[Changes in surgical therapy of pulmonary hydatid cysts].

The lung is the second most common site of hydatid cysts after the liver. The authors analyse retrospectively the results of patients treated with pulmonary hydatid cysts in the past 18 years, considering video-thoracoscopy. Twenty eight patients were treated during this period in 31 cases. Hydatid disease affected only the lung in case of 22 patients, while in 6 cases it was present in the liver and lung simultaneously. Pulmonary hydatid disease affected one side in 24 and both sides in 4 cases. For surgical treatment pericystectomy in one, atypical segment resection in 18, anatomical segment

Fundus-first laparoscopic cholecystectomy.

BACKGROUND: Fundus-first dissection (FFD) is an established technique to deal with difficult open cholecystectomies. Although the indications for such an approach are similar for laparoscopic cholecystectomy (LC), FFD is not widely practiced because of difficulties that arise with liver retraction, the dissection of dense adhesions, or obscured cystic pedicles, often necessitating conversion to an open procedure. METHODS: The aim of this study was to evaluate the indications for FFD and the technical aspects of the procedure in cases with a difficult cystic pedicle. Prospectively collected da

Epidemiology, patient management, and survival outcomes in resected patients with non-metastatic non-small cell lung cancer: a nationwide real-world study.

INTRODUCTION: Surgery is the standard of care for eligible patients with localized or stage IIIA locally advanced non-small cell lung cancer (NSCLC) current guidelines recommend the most conservative surgeries possible. The aim of this study was to bring new real-world data on resected NSCLC epidemiology, management, and survival outcomes in patients with resected non-metastatic NSCLC. MATERIALS AND METHODS: This is a descriptive, non-interventional, national, retrospective claims study using data from the French National Hospitalization Database (PMSI) describing the management of patients w

Temperature safety profile of laparoscopic devices: Harmonic ACE (ACE), Ligasure V (LV), and plasma trisector (PT).

BACKGROUND: Reports of iatrogenic thermal injuries during laparoscopic surgery using new generation vessel-sealing devices, as well as anecdotal reports of hand burn injuries during hand-assisted surgeries, have evoked questions about the temperature safety profile and the cooling properties of these instruments. METHODS: This study involved video recording of temperatures generated by different instruments (Harmonic ACE [ACE], Ligasure V [LV], and plasma trisector [PT]) applied according the manufacturers' pre-set settings (ACE setting 3; LV 3 bars, and the PT TR2 50W). The video camera used

[Laparoscopic training in urology. An essential principle of laparoscopic interventions in the retroperitoneum].

The main problem with clinical introduction of laparoscopic techniques in urology is that despite experience with endoscopy no significant endoscopic training is given. Laparoscopic nephrectomy, in particular, is a rather complicated procedure and needs an adequate training concept: The beginner should have the possibility of practising in vitro in a closed "pelvi-trainer" to learn video-optical coordination and orientation; then it is necessary to get used to handling the instruments and practise the different steps of the procedure (i.e. clipping of vessels, sacking of the kidney) either in

Body weight-associated procedural bottlenecks in robot-assisted surgery for congenital biliary dilatation: a step-specific operative time analysis.

Adults and heavier patients undergoing robot-assisted surgery for congenital biliary dilatation may require longer operative times. This study used step-specific video analysis to identify where additional time accumulated and which components remained relatively stable across patient size. Fifty-five patients (32 children and 23 adults) were retrospectively analyzed. Operative videos were divided into seven predefined steps and unclassified time. Step-specific durations were compared between groups. Restricted cubic spline and segmented regression analyses explored relationships between body

Item-Level Scoring Profiles and Technical Domains in the Colorectal Endoscopic Surgical Skill Qualification Examination.

BACKGROUND: Video-based assessment is increasingly used to evaluate operative performance, but pass-fail outcomes alone provide limited information on the technical components associated with the decision. This study aimed to clarify item-level scoring patterns and latent technical domains underlying pass-fail decisions in the colorectal Endoscopic Surgical Skill Qualification examination. METHODS: This secondary item-level analysis used the colorectal ESSQS examination dataset of the Japan Society for Endoscopic Surgery from 2020 to 2024. The final application-level pass-fail decision was th

Eye-hand coordination during robotic prostate cancer surgery - how surgical vision reflects on instrument kinematics and vice versa.

During robot-assisted radical prostatectomy (RARP), vision drives the (tele)surgical actions. As such, endoscope movements can be considered a performance-defining parameter during robotic surgery. While instrument kinematics have been the subject of research, it remains unclear how steps in the surgical procedure are reflected in endoscope kinematics, and vice versa. Using the AP4000 mechanical instrument movement recorder, we digitized the x-, y-, and z-trajectories of the endoscope and the 3 robotic arms holding the dissecting instruments and synchronized these data with the endoscopic vide

Primary pulmonary myxoid sarcoma with detected EWSR1 gene translocation: a case report.

BACKGROUND: Primary pulmonary myxoid sarcoma (PPMS) is an exceedingly rare low-grade malignant sarcoma characterized by the presence of EWSR1 rearrangement and EWSR1::CREB1 fusion, which serve as critical diagnostic markers. Due to its nonspecific symptoms, PPMS is often detected incidentally, complicating its differentiation from other pulmonary neoplasms. CASE DESCRIPTION: We report a case of a 52-year-old female patient who was admitted in May 2024 after a nodule was incidentally found in the right middle lobe during a routine health examination. Imaging studies revealed a solid nodule wit

Predicting the need for drainage in pleural infection: A comparison of clinical guidelines.

BACKGROUND: Pleural infection requires early identification of individuals who are unlikely to respond to antibiotic therapy and may require drainage. Although several algorithms-Light's classification, the American College of Chest Physicians (ACCP) consensus statement, and the British Thoracic Society (BTS) 2003 and 2023 guidelines-are available, their predictive performance remains uncertain. METHODS: This retrospective diagnostic accuracy study analyzed individuals with pleural infection treated at two municipal hospitals between 2017 and 2023. Missing data were handled using multiple imp

Management of a Pulmonary Sequestration With a Large Aberrant Artery Using a Hybrid Approach: Thoracic Endovascular Aortic Repair Followed by Video-Assisted Thoracoscopic Lobectomy.

Pulmonary sequestration is a rare congenital malformation of the lower respiratory tract, characterized by nonfunctioning lung tissue without tracheobronchial communication and receiving its blood supply from the systemic circulation. It can present as either intralobar or extralobar sequestration, with intralobar being more common. Surgical resection remains the treatment of choice, although preoperative strategies such as endovascular occlusion can reduce the risk of intraoperative complications. We report the case of a 52-year-old woman with a history of recurrent pulmonary infections who w
