Differential effects of sex on upper body kinematics and kinetics during fatiguing, Asymmetric lifting.

This study quantified sex-specific biomechanical adaptations to fatigue in asymmetric lifting. Twenty-one females and fifteen males performed a prolonged asymmetric lifting protocol while upper body, trunk and pelvis kinematics were collected. Features of movement identified with principal component analysis, and peak joint angular velocities and moments were calculated. Sex-specific kinematic adaptations to fatigue included females adopting a 'stoop-like' lifting strategy to a greater extent than males. Additionally, females exhibited higher vertical elbow positions during load rotation, move

Retrograde Drilling With Tibial Autograft in Osteochondral Lesions of the Talar Dome.

Osteochondral lesions that compromise the ankle are rare, with an incidence between 0.02% and 1.5% according to different series. This location is the third in frequency, after knee and elbow. The location of the osteochondral lesion allows one to infer the producing mechanism. Lateral defects are produced by inversion and dorsiflexion of the ankle (usually anterior, affecting 3 and 6 talar zones), whereas medial defects are produced by plantar flexion, inversion, and internal rotation (most commonly posterior, affecting 4 and 7 talar zones). The injury causes pain associated with weight load,

Characteristics of potential concussive events in three elite football tournaments.

OBJECTIVE: Identify patterns in the nature and characteristics of potential concussive events (PCEs) in football. METHODS: This study analysed the incidence and characteristics of PCEs that occurred during the 2014 and 2018 Fédération Internationale de Football Association World Cups, and the 2016 UEFA Euro Cup. PCEs were defined as direct head collision incidents resulting in the athlete being unable to immediately resume play for at least 5 sec following impact. RESULTS: A total of 218 incidents were identified in 179 matches (1.22 per match, 36.91 per 1000 hours of exposure). The most com

Control of a time-delayed 5 degrees of freedom arm model for use in upper extremity functional electrical stimulation.

The goal of this work is to design a controller for a functional electrical stimulation (FES) neuroprosthesis aimed at restoring shoulder and elbow function in individuals who have suffered a high-level cervical (C3-C4) spinal cord injury (SCI). The controller is a mathematical algorithm that coordinates the electrical stimulations applied to the paralyzed muscles such that the arm closely tracks a given desired trajectory. An issue that so far has received little attention is that of time-delays. These delays arise from two sources: (1) the muscle excitation-activation dynamics (10-30 ms) and

Outcomes after percutaneous reduction and fixation of proximal humeral fractures.

The purpose of this study is to report the outcomes after closed or percutaneous reduction with percutaneous fixation of displaced proximal humeral fractures. This study included 35 patients from 3 institutions. Of these, 27 were followed up for a minimum of 1 year after surgery. The mean age at injury was 61 years. There were seven 2-part, eight 3-part, and twelve valgus-impacted 4-part proximal humeral fractures. All fractures were reduced and stabilized with percutaneous techniques only. The mean duration of follow-up was 35 months. All fractures healed after the index procedure. The mean p

Ergonomic exposures in apple harvesting: preliminary observations.

BACKGROUND: Recent research on occupational injury to apple harvest workers in New York and Pennsylvania indicates that muscle strain is one of their leading problems. A descriptive study of ergonomic risks for strains of the shoulder and lower back among apple harvesters was carried out to identify opportunities for intervention. METHODS: Based upon pilot observation in four orchards, a Posture-Activities-Tools-Handling (PATH) data collection template was designed and used to analyze the work of fifty-one workers in eight additional apple orchards. RESULTS: Physical loading on the shoulder

Feasibility of using fNIRS to explore motor-related regional haemodynamic signal changes in patients with sensorimotor impairment and healthy controls: A pilot study.

BACKGROUND: While functional near-infrared spectroscopy (fNIRS) can provide insight into cortical brain activity during motor tasks in healthy and diseased populations, the feasibility of using fNIRS to assess haemoglobin-evoked responses to reanimated upper limb motor function in patients with tetraplegia remains unknown. OBJECTIVE: The primary objective of this pilot study is to determine the feasibility of using fNIRS to assess cortical signal intensity changes during upper limb motor tasks in individuals with surgically restored grip functions. The secondary objectives are: 1) to collect
