CASPR2-related epilepsy: A distinctive and unrecognized form of epilepsy in adult and elderly males.

OBJECTIVE: The aim of this study was to describe the clinical features of contactin-associated protein-like 2 (CASPR2)-IgG-associated seizures. METHODS: Nine patients were retrospectively collected from two epilepsy centers. For each patient we obtained a full clinical, neurophysiological, and MRI study along with detection of antineuronal autoantibodies from serum and CSF. The patients were followed up for 1-6 years. RESULTS: The patients were nine male subjects aged 56-85 years (mean: 66) with a 1- to 14-year (mean: 6,3 median: 6) history of seizures. The seizures were classified as focal

Structure and function in acquired prosopagnosia: lessons from a series of 10 patients with brain damage.

Acquired prosopagnosia varies in both behavioural manifestations and the location and extent of underlying lesions. We studied 10 patients with adult-onset lesions on a battery of face-processing tests. Using signal detection methods, we found that discriminative power for the familiarity of famous faces was most reduced by bilateral occipitotemporal lesions that involved the fusiform gyri, and better preserved with unilateral right-sided lesions. Tests of perception of facial structural configuration showed severe deficits with lesions that included the right fusiform gyrus, whether unilatera

Regional microglial populations in central autonomic brain regions in SUDEP.

OBJECTIVE: Sudden unexpected death in epilepsy (SUDEP) may arise as a result of autonomic dysfunction during a seizure. The central autonomic networks (CANs) modulate brainstem cardiorespiratory regulation. Recent magnetic resonance imaging (MRI) studies in SUDEP have shown cortical and subcortical volume changes and altered connectivity between CAN regions, but the pathological correlate is unknown. Because neuroinflammation is both a cause and a consequence of seizures and may relate to regional brain pathology, our aim was to evaluate microglial populations in CANs in SUDEP. METHODS: In 55

Seizure characteristics, treatment, and outcome in autoimmune synaptic encephalitis: A long-term study.

OBJECTIVES: The objective of this study was to report seizure characteristics, long-term outcome, and potential factors associated with persistent seizures in patients with autoimmune synaptic encephalitis (ASE). METHOD: Clinical data and courses of 52 patients with ASE who presented with seizures at the Department of Neurology of the First Hospital of Jilin University from January 2015 to August 2017 were reviewed. Seizure outcomes were assessed with a median follow-up duration of 30 months (8-40 months). RESULTS: Most patients (71.2%) presented with seizure at initial consultation; focal t

Pergularia daemia hydro-ethanolic extract protects against pentylenetetrazole kindling-induced seizures, oxidative stress, and neuroinflammation in mice.

ETHNOPHARMACOLOGICAL RELEVANCE: Current antiepileptic drugs fail to control approximately 30% of epilepsies. Therefore, there is a need to develop more effective antiepileptic drugs, and medicinal plants provide an attractive source for new compounds. Pergularia daemia (Asclepiadaceae) is used in Cameroon traditional medicine to treat stroke, anemia, inflammation, and epilepsy. Recently, traditional healers claim that an hydro-ethanolic extract of the roots of P. daemia is more effective than an aqueous extract on refractory seizures. AIM OF THE STUDY: The antiepileptic effect of P. daemia hy

Voxel-based iterative sensitivity (VBIS) analysis: methods and a validation of intensity scaling for T2-weighted imaging of hippocampal sclerosis.

Abnormalities in the brain generally manifest on MRI as changes in shape (morphometry) or changes in the nature of the tissue (signal intensity). Voxel Based Morphometry (VBM) is a whole brain quantitative way of assessing morphometric changes. Voxel Based Relaxometry (VBR) directly assesses signal intensity changes in quantitative maps of T2 relaxation time, but this requires specialised multiple-echo acquisition sequences that are not usually available at clinical sites. This paper introduces and assesses an objective voxel-based statistical method for evaluation of signal intensity in group

Extraventricular neurocytoma with ganglionic differentiation associated with complex partial seizures.

We report an unusual case of extraventricular ("cerebral") neurocytoma with ganglion cells located in the right temporal lobe in a 9-year-old girl with complex partial seizures and precocious puberty. CT showed a calcified mass with central cystic zones. MR imaging showed a markedly hyperintense predominately solid tumor on both T1- and T2-weighted images, without appreciable contrast enhancement. Cerebral neurocytomas are histologically benign and radical surgery is curative; they should be included in the differential diagnosis of temporal lobe tumors in children.

Resection planning in extratemporal epilepsy surgery using 3D multimodality imaging and intraoperative MRI.

Surgical resection in non-lesional, extratemporal epilepsy, informed by stereoEEG recordings, is challenging. There are no clear borders of resection, and the surgeon is often operating in deep areas of the brain that are difficult to access. We present a technical note where 3D multimodality image integration in EpiNavTM is used to build a planned resection model, based on a previous intracranial EEG evaluation. Intraoperative MRI is then used to ensure a complete resection of the planned model. As stereoEEG becomes more common in the presurgical evaluation of epilepsy, these tools will becom

Combined sub-threshold dosages of phenobarbital and low-frequency stimulation effectively reduce seizures in amygdala-kindled rats.

Low-frequency stimulation (LFS) is a potential therapy utilized in patients who do not achieve satisfactory control of seizures with pharmacological treatments. Here, we investigated the interaction between anticonvulsant effects of LFS and phenobarbital (a commonly used medicine) on amygdala-kindled seizures in rats. Animals were kindled by electrical stimulation of basolateral amygdala in a rapid manner (12 stimulations/day). Fully kindled animals randomly received one of the three treatment choices: phenobarbital (1, 2, 3, 4 and 8 mg/kg; i.p.; 30 min before kindling stimulation), LFS (one o

Bilateral capacity for speech sound processing in auditory comprehension: evidence from Wada procedures.

