Closed-Wedge Osteotomy of the Distal Humerus for Treating Osteochondritis Dissecans of the Capitellum in Young Patients.

BACKGROUND: Even in patients with an open capitellar physis, nonsurgical treatment for advanced-stage osteochondritis dissecans (OCD) of the capitellum often yields poor outcomes. However, surgical methods for such patients are controversial. At our institution, we have consistently performed closed-wedge osteotomy of the distal humerus to treat advanced-stage OCD of the capitellum, regardless of the OCD grade or condition of the capitellar physis. PURPOSE: To clarify the clinical and radiological results and determine the influence of the capitellar physis on closed-wedge osteotomy for advan

Skeletal phenotype/genotype in progressive pseudorheumatoid chondrodysplasia.

BACKGROUND: Axial and extra-axial deceleration in function and progressive joint pain with subsequent development of antalgic gait associated with swellings, and stiffness of the joints with loss of the physiological spine biomechanics were the natural history in this group of patients. Clinical and radiological phenotypes have been analysed carefully to further understand the aetiology behind. METHODS: Seven patients (three children around the age of 9-11 and one child of 17 years old). Three adults aging 25, 30, 33 and 40 years old were seen and examined. The paediatric group of patients we

A morphometric study of the middle and lower cervical vertebral endplates and their components.

Cervical disc arthroplasty is a common method of treating cervical degenerative disease. However, the footprints of most prosthesis dimensions are obtained from data of Caucasian individuals. Besides, there is a large discrepancy between footprints of currently available cervical disc prostheses and anatomic dimensions of cervical endplates. We aimed to detail the three-dimensional (3D) anatomic morphology of the subaxial cervical vertebral endplate, utilizing high-precision, high-resolution scanning equipment, and provide a theoretical basis for designing appropriate disc prostheses for Chine

Proximal Tibial Epiphysis Injury (Flexion Type, Salter-Harris Type 1).

INTRODUCTION: Fractures of the proximal tibial epiphysis rare. It has been estimated that fractures of the upper tibial epiphysis account for 0.5-3.1% of all epiphyseal injuries. Who had no neurovascular deficit, with fixed extension deformity at the left knee was treated early with closed reduction techniques. CASE REPORT: We present a case of a 16-year-old boy who while playing cricket on the road was hit by a car. The patient presented in emergency room with extremely swollen knee and soft tissue swelling (hemarthrosis), he was unable to lift his leg actively due to severe pain because of

Effect of a mixture of GOS/FOS® on calcium absorption and retention during recovery from protein malnutrition: experimental model in growing rats.

INTRODUCTION: During growth, protein deprivation impairs epiphyseal growth plate (EGP) height, bone volume (BV) and endochondral ossification. During catch-up growth, Ca availability becomes essential to ensure the extra amount needed to achieve optimal peak bone mass and strength. GOS and FOS improve mineral absorption in the colon. PURPOSE: The effect of a mixture of GOS/FOS® 9:1 added to a 0.5 %Ca (NCa) and a 0.3 %Ca (LCa) diets on Ca, P and Mg absorptions and bone mineralization, density and structure using an experimental model of growing rats recovering from early protein malnutrition w

Bone morphogenetic protein-associated complications in pediatric spinal fusion in the early postoperative period: an analysis of 4658 patients and review of the literature.

OBJECT: Use of recombinant human bone morphogenetic protein-2 has risen steadily since its approval by the FDA for use in anterior lumbar interbody fusion in 2002. The FDA has not approved the use of bone morphogenetic protein (BMP) in children. Age less than 18 years or lack of evidence of epiphyseal closure are considered by the manufacturer to be contraindications to BMP use. In light of this, the authors performed a query of the database of one of the nation's largest health insurance companies to determine the rate of BMP use and complications in pediatric patients undergoing spinal fusio

The role of alpha 6 integrin in prostate cancer migration and bone pain in a novel xenograft model.

Of the estimated 565,650 people in the U.S. who will die of cancer in 2008, almost all will have metastasis. Breast, prostate, kidney, thyroid and lung cancers metastasize to the bone. Tumor cells reside within the bone using integrin type cell adhesion receptors and elicit incapacitating bone pain and fractures. In particular, metastatic human prostate tumors express and cleave the integrin A6, a receptor for extracellular matrix components of the bone, i.e., laminin 332 and laminin 511. More than 50% of all prostate cancer patients develop severe bone pain during their remaining lifetime. On

Disease-causing mutations in cartilage oligomeric matrix protein cause an unstructured Ca2+ binding domain.

Chondrocytes from pseudoachondroplasia (PSACH) and multiple epiphyseal dysplasia (EDM1) patients display an enlarged rough endoplasmic reticulum that accumulates extracellular matrix proteins, including cartilage oligomeric matrix protein (COMP). Mutations that cause PSACH and EDM1 are restricted to a 27-kDa Ca(2+) binding domain (type 3 repeat). This domain has 13 Ca(2+)-binding loops with a consensus sequence that conforms to Ca(2+)-binding loops found in EF hands. Most disease-causing mutations are found in the 11-kDa C-terminal region of this domain. We expressed recombinant native and mut

Mutations in the diastrophic dysplasia sulfate transporter (DTDST) gene (SLC26A2): 22 novel mutations, mutation review, associated skeletal phenotypes, and diagnostic relevance.

