Management of psoriatic nail disease.

Nail involvement is common at some point in the life of the patient with psoriasis. Simple hand care, keeping nails cut short and avoiding nail trauma, will all help in management. Medical interventions include topical therapies used for psoriasis at other body sites, directed at the location of the disease within the nail unit. Individual digits may require focused intensive treatment, such as steroid injections. Systemic therapy for psoriatic nail disease can be justified when the disease presents in tandem with severe skin disease or where function and quality of life are sufficiently dimin

Surgical approach to hysterectomy for benign gynaecological disease.

BACKGROUND: The three approaches to hysterectomy for benign disease are abdominal hysterectomy (AH), vaginal hysterectomy (VH), and laparoscopic hysterectomy (LH). Laparoscopic hysterectomy has three further subdivisions depending on the part of the procedure performed laparoscopically. OBJECTIVES: To assess the most beneficial and least harmful surgical approach to hysterectomy for women with benign gynaecological conditions. SEARCH STRATEGY: We searched the Cochrane Menstrual Disorders and Subfertility Group Specialised Register of controlled trials (15 August 2008), CENTRAL (The Cochrane

Antiviral treatment for preventing postherpetic neuralgia.

BACKGROUND: Postherpetic neuralgia (PHN) is a painful and refractory complication of herpes zoster. Treatments are either partially or totally ineffective for many people with PHN. Antiviral agents, used at the time of the rash, are one of the best-established approaches that may prevent the development of PHN. OBJECTIVES: To investigate the effectiveness of antiviral agents in preventing PHN. SEARCH STRATEGY: We searched the Cochrane Neuromuscular Disease Group Trials Register (January 13 2009), the Cochrane Central Register of Controlled Trials (The Cochrane Library 2008, Issue 4), MEDLINE

Systematic review: tolerable amount of gluten for people with coeliac disease.

BACKGROUND: The threshold amount of gluten in 'gluten-free' products that can be tolerated by people with coeliac disease is unclear. AIM: To investigate the threshold amount of gluten and the threshold concentration of gluten in food products that can be tolerated by people with coeliac disease. DESIGN: Systematic review of studies published between 1966 and May 2007. METHODS: The data sources used were MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, CINAHL, and reference lists of retrieved articles. We included studies that evaluated the amount of dietary gluten or the co

Olanzapine pamoate: a stick in time? A review of the efficacy and safety profile of a new depot formulation of a second-generation antipsychotic.

Olanzapine pamoate, a long-acting depot preparation of olanzapine, is being evaluated by regulatory agencies for the treatment of schizophrenia. Clinical trial information was accessed by on-line query of http://www.pubmed.gov, http://www.clinicaltrials.gov and http://www.fda.gov, along with an examination of poster presentations at scientific meetings held in 2008. Two double-blind randomised clinical trials of olanzapine pamoate were conducted and demonstrate efficacy for both the acute treatment of schizophrenia and for the maintenance of antipsychotic response. Long-term open-label studies

Interventions (other than pharmacological, psychosocial or psychological) for treating antenatal depression.

BACKGROUND: Although pregnancy was once thought of as a time of emotional well-being for many women, conferring 'protection' against psychiatric disorders, a recent meta-analysis of 21 studies suggests the mean prevalence rate for depression across the antenatal period is 10.7%, ranging from 7.4% in the first trimester to a high of 12.8% in the second trimester. Due to maternal treatment preferences and potential concerns about fetal and infant health outcomes, non-pharmacological treatment options are needed. OBJECTIVES: To assess the effects, on mothers and their families, of non-pharmacolo

Investigational treatment options in microscopic colitis.

Collagenous and lymphocytic colitis are the two recognized major presentations of microscopic colitis. Both diseases present with chronic watery diarrhea and a chronic inflammatory infiltrate in the colonic mucosa without specific endoscopic abnormalities, and hence diagnosis is established by histology. Recent epidemiological studies suggest that microscopic colitis may affect as many patients as Crohn's disease or ulcerative colitis. The cause of these diseases is unknown; however, several lines of evidence support the hypothesis of mucosal injury from an unknown agent in the fecal stream. D

Teriparatide: a review of its use in osteoporosis.

Recombinant teriparatide (Forteo; Forsteo) is an anabolic (bone forming) agent. Studies have shown that subcutaneous teriparatide 20 microg/day is effective in women with postmenopausal osteoporosis, men with idiopathic or hypogonadal osteoporosis and patients with glucocorticoid-induced osteoporosis. Teriparatide improves bone mineral density (BMD) and alters the levels of bone formation and resorption markers; histomorphometric studies showed teriparatide-induced effects on bone structure, strength and quality. Subcutaneous teriparatide 20 microg/day administered over a treatment period of 1

Meta-analysis: mortality in Crohn's disease.

AIM: To perform a meta-analysis is of published literature reporting standardized mortality ratios (SMR) for Crohn's patients from 1970 to date. METHODS: Medline search identified relevant papers. Exploding references identified additional papers. When two papers reviewed mortality of one patient group at different times, the later publication was used. RESULTS: Of 13 papers identified, three studies reported SMR below 1.0, two others had confidence intervals including 1.0. All other studies reported mortality higher than the general population. Meta-analysis using a random effects model sho

Treatment of myasthenia gravis acute exacerbations with intravenous immunoglobulin.

The results of four randomized controlled trials were examined to assess the efficacy of intravenous immunoglobulin (IVIG) for myasthenia gravis (MG) acute exacerbations. The first trial compared plasma exchange (PE) and IVIG in 87 patients. At day 15, the mean change in the Myasthenic Muscular Score (MMS) was 16.6 (95% CI 11.6-21.6) in the PE group and 15.6 (95% CI 10.9-20.3) in the IVIG group (Wilcoxon signed rank test, P = 0.65). The second trial compared IVIG to oral methylprednisolone, and 33 patients were included. The mean (SD) sum of the two most pathological items of the Quantitative

Types of indwelling urinary catheters for long-term bladder drainage in adults.

BACKGROUND: Prolonged urinary catheterisation is common amongst people in long-term care settings and this carries a high risk of developing a catheter-related urinary tract infection and associated complications. A variety of different kinds of urethral catheters are available. Some have been developed specifically to lower the risk of catheter-associated infection, for example antiseptic or antibiotic impregnated catheters. Ease of use, comfort and handling for the caregivers and patients, and cost-effectiveness are also important factors influencing choice. OBJECTIVES: The primary objectiv
