Current practice in prescribing footwear and insoles to reduce the risk of neuropathic plantar forefoot ulceration and re-ulceration in people with diabetes.

BACKGROUND: Footwear and insoles are frequently used to offload the neuropathic diabetic foot at risk of ulceration and re-ulceration. This study aims to explore the prescription habits of Australian pedorthists and the variations in footwear and insole design due to variations in foot pathology, comorbidities and patient-specific factors consideration. METHODS: An online survey link was sent to Australian pedorthists (n = 42), who were asked to answer questions relating to four hypothetical cases exploring current footwear and insole prescribing habits. Qualtrics research and data analysis s

"If I have any problems, I just want to see my [primary care] doctor": perspectives on a proposed collaborative RUral PALliative care intervention (RuPal).

BACKGROUND: People living with advanced heart failure in rural areas have poorer quality of care as their disease progresses, which may be due to lack of access to specialty palliative care. A collaborative care model, connecting specialty palliative care clinicians to embedded complex care teams in primary care practices, may increase access to palliative care in this population. Research objectives To explore perspectives on a proposed collaborative palliative care intervention and whether this intervention would be appropriate for people living with heart failure in rural areas. METHODS: W

Attitudes of anesthesiologists towards implementation of PENG block in non-operative treatment of hip fractures in the Netherlands: a national survey study.

BACKGROUND: Effective pain management in frail patients with hip fractures who are not candidates for surgery remains challenging. The pericapsular nerve group (PENG) block with phenol has been proposed as a minimally invasive option in this context. However, its integration into palliative care pathways is not well established. This study evaluated how Dutch anesthesiologists perceive the PENG block with phenol for non-operative pain management and explored the current state of its implementation. METHODS: We conducted a national cross-sectional survey among all registered anesthesiologist-p

Assessing and Improving Access to Health and Social Care Services for Children Rendered Vulnerable by Abuse: Protocol for a Cross-Sectoral Longitudinal, Mixed Methods, Multi-Country Study Using Nationwide Data in Europe.

Child maltreatment (CM) is a widespread and underreported public health concern with long-term health and wellbeing outcomes. In Europe, access to timely, effective support remains limited. Inadequate responses exacerbate long-term outcomes and influence life course trajectories, with substantial societal and economic costs. The EU-funded SERENA project aims to improve access to health and social care (HSC) services for individuals who experience CM throughout Europe by enhancing detection and interventions, limiting consequences, and reducing societal burdens. SERENA takes an early life cours

Impact of Leadership Structures on Sterile Processing Performance and Patient Safety.

Background: Sterile processing departments (SPDs) are foundational to perioperative safety and play a critical role in preventing surgical-site infections and ensuring procedural reliability. Despite this essential function, SPD professionals often report limited organizational visibility and constrained decision-making authority. Many also experience insufficient integration into formal governance structures. Empirical literature examining the relationship among reporting structures, professional autonomy, and patient safety outcomes in SPDs remains limited. Methods: A convergent mixed-method

Motivations to Enter and Remain in Community Nursing: Implications for Workforce Sustainability.

AIM: To explore what motivates nurses to enter and remain in community nursing within Austria's national pilot project and to examine how autonomy, competence and relatedness shape their professional experiences and retention intentions. BACKGROUND: Community nursing has been introduced in Austria as an innovative, preventive, community-based model of care. As nursing systems face growing workforce shortages, understanding why nurses choose to transition into and remain in new nursing roles is critical for workforce sustainability and effective service implementation. METHODS: A qualitative

Clinical Practice Guideline: The Diagnosis, Treatment, and Follow-Up Management of Thrombotic Thrombocytopenic Purpura.

BACKGROUND: Thrombotic thrombocytopenic purpura (TTP) is a life-threatening thrombotic microangiopathy. The global incidence of immune-mediated TTP (iTTP) is 1-6 cases per million people per year. The new (S3-level) clinical practice guideline is the first in the German-speaking world to compile evidence- and consensus-based recommendations on the diagnosis, treatment, follow-up management, and care of patients with immune-mediated thrombotic thrombocytopenic purpura (iTTP) and congenital thrombotic thrombocytopenic purpura (cTTP). METHODS: This clinical practice guideline was developed under

Prevalence and risk factors of stress urinary incontinence among female horseback riders in Poland.