Mutations in the DTDST gene can result in a family of skeletal dysplasia conditions which comprise two lethal disorders, achondrogenesis type 1B (ACG1B) and atelosteogenesis type 2 (AO2); and two non-lethal disorders, diastrophic dysplasia (DTD) and recessive multiple epiphyseal dysplasia (rMED). The gene product is a sulfate-chloride exchanger of the cell membrane. Inactivation of the sulfate exchanger leads to intracellular sulfate depletion and to the synthesis of undersulfated proteoglycans in susceptible cells such as chondrocytes and fibroblasts. Genotype-phenotype correlations are recog

Distal ulna physeal fracture Salter-Harris type IV versus cleft ulnar epiphysis: Two pediatric case report.

Forearm fractures in children are among the most common fractures however isolated distal ulnar physeal fractures are rare, especially Salter-Harris type IV fracture involving epiphysis, physis and metaphyseal parts. Growth disturbances are not uncommon in such fractures. Also, certain anatomical variants, including cleft ulna epiphysis, may mimic fracture lines on radiographs, unnecessary operational intervention may result from misinterpretation. We describe 2 cases the first of a 12-year-old child who had sustained a bilateral wrist trauma after falling from a bicycle. A left distal forearm

Radiographic and patient-specific predictors of poor outcome following hip reconstruction in children with cerebral palsy.

AIMS: Patients with spastic cerebral palsy (CP) are at a high risk of neurogenic hip dysplasia/subluxation depending on the severity of the neuromuscular disorder. Untreated, approximately one third of all patients develop hip dislocation. Reconstruction with femoral varus derotational osteotomy (VDRO) combined with Dega acetabuloplasty (PO) represents the gold standard. The goal of this study was the radiographic assessment after reconstructive treatment of spastic hip dysplasia/(sub)luxation and to derive specific thresholds and target values of neck shaft angle NSA) and femoro-epiphyseal ac

Direct anterior approach for open correction of Slipped Capital Femoral Epiphysiolysis: surgical technique and clinical outcome.

BACKGROUND: Slipped Capital Femoral Epiphysis (SCFE) is an idiopathic hip disorder in adolescents. With or without surgery, avascular necrosis (AVN) is the most dangerous complication. For open reduction and internal fixation, the modified Dunn procedure is considered the gold standard. This study presents an open epiphyseal realignment through the Hueter-interval and reports on (1) surgical technique of direct anterior approach (DAA) based surgical realignment, (2) radiographic correction of biomechanics, and (3) AVN-incidence after surgery. METHODS: A single-center retrospective case series

Case Report: A novel PTHLH nonsense variant in a mother-son pair with brachydactyly type E and short stature, with a genotype-stature review.

BACKGROUND: Brachydactyly type E is a rare skeletal disorder characterized by variable shortening of the metacarpals and/or metatarsals. Heterozygous pathogenic variants in PTHLH, which encodes parathyroid hormone-related protein, are an established cause of autosomal dominant brachydactyly type E and are variably associated with short stature. CASE PRESENTATION: We report a mother-son pair with brachydactyly type E and short stature caused by a novel heterozygous nonsense variant in PTHLH. The proband, a boy aged 7 years and 8 months, presented with short stature, brachydactyly involving bot

Outcome and Femoral Head Deformity Following Hip Guided Growth in Children With Cerebral Palsy at Skeletal Maturity.

BACKGROUND: Guided growth of the proximal femur, a minimally invasive procedure for coxa valga, shows promising short-term outcomes in cerebral palsy (CP). However, as it alters physis growth, existing studies lack comprehensive long-term analysis until skeletal maturity. METHODS: This retrospective study included children with spastic CP who underwent proximal femur-guided growth surgery between 2012 and 2017, followed until physeal closure. Radiographic measurements included head-shaft angle (HSA), Hilgenreiner-epiphyseal angle (HEA), acetabular index (AI), Reimer's migration percentage (MP

Hereditary multiple exostoses: an educational review.

Hereditary multiple exostoses (HME), an autosomal dominant disorder with an incidence of 1:50,000 to 1:100,000, is characterised by the formation of multiple osteochondromas arising from the metaphyses of long and flat bones. These osteochondromas often present as painless palpable lumps, though some cases are symptomatic due to mechanical compression or bursitis. Diagnosis of HME is typically clinical and radiological. WHO diagnostic criteria include ≥ 2 radiological osteochondromas in the juxta-epiphyseal region of the long bones. Genetic testing is reserved for ambiguous cases. HME is assoc

Comparison of Contrast-enhanced Ultrasound and Contrast-enhanced Magnetic Resonance Imaging in the Assessment of Infant Hip Perfusion and Prediction of Proximal Femoral Growth Disturbance Following Closed and Open Reduction for Developmental Dysplasia of the Hip: A Preliminary Study.

OBJECTIVE: Proximal femoral growth disturbance (PFGD) is a significant complication associated with surgical treatment of infant hip dislocation. Contrast-enhanced magnetic resonance imaging (CEMRI) has been utilized to assess perfusion in these hips and avoid PFGD. Contrast-enhanced ultrasound (CEUS) is an imaging technique utilized to evaluate perfusion in other organs. The aims of this study were to compare perfusion of dysplastic infant hips with CEUS and CEMRI after surgical treatment and to determine whether CEUS was as effective as CEMRI at predicting PFGD. METHODS: A retrospective ana
