This study presents 100 consecutive cases of total salpingectomy by laparoscopy for the treatment of ampullar ectopic pregnancy. A three-puncture technique was used, and the salpingectomy was done by thermocoagulation and transection of the isthmus, mesosalpinx, and tubo-ovarian ligament. Tubal extraction was accomplished by one of the suprapubic punctures with the use of a polyp forceps. No complications have been encountered during the operation. Diffusion of aztreonam into female genital tract tissues was investigated. After a single IV injection of 1 g, aztreonam was assayed in uterine tissue, uterine tube tissue and peritoneal fluid in 15 patients undergoing coelioscopy or hysterectomy. In addition, to evaluate placental transfer of aztreonam, maternal blood, amniotic fluid and cord blood samples were collected in five women undergoing cesarean section. We describe 3 sibs, two females and a male, with hypogonadism, defective Müllerian development in the sisters, and partial alopecia consisting of cranial hair only in the center of the scalp. One sister had absent gonads, the other had streak ovaries; both had markedly hypoplastic internal genitalia. Their brother had hormonal and histologic findings consistent with germinal cell aplasia. In view of the fact that the parents were consanguineous, autosomal recessive inheritance of the syndrome is likely.

The histopathology of 110 cases of tubal gestation was reviewed. No evidence was found of the development of the gestation into the tubal muscularis and beneath the overlying peritoneum. The localized tubal mass identified clinically was secondary to luminal distention. It was clear that tubal rupture occurred secondary to localized distention and vascular compromise with hemorrhagic necrosis and was not secondary to invading chorionic villi. In a case of fallopian pregnancy after a microsurgical performance for fertilisation we obtained the diagnosis by means of ultrasound. We saw the intact gestational sac and the embryo directly and point out the importance of early sonographical screening of pregnancy after surgical treatment of infertility. Two cases of women with actinomycotic tubo-ovarian abscesses are reported. Both of the patients were users of intrauterine contraceptive devices. They were successfully treated by abdominal hysterectomy with salpingooophorectomy--in one cases appendectomy in addition--and pelvic drainage combined with antibiotic therapy. Aspects of nature, diagnosis, mode of infection and therapy of actinomycosis are discussed.

Review of sonograms in 84 patients with documented tubal pregnancies yielded 15 cases with discrete, diffusely echogenic, adnexal masses (18%). All these cases were proven at surgery to represent hematosalpinx containing clotted blood. A case of pregnancy in the rudimentary horn of a bicornuate uterus is presented, and a discussion of this rare and potentially catastrophic phenomenon follows. This is the first such case reported in which the diagnosis was made early in gestation through the use of ultrasonography. An extrauterine gestation accompanied by a well-defined placenta visible on ultrasound scan is suggested as a criterion for differentiating rudimentary horn pregnancy from abdominal pregnancy.

Enterobius (Oxyuris) vermicularis is the only nematode which infests man. It inhabits the large intestine and is of low pathogenicity. The ova are occasionally found in ectopic sites within the peritoneal cavity, usually as an asymptomatic granuloma. We describe the first patient to present with generalised peritonitis caused by rupture of a tubo-ovarian abscess containing the ova of Enterobius vermicularis.

The role of microsurgery in reversal of sterilization has been reassessed and 118 cases reviewed. The technical variations for tubo-tubal anastomosis necessitated by the condition of the oviducts have been outlined in detail. In the five-year period 1974 to 1978, there were 23,256 deliveries and 204 ectopic pregnancies at Fairfax Hospital. Forty-six patients (22.5%) had had previous pelvic surgery, including 14 salpingectomies for previous ectopic pregnancies.
