OBJECTIVES: Pulmonary valve replacement (PVR) is performed to reduce right ventricular (RV) volume overload, resulting in improved ventricular function and clinical status. Significant pulmonary regurgitation (PR) after tetralogy of Fallot (TOF) repair could result in RV dysfunction, exercise intolerance, arrhythmia and sudden death. The present study was conducted to investigate the mid-term clinical outcomes of PVR after TOF repair.
METHODS: Between 2001 and 2010, we retrospectively reviewed the outcomes of 131 (89 males and 42 females) PVRs with PR or pulmonary steno-insufficiency after TO

BACKGROUND: Early acute right heart failure (eaRHF) during left ventricular assist device (LVAD) implantation significantly impacts patient survival and complicates perioperative management. Although numerous clinical, echocardiographic, and hemodynamic risk factors have been identified, accurately predicting eaRHF remains challenging. Cardiac magnetic resonance (CMR) provides a precise, non-invasive evaluation of cardiac structure and function and may enhance risk stratification eaRHF. This study aims to assess the predictive value of preoperative CMR-derived parameters, comparing their utili

Atrioventricular septal defect with common valvar orifice and tetralogy of Fallot is a rare combination of congenital cardiac anomalies. Approaches to this lesion have tended to emphasize either staged repair or complete repair beyond infancy. Between July 1992 and August 1997, nine patients underwent repair of complete atrioventricular septal defect with tetralogy of Fallot. One patient, aged 9.6 years at the time of repair, had previously undergone construction of a modified Blalock-Taussig shunt. Primary complete repair was performed in the other 8 patients at ages ranging from 2.5 to 16 mo

OBJECTIVE: Patients with complex congenital heart disease, characterized by right ventricle hypoplasia, had a palliative surgical option with one and a half ventricular repair.
METHODS: From July 2001 to March 2009, nine patients (mean age 5.2 years, range 3 to 9 years) with hypoplastic right ventricle, underwent correction with one and a half ventricle technique. Preoperative diagnoses included: pulmonary atresia with intact ventricular septum, in six and Ebstein's anomaly, in three cases. Six patients had bidirectional cavo-pulmonary shunt (Glenn operation) previously. The surgical approach

INTRODUCTION: The objective of this study was to analyse the influence of fluid restriction (FR) on right heart size and clinical parameters in pulmonary arterial hypertension (PAH) patients with right heart failure (RHF).
METHODS: In this prospective, single-centre, randomised, controlled study (Trial Registration: DRKS00029667), PAH patients with RHF and signs of volume overload were included. Intensified instructions for restriction of fluid intake to ≈1.5 l/d were compared to routine recommendation of FR. The primary endpoint was the change in right atrial (RA) and right ventricular (RV)

Urgent repair (within 48 hr after diagnosis) of posterior ventricular septal defect in the presence of cardiogenic shock, consequent to acute myocardial infarction, is associated with a very high mortality rate. The use of left ventricular mechanical support devices has the potential to impart hemodynamic stability and to delay surgical treatment until such time as scar tissue forms around the defect, sufficient to hold a suture patch. From May 2004 through July 2007, 5 patients who were in cardiogenic shock as a consequence of acute posterior ventricular septal defect underwent early implantat

OBJECTIVES: Although conduction location can be reliably predicted in double inlet ventricle, ventricular septation continues to carry a significant risk of complete heart block. This study describes our experience using intraoperative conduction mapping during ventricular septation.
METHODS: Patients undergoing ventricular septation from 2017 to 2023 were identified. Conduction mapping was performed on the open, decompressed, beating heart to identify the His bundle. Mapped His bundle locations were compared with those predicted from magnetic resonance imaging diagnoses by a senior pediatric

The left and right atria serve as buffer chambers to control the flow of venous blood for ventricular filling. If an atrium is absent, blood does not flow effectively into the ventricle, leading to venous blood retention and low cardiac output. The importance of atrial function has become increasingly recognized, because left atrial (LA) function contributes to cardiac performance, and loss of LA function is associated with heart failure. LA volume change has been used for LA function assessment in experimental and clinical studies. In conjunction with LA pressure, the LA pressure-volume relat

A 12-year-old child with no previous medical history was referred with a 4-day history of cough, shortness of breath, and peripheral blood eosinophilia. Transthoracic echocardiography showed a soft tissue infiltrating the left ventricular free wall, the lateral mitral annulus, and the mitral valve leaflets. A soft tissue strand connecting the lateral left atrial wall and mitral leaflets across the mitral valve orifice was also identified, causing reduced opening and functional mitral stenosis. The diagnosis of Löeffler endocarditis was made, and after 10 weeks of treatment with oral prednisolo

