Cost analysis of a coaching intervention to increase use of transradial percutaneous coronary intervention.

BACKGROUND: The transradial approach (TRA) to cardiac catheterization is safer than the traditional transfemoral approach (TFA), with similar clinical effectiveness. However, adoption of TRA remains low, representing less than 50% of catheterization procedures in 2015. Peer coaching is one approach to facilitate implementation; however, the costs of this strategy for cardiac procedures such as TRA are unclear. METHODS: We conducted an activity-based costing analysis (ABC) of a multi-center, hybrid type III implementation trial of a coaching intervention designed to increase the use of TRA. We

Adapting the Comprehensive Unit Safety Program (CUSP) implementation strategy to increase delivery of evidence-based cardiovascular risk factor care in community mental health organizations: protocol for a pilot study.

BACKGROUND: People with serious mental illnesses (SMI) such as schizophrenia and bipolar disorder experience excess mortality driven in large part by high rates of poorly controlled and under-treated cardiovascular risk factors. In the USA, integrated "behavioral health home" models in which specialty mental health organizations coordinate and manage physical health care for people with SMI are designed to improve guideline-concordant cardiovascular care for this group. Such models have been shown to improve cardiovascular care for clients with SMI in randomized clinical trials, but real-world

An observational study of the impact of COVID-19 and the rapid implementation of telehealth on community mental health center providers.

BACKGROUND: The COVID-19 pandemic has remarkably altered community mental health service delivery through the rapid implementation of telehealth. This study reports provider perspectives on the impact that COVID-19 and the transition to telehealth had on their work and their ability to deliver evidence-based practices (EBPs). METHODS: Providers (n = 93) completed online surveys with quantitative measures and open-ended items exploring their reactions to COVID-19 and to the transition to providing services via telehealth. RESULTS: Perceptions of personal risk and rumination around COVID-19 we

Fidelity of kangaroo mother care services in the public health facilities in Bangladesh: a cross-sectional mixed-method study.

BACKGROUND: Kangaroo mother care (KMC) is a proven low-cost intervention to prevent neonatal mortality of pre-term and low birth weight babies and is very relevant to Bangladesh. KMC provides thermal regulation and thus directly avert neonatal mortality. KMC includes early, continuous, and prolonged skin-to-skin contact between an infant and caregiver, exclusive breastfeeding, early discharge from the hospital, and post-discharge follow-up. The purpose of this study was to investigate the fidelity of this intervention's implementation according to national guidelines across all tiers of govern

Feasibility of a pharmacy-led intervention to de-implement non-guideline-concordant proton pump inhibitor use.

BACKGROUND: Proton pump inhibitors (PPIs) are among the most prescribed medications and are often used unnecessarily. PPIs are used for the treatment of heartburn and acid-related disorders. Emerging evidence indicates that PPIs are associated with serious adverse events, such as increased risk of Clostridioides difficile infection. In this study, we designed and piloted a PPI de-implementation intervention among hospitalized non-intensive care unit patients. METHODS: Using the Systems Engineering Initiative for Patient Safety (SEIPS) model as the framework, we developed an intervention with

Centralized colorectal cancer screening outreach and patient navigation for vulnerable populations in North Carolina: study protocol for the SCORE randomized controlled trial.

BACKGROUND: Although colorectal cancer (CRC) screening is effective in reducing CRC mortality, screening rates in vulnerable populations served by community health centers (CHCs) remain below national targets. CHCs in North Carolina are challenged to reach CRC screening targets as they tend to be under-resourced, have limited capacity to implement and sustain population health interventions, and typically operate independently from one another and from regional colonoscopy providers. The Scaling Colorectal Cancer Screening Through Outreach, Referral, and Engagement (SCORE) project is designed

Scaling up sexually transmissible infections point-of-care testing in remote Aboriginal and Torres Strait Islander communities: healthcare workers' perceptions of the barriers and facilitators.

BACKGROUND: Sexually transmissible infections (STIs), such as gonorrhoea and chlamydia, are highly prevalent, particularly in remote Aboriginal and Torres Strait Islander communities in Australia. In these settings, due to distance to centralised laboratories, the return of laboratory test results can take a week or longer, and many young people do not receive treatment, or it is considerably delayed. Point-of-care testing (POCT) provides an opportunity for same day diagnosis and treatment. Molecular POC testing for STIs was available at 31 regional or remote primary health care clinic sites t

Reducing Heavy Drinking Through the "Sober Curious" Movement in Australia: Protocol for a Mixed Methods Study.

BACKGROUND: Alcohol consumption is a major public health problem. Its socially engrained nature adds complexity to designing successful reduction approaches. Rather than implementing another intervention, we will undertake a natural experiment on the "sober curious" movement, which gained momentum through social media influencers promoting the idea of reducing alcohol consumption for wellness. We focus on ways to reduce alcohol consumption, through sober curiosity, with 4 heavy-drinking population groups: male construction workers; lesbian, gay, or bisexual women; hospitality workers; and tert

The Influence of Physical Exercise on Head and Neck Structures and Functions Related to Swallowing: A Scoping Review Protocol.

