Complete bilateral spontaneous pneumothorax at moderate altitude revealing Marfan syndrome: a case report.

BACKGROUND: Primary spontaneous pneumothorax (PSP) commonly occurs in young adults. However, simultaneous complete bilateral pneumothorax is rare and potentially fatal. Changes in barometric pressure associated with altitude exposure may precipitate pneumothorax in predisposed individuals, particularly those with unrecognized connective tissue disorders. CASE DESCRIPTION: We report a case of a 17-year-old previously healthy male who developed sudden severe dyspnea while recreational skiing at a moderate altitude of 2,200 meters. Emergency on-site management required immediate bilateral needle

Porous Diaphragm Syndrome: A Literature Review.

Porous diaphragm syndrome (PDS) is a rare and underrecognized condition characterized by the transdiaphragmatic migration of peritoneal contents into the pleural cavity through defects in the diaphragm, most often affecting the right hemidiaphragm. It is commonly associated with hepatic hydrothorax, peritoneal dialysis-related hydrothorax, and peritoneal carcinomatosis. Despite its anatomical simplicity and the availability of effective surgical treatment, PDS is frequently misdiagnosed, leading to repeated pleural drainage, inappropriate therapies, and treatment delays. This review synthesize

Spontaneous Hemothorax in Neurofibromatosis Type 1: A Surgical Case Report.

Neurofibromatosis type 1 (NF-1) is an autosomal dominant disorder associated with vascular abnormalities, including spontaneous hemothorax and arterial aneurysms. We present a rare case of spontaneous hemothorax in which an apparently hemostatic sub-pleural hematoma began to bleed again after the patient was repositioned. A 47-year-old man with NF-1 presented with the sudden onset of left-sided chest pain. Contrast-enhanced chest computed tomography revealed a massive left-sided hemothorax with a tortuous intercostal artery and an adjacent hematoma. The patient underwent emergency video-assist

A Rare Case of Pulmonary Alveolar Proteinosis.

Pulmonary alveolar proteinosis (PAP) is a rare surfactant‑clearance disorder most often driven by neutralizing autoantibodies against granulocyte-macrophage colony-stimulating factor (GM-CSF). This leads to accumulation of surfactant in the alveolar spaces, with a symptom spectrum ranging from mild, indolent symptoms to life-threatening respiratory failure. A 50‑year‑old female marathon runner with no smoking history presented with a two-month history of progressively worsening exertional dyspnea and cough. High‑resolution computed tomography (HRCT) showed bilateral peripheral and basal ground

Video-assisted thoracoscopic-guided reduction and external stabilisation of traumatic rib fractures in a cat.

CASE SUMMARY: An 11-year-old male neutered domestic longhair cat was presented to an emergency referral hospital after sustaining severe injuries isolated to the thorax after a dog attack. Initial stabilisation included oxygen supplementation, intravenous fluid therapy, point-of-care ultrasound (POCUS), serial blood gas analyses, thoracic and abdominal radiographs, opioid analgesics and broad-spectrum antibiotics. CT revealed multiple internally displaced rib fractures that punctured through the mediastinum and were in intimate proximity to the cranial vena cava and proximal aortic arch. Video

Giant mediastinal teratoma mimicking pericardial cyst: A case report with brief review of the literature.

Giant teratomas with bilateral mediastinal extension are rare. The present case report highlights the challenging resection of a giant anterior mediastinal cystic teratoma extending bilaterally using video-assisted thoracoscopic surgery (VATS), rendering it one of the largest mediastinal teratomas managed with VATS in the current literature. A 47-year-old woman presented with a 1-year history of central chest pain. The physical examination did not reveal any notable findings. A computed tomography coronary angiogram revealed a thin-walled cyst in the anterior mediastinum, measuring ~12x10x9 cm

Diagnostic Challenge of Tuberculous Pleural Effusion: Elevated Adenosine Deaminase (ADA) as the Key Indicator in the Absence of Microbiological Confirmation.

