The use of vasopressors during spinal anaesthesia for caesarean section.

PURPOSE OF REVIEW: Hypotension remains one of the most researched subjects in obstetric anaesthesia. The purpose of this study is to review the most recent published articles on the use of vasopressors during spinal anaesthesia for caesarean section. RECENT FINDINGS: Despite continued research indicating advantages of phenylephrine over ephedrine, practitioners in some countries continue to favour ephedrine. Recent research has continued to compare the two drugs with some work emerging on high-risk patients. Concern about reflexive bradycardia during phenylephrine use has led to consideration

Retrospective study of association between choice of vasopressor given during spinal anaesthesia for high-risk caesarean delivery and fetal pH.

BACKGROUND: Phenylephrine given during spinal anaesthesia for low-risk caesarean section is associated with higher fetal pH than ephedrine. However, there is little evidence on the effects of ephedrine and phenylephrine in complicated pregnancies. The aim of this study was to compare umbilical artery pH with phenylephrine and ephedrine given during spinal anaesthesia where caesarean section was performed because of an increased risk of fetal compromise. METHODS: We reviewed the case notes of all women at our hospital from 2000-2003 who had undergone high-risk caesarean section under spinal an

Anaesthesia for coronary artery bypass surgery supplemented with subarachnoid bupivacaine and morphine: a report of 18 cases.

We report our experience with general anaesthesia (GA) supplemented with subarachnoid bupivacaine and morphine for coronary artery bypass surgery (CABG) in 18 patients. Fifteen patients were male, and mean age was 62 yr. Anaesthesia (GA) was induced with alfentanil 97 +/- 22 micrograms.kg-1 and midazolam 0.04 +/- 0.02 mg.kg-1 supplemented with a muscle relaxant, and maintained with isoflurane (0.25-0.5%) in oxygen throughout surgery. Spinal anaesthesia (SA) was then performed at a lumber level using hyperbaric bupivacaine (23-30 mg) and/or lidocaine (150 mg) with morphine (0.5-1 mg). Pooled da

Obstetric analgesia and anaesthesia in women with inherited bleeding disorders.

A retrospective review was carried out on the methods of obstetric analgesia/anesthesia used in 80 pregnancies amongst 63 women with inherited bleeding disorders (19 factor XI deficiency, 16 carriers of haemophilia, 15 von Willebrand disease, seven platelet function disorders, four factor VII deficiency, one factor VII and XI deficiency and one factor X deficiency). In 72 pregnancies, the woman was seen antenatally in a multidisciplinary clinic to discuss and plan pain relief options. Regional block was performed for 41 pregnancies. The mothers were known to have a bleeding disorder in 35 of t

Aspergillus meningitis following spinal anaesthesia for caesarean section in Colombo, Sri Lanka.

We report six cases of Aspergillus meningitis after spinal anaesthesia for caesarean section administered in June and July 2005. Three patients died before a fungal infection was confirmed at the first post-mortem examination in August. Thereafter anti-fungal therapy was successful in saving the lives of the other three patients. Some syringes and spinal needles supplied to the hospitals concerned were found to be contaminated with Aspergillus fumigatus. Investigators found that medical supply storage facilities were substandard following the influx of donations after the tsunami of December 2

Efficacy and Safety of Unilateral Interlaminar Endoscopic Decompression for Lumbar Spinal Stenosis: A Retrospective Study of 176 Cases With a 3-6 Year Follow-Up.

OBJECTIVE: Spinal endoscopic surgery is widely acknowledged as an effective and minimally invasive approach for treating lumbar disc herniation. Comprehensive descriptions of the endoscopic decompression technique for lumbar spinal stenosis (LSS) are limited in existing literature. With a focus on long-term follow-up outcomes, this study investigates the safety and efficacy of endoscopic decompression using a unilateral interlaminar approach. METHODS: Between August 1, 2018, and December 1, 2020, a total of 316 consecutive cases underwent endoscopic decompression for LSS following conservativ

Thoracoscopy in children less than 20 kg for the management of spinal disorders: efficacy at long-term follow-up.

BACKGROUND: Thoracoscopy is now considered a safe and feasible method for surgical management of spinal disorders in both adults and children. Nevertheless, a weight less than 20 kg has been reported as a relative contraindication because of the small volume of the thoracic cage and the anticipated difficulties of single-lung ventilation. The aim of this study was to evaluate the feasibility, as well as the safety and efficacy, of thoracoscopic procedures in such patients. METHODS: This study was a retrospective analysis of a consecutive group of patients less than 20 kg weight, who underwent

Percutaneous Instrumentation of a Complex Lumbar Spine Fracture with Bilateral Pedicle Dissociation: Case Report and Technical Note.

BACKGROUND AND STUDY OBJECTIVE: Complex traumatic lumbar spine fractures are difficult to manage and typically occur in younger patients. Surgical immobilization for unstable fractures is an accepted treatment but can lead to future adjacent-level disease. Furthermore, large variations in fracture morphology create significant difficulties when attempting fixation. Therefore, a surgical approach that considers both long-term outcomes and fracture type is of utmost importance. We present a novel technique for percutaneous fixation without interbody or posterolateral fusion in a young patient wi

[Cerebrospinal fluid leakage. Indications, technique and results of treatment with a blood patch].