Stress urinary incontinence (SUI) is usually associated with aging and parity, but repetitive intra-abdominal pressure during physical activity can also trigger its onset in young women. Horseback riding, despite potential benefits for pelvic floor strengthening, may also contribute to pelvic floor strain and urinary symptoms. The study included 100 Polish women aged 21-54 engaged in horse riding, both recreational (85%) and professional (15%). Participants were divided into three age groups and screened for urinary incontinence symptoms. Quality of life was assessed with validated questionnai

The landscape of cognition and behaviour in the epilepsies: from neuropsychology to lived experience.

Neurobehavioral research in epilepsy has evolved into a broad, multidisciplinary field spanning the full human lifespan. Despite substantial progress, however, its conceptual organization remains unclear. We propose a unifying framework-a "landscape"-that defines the field's structure and clarifies relationships among its major clinical and research domains. Rooted empirically in neuropsychology, but expanding to encompass the wider neurobehavioral dimensions of epilepsy, this framework adopts a lifespan perspective and organizes the field into six dimensions: (1) clinical cognitive domains, (

Impact of Preoperative Semaglutide Discontinuation Timing on Postoperative Outcomes in Aesthetic Abdominoplasty: A Retrospective Comparative Study.

BACKGROUND: The widespread adoption of GLP-1 receptor agonists such as semaglutide for weight loss has led to an increasing number of non-diabetic patients seeking body contouring procedures after pharmacologic weight reduction. However, concerns have emerged regarding postoperative complications potentially linked to GLP-1 therapy, particularly when continued up to the time of surgery. OBJECTIVES: This retrospective cohort study aimed to evaluate the impact of preoperative semaglutide discontinuation timing on short-term postoperative outcomes in aesthetic lipoabdominoplasty. METHODS: Eight

How Effective are Nanotechnology-Based Therapeutics to Treat Autoimmune Diseases.

Autoimmune diseases are chronic, debilitating conditions caused by the immune system mistakenly attacking healthy tissues. Conventional treatments mainly involve broad immunosuppression, which is associated with significant side effects, limited specificity, and suboptimal long-term outcomes. For instance, continuing to take corticosteroids can result in a number of serious dose-linked toxicities, such as osteoporosis, hypertension, and a markedly increased susceptibility to infection, whereas methotrexate, even at therapeutic doses, can still cause liver damage and bone marrow suppression. A narrative review on the neurobiology of lucid dreaming: mechanisms and therapeutic potential.

Источник пометил публикацию как отозванную

BACKGROUND: Lucid dreaming (LD), which involves being aware of and controlling one's dreams, is of particular interest in the study of consciousness and has potential therapeutic applications. In this narrative review, we highlight the neurobiological aspects of LD, specifically its mechanisms associated with the activation of prefrontal and parietal areas, increased gamma oscillations, and the influence of cholinergic and dopaminergic signaling, primarily during rapid eye movement (REM) sleep. OBJECTIVE: To evaluate human data on brain mechanisms and clinical effects, synthesizing the neurob

Unraveling the oral microbiome's role in Alzheimer's disease: From pathophysiology to therapeutic potential.

Oral dysbiosis contributes to Alzheimer's disease (AD) by promoting neuroinflammation. Pathobionts such as Porphyromonas gingivalis, Treponema denticola, and Fusobacterium nucleatum release virulence factors that induce amyloid beta aggregation and tau hyperphosphorylation, while the loss of commensals like Streptococcus salivarius and Neisseria spp. impairs anti-inflammatory protection, worsening neuronal damage. P. gingivalis is strongly linked to an increased risk of AD, especially in individuals with systemic conditions like diabetes, hypertension, and chronic kidney disease. Its presence

Fighting time: the critical importance of pre-TAVR mortality risk prediction.

BACKGROUND: Symptomatic severe aortic valve stenosis (AS) is a life-threatening condition requiring prompt medical attention. While transcatheter aortic valve replacement (TAVR) is an effective treatment, current scheduling practices often do not account for individual patient risk profiles due to limited data on mortality rates during the waiting period and a lack of viable risk assessment. Consequently, non-prioritized wait times may be unacceptably long for high-risk patient populations. OBJECTIVE: This study aimed to evaluate the mortality rate of patients with symptomatic severe AS await
