Modification of Plusoptix referral criteria to enhance sensitivity and specificity during pediatric vision screening.

PURPOSE: To determine the impact of using several different proposed sets of referral criteria on the specificity and sensitivity of the plu … oposed by the Vision Screening Committee of the American Association of Pediatric Ophthalmology and Strabismus (AAPOS). Modifications of these criteria also were evaluated. The screening results obtained by the plusoptiX S08 were compared with the results from the gold standard pediatric ophthalmologic examination, and the respective sensitivities and specif …

A rare chromosomal disorder - isochromosome 18p syndrome.

BACKGROUND: Tetrasomy 18p is a very rare chromosomal disorder and is the result of a spontaneous mutation early in embryonic development in most of the cases. This condition is characterised by the presence of a supernumerary 18p isochromosome (i(18p)) in all or some cells of the affected individual. It has a prevalence of 1/180000 live births and affects both genders equally. MATERIALS AND METHODS: In this paper we report a de novo tetrasomy 18p in a 3 months old female dysmorphic child. The clinical features were distinctive with a particular facies, strabismus, microcephaly, growth delay, …

Monosomy 21q22.11-q22.13 presenting as a Fanconi anemia phenotype.

We report on a 5-year-old Caucasian female with multiple anomalies whose deletion, 46,XX,del(21)(q22.11q22.13), was determined by a 105K oligonucleotide-based microarray. This case is a unique deletion that mimicked Fanconi anemia (combination of thrombocytopenia, thumb anomalies, congenital heart defects, borderline small head circumference, strabismus, hydronephrosis, and significant developmental delay) but testing for Fanconi anemia was negative, as was testing for a wide array of genetic/metabolic conditions. Microarray testing done at 5 months failed to demonstrate the interstitial delet…

Screening for refractive errors in children: the plusoptiX S08 and the Retinomax K-plus2 performed by a lay screener compared to cycloplegic retinoscopy.

PURPOSE: To evaluate the performance of the autorefractor Retinomax K-plus2 and the photoscreener plusoptiX S08 in measuring refractive errors by comparing them with cycloplegic retinoscopy (CR) and to assess limitations associated with their use. METHODS: Cross-sectional study to compare data from CR, performed by an orthoptist, to data from Retinomax K-plus2 and plusoptiX S08 performed by a lay screener. Sensitivity and specificity for the detection of significant refractive errors were determined according to American Academy of Pediatric Ophthalmology and Strabismus criteria. RESULTS: Tw…

Clinical findings and surgical results of Duane retraction syndrome.

PURPOSE: To evaluate clinical properties and surgical outcomes of Duan retraction syndrome (DRS). METHODS: Twenty-one patients with DRS were evaluated. Surgery was indicated for correction of strabismus, abnormal head position, upshoot-downshoot, and globe retraction. Six patients had undergone surgery. In 5 cases, the lateral rectus muscle was recessed from 4.0 to 7.0 mm combined with "Y-splitting" according to deviation in primary position and degree of globe retraction. In case of significantly positive forced duction test on the medial rectus muscle intraoperatively, simultaneous recessio…

Tetrasomy 18p: report of the molecular and clinical findings of 43 individuals.

Thus far, the phenotype of tetrasomy 18p has been primarily delineated by published case series and reports. Findings reported in more than 25% of these cases include neonatal feeding problems, growth retardation, microcephaly, strabismus, muscle tone abnormalities, scoliosis/kyphosis, and variants on brain MRI. Developmental delays and cognitive impairment are universally present. The purpose of this study was to more fully describe tetrasomy 18p at both the genotypic and the phenotypic levels. Array CGH was performed on 43 samples from individuals with tetrasomy 18p diagnosed via routine kar…

PURPOSE: To evaluate the short-term dose effect of botulinum toxin in manifest and intermittent horizontal strabismus types. METHODS: Dysport of 2 international units was injected into a single muscle in cases of constant esotropia and exotropia plus decompensating esophoria and exophoria. Electromyography (EMG) signal was recorded during each procedure. Angle of deviation was measured in each case pretreatment and posttreatment at 2 weeks by prism cover test for near and distance fixation. Change in deviation was evaluated by nonparametric statistical evaluation. RESULTS: Sixty-six patients…

AIMS: To document the prevalence of ocular genetic disorders in a general paediatric ophthalmology clinic in a deprived, ethnically diverse inner city area of London, and to assess the consanguineous relationships and ethnicity among families with these conditions. METHODS: A prospective audit documented all ocular genetic conditions (excluding strabismus and amblyopia) presenting for 16 consecutive weeks to the paediatric ophthalmology clinic. Information regarding ethnicity and consanguinity were sought. The disorders were divided according to the mode of inheritance if known. RESULTS: Thi…

[Retinopathy of prematurity--functional and structural outcome in children treated with diode laser photocoagulation].

AIM: To present the long-term structural and functional outcome of children treated with diode laser for retinopathy of prematurity. PATIENTS AND METHODS: A total of six premature babies (12 eyes) in whom diode laser treatment was performed for acute retinopathy were followed up. The mean birth weight was 1185.0 +/- 309.6 g and mean gestational age was 28.5 +/- 1.87 weeks. Long term follow-up (18 to 41 months) included retinal examination, refractive status of the eye, and orthoptic evaluation: strabismus, amblyopia and nystagmus assessment. RESULTS: Unfavorable structural outcome was detect…

"Inverted Brown pattern": a tight inferior oblique muscle masquerading as a superior oblique muscle underaction--clinical characteristics and surgical management.

PURPOSE: To characterize, and evaluate the surgical management of, patients with unilateral deficiency of depression in adduction, suggesting superior oblique muscle underaction, without significant ipsilateral inferior oblique muscle overaction. METHODS: Such patients were identified who also had received either ipsilateral inferior oblique (IO) muscle weakening or contralateral inferior rectus muscle recession. Their histories, motility patterns, intraoperative findings, types of strabismus surgery, and postoperative results were analyzed. RESULTS: Twelve patients were identified with unil…

Anterior and nasal transposition of the inferior oblique muscles in patients with missing superior oblique tendons.

INTRODUCTION: Patients with missing superior oblique (SO) tendons present with overelevation/underdepression in adduction. Unilateral cases often exhibit abnormal head postures, whereas in bilateral cases, there may be a marked V-pattern with upgaze exotropia. These patients may have craniosynostosis. METHODS: Nine children with unilateral (n = 2) or bilateral (n = 7) absent SO tendons underwent anterior and nasal transposition of the inferior oblique (IO) muscles, some in combination with horizontal rectus recession for horizontal strabismus.