Reoperations can be challenging for patients who experience left and right atrioventricular valve failure after repair of a complete atrioventricular septal defect. Herein, we present a case of a 10-year-old boy who developed dysfunction in left and right atrioventricular valve following surgery for a complete atrioventricular septal defect. The patient underwent successful replacement of his left atrioventricular valve with a mechanical valve due to severe stenosis. Additionally, his right atrioventricular valve, in which the bridging leaflets adhered to the ventricular septal defect patch, c

A new operation to establish functionally normal circulation was performed in five neonates with hypoplastic left heart syndrome. With a limited period of cardiopulmonary bypass, deep hypothermia, and circulatory arrest, the patent ductus arteriosus was ligated. The atrial septum was excised and the atrium was repartitioned with a pericardial baffle so that pulmonary veins were in continuity with the tricuspid valve and right ventricle. The pulmonary artery was divided above the sinuses of Valsalva, and the proximal end was connected to the aortic arch with a tubular Dacron prosthesis. The dis

OBJECTIVES: Pulmonary arterial hypertension (PAH) is a life-threatening complication of SSc. Although selexipag is approved for SSc-PAH, available data are limited. We evaluated the long-term safety and clinical predictors associated with selexipag therapy in a multicentre Italian cohort of SSc-PAH patients.
METHODS: We retrospectively analysed consecutive SSc patients with PAH diagnosed by right heart catheterization and treated with selexipag. Survival, treatment persistence, 1-year mortality risk (COMPERA 2.0) and predictors of clinical outcomes were assessed using appropriate statistical

BACKGROUND: Pulmonary glue/iodized oil embolism is an uncommon form of non-thrombotic pulmonary embolism caused by migration of cyanoacrylate-based embolic agents. Although most cases have been reported after endoscopic treatment of gastric varices, cases after varicocele embolization are exceptionally rare, and longitudinal data on clinically stable patients remain limited.
CASE PRESENTATION: We report a 35-year-old man who developed acute dyspnea, chest pain, and palpitations immediately after varicocele embolization, mimicking thrombotic pulmonary embolism. Computed tomography pulmonary an

BACKGROUND: Temporary transvenous pacing is frequently required in inferior ST-elevation myocardial infarction (STEMI) complicated by high-grade atrioventricular block. Right ventricular perforation is a rare but life-threatening complication that may present after wire removal rather than at insertion, complicating recognition. Management is particularly challenging in district general hospitals (DGHs) without on-site cardiothoracic surgery.
CASE SUMMARY: A 62-year-old woman presented in cardiogenic shock to a DGH with inferior STEMI and complete atrioventricular block requiring emergency te

Takayasu arteritis (TAK) is a rare, chronic large-vessel vasculitis that primarily affects young females. Neurological and cardiac involvement in paediatric TAK is uncommon, and their concurrent occurrence is even rarer. We report a unique case of TAK in a girl in middle childhood who presented with progressive abdominal distension, right-sided hemiparesis, right upper limb dystonia and right-sided upper motor neuron (UMN) facial palsy. Over the preceding 6 months, she experienced recurrent episodes of cough, dyspnoea, lower limb pain and abdominal discomfort. Examination revealed cold extremi

OBJECTIVE: To investigate the correlation of obesity and overweight with cardiac ultrasound parameters and future cardiovascular risk among healthy populations.
METHODS: Basic clinical characteristics as well as cardiac ultrasound parameters were collected from healthy people. Firstly, all participants were divided into three groups: normal, overweight, and obese. Then the differences in cardiac ultrasound parameters between the three groups were calculated. Subsequently, those aged 35-60 years were screened to determine their cardiovascular risk according to the SCORE system. Finally, the co

While right heart catheterization (RHC) at rest is the gold standard to assess pulmonary hemodynamics in patients with chronic obstructive pulmonary disease (COPD) and pulmonary hypertension (PH), the invasive measurement of exercise hemodynamics is less well established in this group. Since exercise hemodynamics are increasingly recognized as important clinical information in patients with PH, our goal was to review the literature in this field to provide a basis for clinical use, further studies, and future recommendations. We identified 69 studies (published since 1968) reporting RHC data i

OBJECTIVE: Right ventricular (RV) dysfunction is a significant cause of morbidity and mortality after surgical correction of tetralogy of Fallot (TOF). Transatrial/transpulmonary repair avoids a ventriculotomy (in contrast to the transventricular approach) in order to preserve the structure and function of the right ventricle. We performed a pilot prospective randomized controlled trial in infants with TOF undergoing primary repair.
METHODS: A pilot prospective controlled clinical trial was conducted in infants with TOF undergoing primary repair between January 2008 and December 2009. One hun