BACKGROUND: In most cases cerebrospinal fluid (CSF) leaks are iatrogenic and caused by medical interventions, such as lumbar puncture, peridural anesthesia and surgical interventions on the spine, However, spontaneous cerebral hypotension is currently detected more frequently due to improvements in diagnostic possibilities but often the cause cannot be clarified with certainty. METHODS: There are various diagnostic tools for confirming the diagnosis and searching for the site of CSF leakage, such as postmyelography computed tomography (postmyelo-CT), indium(111) radioisotope cisternography an

The Influence of Vasopressor-Induced Arterial Blood Pressure Elevation on Muscle-Recorded Motor Evoked Potentials.

BACKGROUND: Transcranial electrical stimulation muscle-recorded motor evoked potentials (Tc-mMEPs) are used to monitor the spinal cord motor tracts during spinal surgery. There is considerable intra- and interindividual variability in the signals recorded, causing a high incidence of false positive warnings. Inadequate blood pressure is commonly blamed for false positive warnings and is usually managed with fluid and vasopressor therapy. The aim of the study was to systematically investigate the effects of norepinephrine and ephedrine-induced arterial blood pressure elevation on Tc-mMEPs. MET

Cervical spine clearance in pediatric trauma patients - Consensus algorithm of the Pediatric Spinal Trauma Group of the Spine Section of the German Society for Orthopedic and Trauma Surgery (DGOU).

PURPOSE: Although cervical spine injuries in pediatric patients are rare, clinical and radiological examinations are clinical routine. Currently, no standardized decision algorithm does exist for detection of these injuries in the European region. The proposed algorithm is aiming to safely identify significant injuries through consistent diagnostics while minimizing immobilization and radiation exposure at the same time. METHODS: The Pediatric Spinal Trauma Group of the Spine Section of the German Society for Orthopedic and Trauma Surgery (DGOU) developed an algorithm based on own scientific

Blood pressure responses to captive bolt and electrical stunning in sheep and cattle: A systematic review.

Cattle and sheep are often slaughtered after electrical stunning or captive bolt stunning utilizing pneumatic or cartridge-driven devices. While the procedure is effective, a percentage of animals regain consciousness after the initial shock. One factor that has not been considered to explain this recovery of consciousness is the effect on systemic arterial blood pressure (BP) - the major determinant of cerebral perfusion. A systematic literature search identified papers measuring BP after captive bolt or electrical stunning. BP increases were observed in all studies except a protocol which us

Pediatric Neck Impalement Injuries: A Report of Two Cases and Review of the Literature.

Penetrating neck impalement injuries in children are rare but potentially life-threatening. We present two cases to illustrate the clinical spectrum of this injury pattern and reinforce key management principles. Case 1 was a 13-year-old female patient who sustained a wooden stick impalement to the right neck during a sledding accident. She was hemodynamically stable with a Glasgow Coma Scale (GCS) score of 15 and no neurological deficits. Computed tomography (CT) angiography showed a 2.5 cm soft tissue laceration with no vascular or aerodigestive injury. Case 2 was a 17-year-old female patien

Case Report: Transanal extraction combined with wound management for a broken glass foreign body and associated rectal injuries.

BACKGROUND: Rectal foreign bodies (RFBs) have become a relatively common yet often underestimated clinical entity. Fragile and sharp objects like glass pose a perplexing challenge, as unsafe removal may lead to complications or even life-threatening injuries. This report details the management of a complex case involving a broken glass rectal foreign body with associated rectal injuries. CASE PRESENTATION: A 55-year-old male was brought to the emergency department complaining of rectal fullness and bleeding after inserting a glass cup. Plain abdominal X-ray film showed an irregular dense shad

Presumed Amniotic Fluid Embolism Complicated by Disseminated Intravascular Coagulation and Refractory Postpartum Hemorrhage: A Case Report and Narrative Review.

Amniotic fluid embolism (AFE) is a rare but catastrophic obstetric emergency characterized by sudden cardiorespiratory collapse, disseminated intravascular coagulation (DIC), and a high case-fatality rate. Because no confirmatory test exists, the diagnosis remains clinical and one of exclusion. We report a case of presumed AFE in a 39-year-old primigravida with uterine myomas, obesity, and chronic hypertension who underwent an elective primary cesarean delivery under spinal anesthesia. During manipulation of the placenta, the patient developed abrupt cardiovascular collapse requiring cardiopul

A systematic review and narrative synthesis of functional neuroimaging studies for chronic ocular surface pain.

Chronic ocular surface pain (COSP) is a debilitating condition of the eye driven by nociceptive, nociplastic, and neuropathic mechanisms. While individual studies using functional neuroimaging techniques to investigate ocular pain exist, the full extent, methodologies, and cumulative findings of this body of literature have not been systematically synthesized. Using a systematic review approach, we identified 5 articles evaluating the use of functional neuroimaging in COSP patients from an initial pool of 643 unique studies. All studies used functional magnetic resonance imaging exclusively as

Comparison of proximal versus distal approach for ultrasound-guided obturator nerve block for prevention of adductor spasm in patients undergoing transurethral resection of urinary bladder tumors.

BACKGROUND AND AIMS: Adductor muscle spasm occurs when the obturator nerve is directly stimulated by the electrical current transmitted by the resectoscope. Ultrasound-guided ONB is regarded as the optimal technique and is classified as the proximal and distal approach based on the anatomical location of the obturator nerve. MATERIAL AND METHODS: This study included 90 adult patients of ASA grades I-III and aged more than 18 years undergoing transurethral resection of bladder tumors under spinal anesthesia. Patients were randomized into two groups: group P received ultrasound-guided obturator
