Co-creation as a knowledge mobilization process in digital health regulation in East Africa: The making of the DigiReg compass.

Regulating digital health in low- and middle-income countries requires policy-makers to organize and use multiple forms of knowledge, including academic research, technical expertise and experiential insights, to make tough decisions under conditions of existential uncertainty. We examine co-creation as a mode of knowledge mobilization in digital health regulation in Kenya, Rwanda and Uganda. Using content analysis and a multi-country stakeholder workshop, we trace how regulators, clinicians, technologists and civil society actors collectively interpreted comparative evidence on existing frame

Metronomic chemotherapy in resource-limited settings: evidence, opportunities, and future directions.

Metronomic chemotherapy-frequent, continuous delivery of low-dose chemotherapeutic drugs without drug-free breaks-is a promising treatment option for resource-limited situations. This narrative review analyzes the scientific justification, clinical evidence, cost-effectiveness, implementation challenges, and prospective developments of metronomic chemotherapy in low- and middle-income countries (LMICs), which account for 80% of childhood cancers and a rising incidence of adult malignancies. Metronomic regimens provide accessible and cost-effective therapy alternatives by targeting tumor vascul

Global Collaboration in Cystic Fibrosis: Lessons From the COVID-19 Pandemic for Low- and Middle-Income Countries.

BACKGROUND: Cystic fibrosis (CF) is a genetic disease that affects individuals across the globe, yet people with CF (pwCF) in low- and middle-income countries (LMICs) have historically been under-represented in research due to diagnostic barriers, limited resources, and lack of registry infrastructure. AIMS: The COVID-19 pandemic created an urgent need for real-time, standardized data on SARS-CoV-2 infection in pwCF. The aim of this work was to determine the impact of infection on the CF community. METHODS: Leveraging existing international relationships and national registries, CF leaders r

Identification, Prevention, and Treatment of Perinatal Depression in Low- and Middle-Income Countries: A Narrative Review.

Perinatal depression is more prevalent in low- and middle-income countries (LMICs) than in high-income countries, reflecting greater exposure to physical, psychosocial, and environmental adversities. It can have lasting consequences for the woman, her child, and wider family. Despite its high burden, access to appropriate mental health care for perinatal depression remains extremely limited in most LMICs. In this narrative review, we conducted a search of PubMed and Google Scholar from 2015 to 2024 (with the addition of other key articles based on authorship team consensus) for recent and poli

Community-Based Impact of Community-Based Antenatal Care Models on Prevention and Early Detection of Obstetric Complications: A Systematic Review.

Obstetric complications remain a leading cause of preventable maternal and neonatal morbidity and mortality in low- and middle-income countries (LMICs). Persistent barriers to facility-based antenatal care (ANC), including geographic remoteness, transportation costs, socio-cultural norms, and fragmented health systems, have prompted the large-scale expansion of community-based ANC models designed to bring preventive and early-detection services directly to underserved populations. This systematic review evaluated the impact of community-based ANC models on the prevention and early detection of

Inequalities in Severe Maternal Morbidity and Mortality in High-Income Countries: Patterns, Drivers, and Pathways to Action.

Inequalities in severe maternal morbidity (SMM) and mortality in high-income countries are persistent, socially patterned, and evident across multiple dimensions, including socioeconomic deprivation, race and ethnicity, and migration status. These inequalities are not fully explained by individual clinical risk factors but arise from the interaction of structural disadvantage, intermediate social conditions, and health systems. Many determinants of risk are established before pregnancy; however, variation in access to care, quality of care, and responsiveness to symptoms during pregnancy and c

Epidemiology and Control of Foodborne Diseases in Resource-Limited Settings: Insights from Afghanistan.

Food safety is a major global public health concern, with unsafe foods contaminated by biological, chemical, and physical hazards causing foodborne diseases (FBDs), resulting in a high global burden of illness and death. This narrative review provides an overview of current evidence on the epidemiology, risk factors, and control strategies of FBDs, with particular attention to challenges in resource-limited settings. These diseases are disproportionately severe in low- and middle-income countries (LMICs) due to interconnected factors such as poor sanitation, limited access to clean water, inad

HIV broadly neutralizing antibodies for children: implementation barriers in low-income settings.

PURPOSE OF REVIEW: Broadly neutralizing monoclonal antibodies (bNAbs) represent a transformative frontier for the prevention, treatment, and potential remission of HIV in children. Given that over 95% of the pediatric population living with or at risk for HIV resides in low-income and middle-income countries (LMICs), prioritizing trials and implementation in these regions is an ethical and clinical imperative. This review identifies the primary structural, biological, and regulatory barriers that must be addressed to ensure equitable global access to bNAb-based interventions. RECENT FINDINGS:

Health-seeking behaviors of pregnant women with hypertensive disorders of pregnancy in low- and middle-income countries: A scoping review protocol.

Hypertensive disorders of pregnancy (HDP) are among the leading causes of maternal and perinatal illness and death worldwide. Despite advances in its management, the burden of HDP remains disproportionately high in Low- and Middle-Income Countries (LMICs), where issues related to healthcare access, cultural beliefs, and systemic inefficiencies hinder timely diagnosis and intervention. Health-seeking behavior is crucial for preventing severe complications linked to HDP. An initial review of the literature was conducted and revealed no scoping or systematic reviews on the health-seeking behavior

UK aid cuts threaten global child health: a call to rethink international assistance.

Recent reductions in UK aid have demonstrably harmed child health in low-income and middle-income countries. Increased child morbidity and mortality are directly linked to funding cuts impacting vaccination programmes, maternal and child health services and nutritional interventions, exacerbating the health crisis. This article analyses these impacts, criticises systemic flaws in the aid architecture, particularly the impact of the abolition of Department for International Development and advocates for a decolonised, localised and accountable aid system prioritising child health.

Global disparities and data transparency in editorial boards of prominent Internal medicine journals including allied super specialities: A wake-up call.

BACKGROUND: Global healthcare research suffers from inequalities, favouring high-income countries, which hampers fair healthcare access. Low and lower-middle-income nations face limited participation and editorial bias, posing concerns for research integrity. Potential reasons for this pattern might be the overrepresentation of researchers from the Global North on editorial boards, disparities in the quality of scientific research, lack of support for local research initiatives, and challenges posed by language barriers. The aim of this study was to assess data transparency and representation