Tuberculous pleural effusion (TPE) is frequently paucibacillary. Consequently, acid-fast bacilli (AFB) smears, polymerase chain reaction (PCR), and mycobacterial cultures from pleural fluid are often negative. Pleural adenosine deaminase (ADA) is an important adjunct, but interpretation requires an appropriate clinical context. A previously healthy 20-year-old man presented with pleuritic chest pain, dyspnea, fever, and bilateral pleural effusions. Despite repeated negative microbiology for Mycobacterium tuberculosis (AFB smear, PCR, and cultures from sputum, bronchoalveolar lavage, pleural fl

Localized nodular pulmonary amyloidosis mimicking primary lung cancer associated with cystic airspaces: A case report.

Localized nodular pulmonary amyloidosis can form pulmonary nodules associated with cystic air spaces, but due to its rarity, it cannot be included in the differential diagnosis without appropriate knowledge. Among the differential diagnoses of nodules with cysts in the lungs is primary lung cancer, however, diagnosis based solely on imaging findings is challenging. A 59-year-old Japanese female was referred to our hospital for an abnormality noted on the chest radiograph of an annual health check. She had no history of smoking or medical issues. Chest computed tomography revealed a 1.2 cm pulm

A Case of Lemierre-Like Syndrome in a Pediatric Patient.

Lemierre-like syndrome is a rare, systemic sequelae following a persistent oropharyngeal infection, leading to septic thrombophlebitis of the internal jugular vein (IJV). Lemierre syndrome is caused by the obligate anaerobic organism Fusobacterium necrophorum, innate to the oropharyngeal tract. Lemierre-like syndrome is due to infections caused by other organisms, including methicillin-resistant Staphylococcus aureus (MRSA). We are reporting a case of a five-month-old male who presented with one week of fever that was not alleviated by acetaminophen, bilateral otitis media, and left-sided cerv

Adding Dexmedetomidine to Methylene Blue in Thoracic Paravertebral Block for Video-Assisted Lobectomy: A Case Series Study.

PURPOSE: Thoracic surgery often results in severe chronic postoperative pain. Current evidence favors two locoregional techniques. Thoracic Epidural Anesthesia (TEA), the gold standard, and Thoracic Paravertebral Block (TPVB), which is associated with fewer side effects but is limited by short duration of action of local anesthetics (LA) and potential failure due to improper drug distribution. This study investigates the use of dexmedetomidine (DEX) as adjuvant to prolong the effects of LA in TPVB, with methylene blue used for visual confirmation of accurate injectate spread. Metaplastic thymoma in the middle mediastinum: a rare case report and surgical treatment analysis of a 32-year-old female patient.

BACKGROUND: Metaplastic thymoma (MT), an exceedingly rare variant of primary thymic epithelial neoplasms, is distinguished by its indolent progression and unique histopathological profile. It presents a biphasic pattern characterized by solid epithelial and spindle cell components, potentially leading to diagnostic confusion with type A thymomas or the type A component of type AB thymomas. Accurate diagnosis is pivotal for optimal therapeutic strategies and prognostication. CASE DESCRIPTION: We document an exceptional case of a 32-year-old woman, incidentally discovered to have a mediastinal

A Critical Case of Streptococcal Toxic Shock Syndrome: A Case Report.

A 41-year-old woman with a history of asthma presented to the emergency department with complaints of progressive malaise, dyspnea, vomiting, and diarrhea for a week. Upon presentation, the patient was hemodynamically unstable and exhibited severe respiratory distress. A chest computed tomography revealed consolidation of the left upper lobe with confluence in the left perihilar region and a left pleural effusion. The patient was admitted to the intensive care unit for further management of respiratory failure, and a chest tube was placed on the left side. Despite the absence of bacteremia, th

Displaced bronchus and anomalous pulmonary vein passing dorsal to the pulmonary artery in a patient who underwent right upper lobectomy for lung cancer with lymph node metastases: a case report.