Data from lesion studies suggest that the ability to perceive speech sounds, as measured by auditory comprehension tasks, is supported by temporal lobe systems in both the left and right hemisphere. For example, patients with left temporal lobe damage and auditory comprehension deficits (i.e., Wernicke's aphasics), nonetheless comprehend isolated words better than one would expect if their speech perception system had been largely destroyed (70-80% accuracy). Further, when comprehension fails in such patients their errors are more often semantically-based, than-phonemically based. The question

FMRI-determined language lateralization in patients with unilateral or mixed language dominance according to the Wada test.

Due to the reported variability of the language laterality index (LI) across fMRI studies, reliable distinction between patients with unilateral and mixed language dominance is currently not possible, preventing clinical implementation of fMRI as a replacement for the invasive Wada test. Variability of the LI may be related to differences in experimental and control tasks, and statistical methodology. The goal of this study was to improve detection power of fMRI for hemispheric language dominance by using a combined analysis of four different language tasks (CTA), that has previously shown mor

Network properties of the dentate gyrus in epileptic rats with hilar neuron loss and granule cell axon reorganization.

Neuron loss in the hilus of the dentate gyrus and granule cell axon reorganization have been proposed as etiologic factors in human temporal lobe epilepsy. To explore these possible epileptogenic mechanisms, electrophysiological and anatomic methods were used to examine the dentate gyrus network in adult rats that had been treated systemically with kainic acid. All kainate-treated rats, but no age-matched vehicle-treated controls, were observed to have spontaneous recurrent motor seizures beginning weeks to months after exposure to kainate. Epileptic kainate-treated rats and control animals we

Perception of apparent motion of colored stimuli after commissurotomy.

One subject with complete forebrain commissurotomy (L.B.), another with posterior callosotomy (D.K.), and eight normal controls were presented with successive pairs of red and/or green lights, on either side of the retinal midline and within the left and right visual fields. All of the subjects could discriminate the direction of apparent motion in all three locations, although L.B. was poorer on bilateral than unilateral presentations. Moreover, on bilateral presentations, L.B. was significantly poorer at identifying the color of the first light than the color of the second light. In contrast

Dual mental functioning in a split-brain patient.

Case studies of two individuals who had undergone complete corpus callosotomies are presented. In addition to anecdotal observations, controlled neurobehavioral and neuropsychological testing indicated that although both parents demonstrated the disconnection syndromes unique to "split-brain" patients, they also differed rather dramatically. For example, 1-C's left hemisphere developed bilateral motor control, whereas 2-C demonstrated the presence of two independent forms of conscious-awareness, one of which resided in the right and the other in the left hemisphere. Hence, 2-C's right brain wa

Serotonin Syndrome Without Overdose: Polypharmacy-Induced Toxicity in a Medically Stable Young Adult.

Serotonin syndrome is a potentially life-threatening condition caused by excessive serotonin activity in the central and peripheral nervous systems that can lead to symptoms such as altered mental status, autonomic instability (e.g., hyperthermia and hypertensive tachycardia), and neuromuscular abnormalities (e.g., clonus and tremor). We present the case of a 29-year-old male healthcare worker with a history of major depressive disorder (MDD), temporal lobe epilepsy, and suspected obsessive-compulsive disorder (OCD) who presented with sudden-onset agitation, confusion, and abnormal limb moveme

Attention-gated 3D CapsNet for robust hippocampal segmentation.

PURPOSE: The hippocampus is organized in subfields (HSF) involved in learning and memory processes and widely implicated in pathologies at different ages of life, from neonatal hypoxia to temporal lobe epilepsy or Alzheimer's disease. Getting a highly accurate and robust delineation of sub-millimetric regions such as HSF to investigate anatomo-functional hypotheses is a challenge. One of the main difficulties encountered by those methodologies is related to the small size and anatomical variability of HSF, resulting in the scarcity of manual data labeling. Recently introduced, capsule networks

Fine hippocampal morphology analysis with a multi-dataset cross-sectional study on 2911 subjects.

CA1 subfield and subiculum of the hippocampus contain a series of dentate bulges, which are also called hippocampus dentation (HD). There have been several studies demonstrating an association between HD and brain disorders. Such as the number of hippocampal dentation correlates with temporal lobe epilepsy. And epileptic hippocampus have a lower number of dentation compared to contralateral hippocampus. However, most studies rely on subjective assessment by manual searching and counting in HD areas, which is time-consuming and labor-intensive to process large amounts of samples. And to date, o

Selective activation of the hypothalamic orexinergic but not melanin-concentrating hormone neurons following pilocarpine-induced seizures in rats.

INTRODUCTION: Sleep disorders are common comorbidities in patients with temporal lobe epilepsy (TLE), but the underlying mechanisms remain poorly understood. Since the lateral hypothalamic (LH) and the perifornical orexinergic (ORX) and melanin-concentrating hormone (MCH) neurons are known to play opposing roles in the regulation of sleep and arousal, dysregulation of ORX and MCH neurons might contribute to the disturbance of sleep-wakefulness following epileptic seizures. METHODS: To test this hypothesis, rats were treated with lithium chloride and pilocarpine to induce status epilepticus (S