BACKGROUND: Dysphagia (swallowing impairment) can lead to malnutrition, dehydration, aspiration pneumonia, social isolation and even death. Current interventions for dysphagia include rehabilitation that improves swallowing physiology and function. Physical exercise is known to directly benefit skeletal muscles and may also benefit non-targeted skeletal muscles involved in swallowing, as these muscles are often engaged during vigorous exercise. Implementing physical exercise as a dysphagia intervention may help counteract sarcopenic dysphagia and delay symptoms associated with frailty. The obj

Financial costs of assisted reproductive technology for patients in low- and middle-income countries: a systematic review.

STUDY QUESTION: What are the direct costs of assisted reproductive technology (ART), and how affordable is it for patients in low- and middle-income countries (LMICS)? SUMMARY ANSWER: Direct medical costs paid by patients for infertility treatment are significantly higher than annual average income and GDP per capita, pointing to unaffordability and the risk of catastrophic expenditure for those in need. WHAT IS KNOWN ALREADY: Infertility treatment is largely inaccessible to many people in LMICs. Our analysis shows that no study in LMICs has previously compared ART medical costs across count

Bedaquiline resistance in patients with drug-resistant tuberculosis in Cape Town, South Africa: a retrospective longitudinal cohort study.

BACKGROUND: Bedaquiline is a life-saving tuberculosis drug undergoing global scale-up. People at risk of weak tuberculosis drug regimens are a priority for novel drug access despite the potential source of Mycobacterium tuberculosis-resistant strains. We aimed to characterise bedaquiline resistance in individuals who had sustained culture positivity during bedaquiline-based treatment. METHODS: We did a retrospective longitudinal cohort study of adults (aged ≥18 years) with culture-positive pulmonary tuberculosis who received at least 4 months of a bedaquiline-containing regimen from 12 drug-r

Biomedical Response to Neisseria gonorrhoeae and Other Sexually Transmitted Infections in the US Military.

INTRODUCTION: Sexually transmitted infections (STIs) continue to plague militaries and defense forces. While the historical recognition of the impact of STIs on operations is evident, contemporary surveillance and research activities are limited. As Neisseria gonorrhoeae and other sexually transmitted pathogens become increasingly resistant to antibiotics, the role of the Department of Defense (DoD) in disease surveillance and clinical research is essential to military Force Health Protection. METHODS: The Infectious Disease Clinical Research Program (IDCRP) of the Uniformed Services Universi

Translating clinical guidelines into practice: the effective and appropriate use of human growth hormone.

BACKGROUND: Dog-mediated rabies is endemic across Africa causing thousands of human deaths annually. A One Health approach to rabies is advocated, comprising emergency post-exposure vaccination of bite victims and mass dog vaccination to break the transmission cycle. However, the impacts and cost-effectiveness of these components are difficult to disentangle. METHODS: We combined contact tracing with whole-genome sequencing to track rabies transmission in the animal reservoir and spillover risk to humans from 2010 to 2020, investigating how the components of a One Health approach reduced the

Clinical utility and reproducibility of intravoxel incoherent motion MRI in rectal cancer: a structured narrative review.

INTRODUCTION: Magnetic resonance imaging (MRI) is a crucial imaging modality for the management of rectal cancer. Conventional MRI provides detailed anatomical information but has a limited capacity to evaluate tumor biology. Intravoxel Incoherent Motion (IVIM) is an advanced functional MRI technique that simultaneously measures tissue cellularity and microvascular perfusion without contrast agents, providing deeper insights into the tumor microenvironment. This narrative review critically examines the current evidence on the clinical usefulness of IVIM MRI in rectal cancer, focusing on three

Are Asymmetry-Focused Training Interventions More Effective than General Programs in Reducing Interlimb Asymmetries and Improving Athletic Performance? A Systematic Review.

BACKGROUND: Interlimb asymmetries (ILA) have been associated with an increased risk of injury and decreased athletic performance. However, despite the growing number of training interventions targeting ILA, their comparative effectiveness relative to general strength and conditioning approaches has not yet been systematically reviewed. OBJECTIVE: This systematic review aimed to compare the effectiveness of asymmetry-focused training interventions versus general training programs in reducing ILA and improving physical performance in athletic populations. Specifically, it sought to compare indi

Public health risks and control measures associated with live animal and animal product trade in traditional food markets in low- and middle-income countries: a scoping review.

INTRODUCTION: Traditional food markets (TFMs) support food security and livelihoods in low- and middle-income countries (LMICs), but the trade of live animals and products of animal origin may facilitate zoonotic and foodborne disease transmission. Evidence on associated risks and control measures remains fragmented across pathogens, market types and interventions. This scoping review maps the literature on zoonotic and foodborne public health risks, risk factors and control measures related to the trade of live animals and animal products in TFMs in LMICs. METHODS: Web of Science, PubMed and

Echocardiographic Phenomapping in HFpEF: From Imaging Biomarkers to Precision Cardiovascular Medicine.

BACKGROUND: Heart failure with preserved ejection fraction (HFpEF) is a highly prevalent and clinically heterogeneous syndrome characterized by substantial variability in pathophysiology, disease progression, therapeutic response, and clinical outcomes. Although conventional echocardiography remains central to diagnosis, traditional imaging parameters incompletely capture the biological complexity of HFpEF. Growing recognition of distinct disease phenotypes has stimulated interest in echocardiographic phenomapping as a strategy to improve patient characterization and support future precision m