BACKGROUND: Tracheobronchial anomalies are extremely rare and are often associated with pulmonary arteriovenous anatomical anomalies. An anomalous right upper vein segment that passes between the pulmonary artery (PA) and bronchus is a rare vascular abnormality. We report a case of a displaced superior posterior branch (B2) that was independent of the superior apical/anterior branch (B1 + 3) accompanied by anomalous right superior pulmonary vein (SPV) anatomy in a patient who underwent right upper lobectomy for lung cancer with lymph node metastases. CASE PRESENTATION: A 73-year-old asymptoma

Posterior mediastinal extralobar pulmonary sequestration misdiagnosed as a neurogenic tumor: A case report.

BACKGROUND: Pulmonary sequestration-both intralobar and extralobar-is a rare congenital developmental malformation. Extralobar pulmonary sequestrations (EPS) have their own pleura but are separated from the bronchus and usually occur in the left lung. They are mainly found mainly between the lower lobe and the mediastinum. EPS is rarely found within the mediastinum itself, even rarer so in the posterior mediastinum. CASE SUMMARY: We report the case of a 27-year-old man who was misdiagnosed with a neurogenic tumor based on preoperative contrast-enhanced computed tomography (CT) and magnetic re

Acupuncture against chronic postsurgical pain in non-small cell lung cancer patients: A protocol of randomized controlled trial.

INTRODUCTION: Video-assisted thoracoscopic lobectomy is the prior recommended treatment for non-small cell lung cancer (NSCLC), with the advantages of small trauma, less postoperative pain, and quick recovery. However, a large number of patients may suffer chronic postsurgical pain (CPSP), which makes the patients unwilling to practice pulmonary exercises, and it would directly affect patient's cough, sputum expectoration, and mobility. Opioids could greatly improve the quality of postoperative analgesia and the quality of life after surgery, but it is accompanied with obvious side effects. A

Comparison of direct laryngoscopy and video-assisted laryngoscopy in pediatric intensive care unit.

Our objective was to compare video-assisted laryngoscopy (VAL) with direct laryngoscopy (DL) for glottic visualization in a pediatric intensive care unit in terms of the success rate in first attempts. Our study included patients aged from 1 month to 18 years who were admitted to the pediatric intensive care unit. We excluded patients with limited neck extension (C-spine immobilization, congenital abnormality), congenital anomalies (e.g., Pierre Robin syndrome, micrognathia, macroglossia), and recent airway surgery. Patients were premedicated before intubation. The time to intubation was defin

Penetrating pulmonary injury due to a thrown rusty nail while using a lawn mower: a case report.

Accidents while using lawn mowers are often reported, but chest injuries are rare. We report a case of a penetrating pulmonary injury due to a rusty nail thrown by a lawn mower. A 63-year-old man injured his left precordium by a thrown object while using a lawn mower; he thereafter visited a nearby hospital. A computed tomography image showed a left-sided pneumothorax and a penetrating pulmonary injury of the left upper lobe due to a foreign body. Pneumothorax was treated with thoracic drainage and resolved. He was transferred to our hospital on the second day after the injury, and video-assis

Large Spontaneous Right Catamenial Pneumothorax with Diaphragmatic Defect and Liver Herniation.

Catamenial pneumothorax is a spontaneous pneumothorax that occurs predominantly women of child bearing age. We describe a case of a 40-year-old nulliparous woman with medical history significant for endometriosis who presented with severe chest tightness of one-day duration. Chest radiography (CXR) showed a large right spontaneous pneumothorax, what was thought to be a 5.6 cm pleural mass at the right lung base. Following pneumothorax diagnosis, the patient underwent emergent right thoracostomy with pigtail catheter placement. A repeat CXR revealed marked re-expansion of the lung but persisten
