A variation of the extensor carpi radialis brevis muscle (ECRB) origin was discovered during a cadaveric dissection. The identified ECRB originated from the fascia/tendon of the extensor digitorum communis (EDC), without any of its origin at the usual location on the lateral epicondyle. The ECRB tendon inserted onto the third metacarpal base. A branch of the posterior interosseous nerve innervated the ERCB. This is the first report of an ECRB originating from the EDC fascia/tendon. Keratoacanthoma is a common benign squamous neoplasm that only rarely progresses to metastatic carcinoma. The cause of keratoacanthoma is unclear; however, suspected causes include ultraviolet light, genetic factors, immunosuppression, chemical carcinogens, viruses, and trauma. Keratoacanthomas have been reported only rarely to arise in sites of previous trauma. We report 2 cases of keratoacanthomas arising shortly after episodes of cutaneous trauma and provide a review of the literature of similar cases previously reported. A 24 year old combat medic was admitted to the field hospital at Tomislavgrad in Bosnia, with a suspected forearm, fracture. Radiographs did not show any bony injury. Clinical examination showed marked swelling and tenderness over the extensor compartment. Because of the difficulties in reconstructing chronic, unrecognized tendon injuries, early diagnosis and proper management are of the utmost importance. The initial examining physician is key to the treatment pathway the patient will take. A high index of suspicion for possible injuries to deep structures beneath the laceration followed by appropriate examination of each of the involved parts will usually result in an accurate diagnosis.

Two cases of a rare anomalous muscle in the forearm, palmaris profundus, are described. In one case, symptoms were caused by the median cutaneous palmar branch which crossed the tendon at a point where it could easily be traumatized by different movements of the wrist. In the other, the tendon was an incidental discovery in one wrist of a patient with a bilateral carpal tunnel syndrome caused by tenosynovitis.

Zyderm collagen implant, a solubilized, purified, bovine collagen preparation for augmentation of soft tissue, produced a foreign body granuloma in a 37-year-old woman who had no history of immunologic disorders or autoimmune disease. The patient had received a small test implant to determine if she were sensitive to Zyderm. A very slight, borderline response occurred and a second test implant was administered. This second implant resulted in a foreign body granuloma that has persisted for ten months.
