Otolaryngology-specific emergency room as a model for resident training.

OBJECTIVES/HYPOTHESIS: There is a paucity of data on junior resident training in common otolaryngology procedures such as ear debridement, nasal and laryngeal endoscopy, epistaxis management, and peritonsillar abscess drainage. These common procedures represent a critical aspect of training and are necessary skills in general otolaryngology practice. We sought to determine how a dedicated otolaryngology emergency room (ER) staffed by junior residents and a supervising attending provides exposure to common otolaryngologic procedures. STUDY DESIGN: Retrospective review. METHODS: Diagnostic and

Deliberation at the hub of medical education: beyond virtue ethics and codes of practice.

Although both codes of practice and virtue ethics are integral to the ethos and history of "medical professionalism", the two trends appear mutually incompatible. Hence, in the first part of the paper we explore and explicate this apparent conflict and seek a direction for medical education. The theoretical and empirical literature indicates that moral deliberation may transcend the incompatibilities between the formal and the virtuous, may enhance moral and other aspects of personal sensitivity, may help design and improve other parts of the curricula, and may foster self-awareness and clarif

Arthroscopy or ultrasound in undergraduate anatomy education: a randomized cross-over controlled trial.

BACKGROUND: The exponential growth of image-based diagnostic and minimally invasive interventions requires a detailed three-dimensional anatomical knowledge and increases the demand towards the undergraduate anatomical curriculum. This randomized controlled trial investigates whether musculoskeletal ultrasound (MSUS) or arthroscopic methods can increase the anatomical knowledge uptake. METHODS: Second-year medical students were randomly allocated to three groups. In addition to the compulsory dissection course, the ultrasound group (MSUS) was taught by eight, didactically and professionally t…

Voice training in teacher education: the effect of adding an individualized microteaching session of 30 minutes to the regular 6-hour voice training program.

In a previous study, we investigated the effect of a twofold voice-training module in student teachers. In the present study, the original training module of 3 hours of indirect and 3 hours of direct group training was expanded with a 30-minute individual counseling session for each participant. The main focus was on the effects of this threefold training paradigm on the voice of the participants. The subjects were 81 students at the academic teaching program at the Vrije Universiteit Brussel. The trained group (n=51) received the entire voice-training program, whereas the control group (n=30)

An interprofessional service-learning course: uniting students across educational levels and promoting patient-centered care.

Recognizing the importance of interprofessional education, we developed a pilot interprofessional education course at our institution that included a total of 10 nursing, BS health psychology, premedical, and pharmacy students. Course goals were for students to: 1) learn about, practice, and enhance their skills as members of an interprofessional team, and 2) create and deliver a community-based service-learning program to help prevent or slow the progression of cardiovascular disease in older adults. Teaching methods included lecture, role-play, case studies, peer editing, oral and poster pre

Dual diagnosis--policy and practice in Italy.

In Italy, dual diagnosis issues are affected by distinctive historical pathways. Since the end of the 1970s, the addiction field has been starved of psychiatric contribution, with the emphasis being on a pedagogical model of substance-related disorders and a bio-psycho-social model for mental health. As such, dual diagnosis treatment, service models, and research have only been developed in the last fifteen years. Substantial training needs with regard to dual diagnosis are identified in addiction and mental health professionals, and new graduate and undergraduate programs are required. Furthe

Pulse wave velocity and short-term outcome in patients requiring intravascular volume expansion: a pilot study.

BACKGROUND: Fluid therapy has a pivotal role in the management of acutely ill patients. However, whether or not a patient can tolerate additional intravascular volume is controversial and optimal strategy is unknown. Carotid femoral pulse wave velocity (cfPWV) evaluates arterial stiffness. OBJECTIVE: To determine whether cfPWV can predict the ability of patients to tolerate clinically indicated acute fluid expansion. METHODS: 50 consecutive patients requiring intravascular volume expansion were prospectively recruited in intensive care units. All subjects underwent transthoracic echocardiogr

Language and hope in schizophrenia-spectrum disorders.

Hope is integral to recovery for those with schizophrenia. Considering recent advancements in the examination of clients' lexical qualities, we were interested in how clients' words reflect hope. Using computerized lexical analysis, we examined social, emotion, and future words' relations to hope and its pathways and agency components. Forty-five clients provided detailed narratives about their life and mental illness. Transcripts were analyzed using the Linguistic Inquiry and Word Count program (LIWC), which assigns words to categories (e.g., "anxiety") based on a pre-existing dictionary. Cor

[Patients with Acute, Non-Traumatic Abdominal Pain in German Emergency Departments: A Prospective Monocentric Observation Study].

Background: Up to 11 % of patients in an Emergency Department (ED) present with non-traumatic acute abdominal pain. Based on this presenting symptom, this study aimed to analyse how residents (surgery, internal medicine, anaesthesiology and other fields) working in an ED during their second and third year of education treat these patients. Material and Methods: We performed a prospective, monocentric observation study in an ED in accordance with the STROBE recommendations, following the recommendations from the Ethics Committee of the University of Ulm (application no. 335/12) and the Declarat

Procedural specificity in laparoscopic simulator training: protocol for a randomised educational superiority trial.

BACKGROUND: The use of structured curricula for minimally invasive surgery training is becoming increasingly popular. However, many laparoscopic training programs still use basic skills and isolated task training, despite increasing evidence to support the use of training models with higher functional resemblance, such as whole procedural modules. In contrast to basic skills training, procedural training involves several cognitive skills such as elements of planning, movement integration, and how to avoid adverse events. The objective of this trial is to investigate the specificity of procedur

Newborn mortality and fresh stillbirth rates in Tanzania after helping babies breathe training.

BACKGROUND: Early neonatal mortality has remained high and unchanged for many years in Tanzania, a resource-limited country. Helping Babies Breathe (HBB), a novel educational program using basic interventions to enhance delivery room stabilization/resuscitation, has been developed to reduce the number of these deaths. METHODS: Master trainers from the 3 major referral hospitals, 4 associated regional hospitals, and 1 district hospital were trained in the HBB program to serve as trainers for national dissemination. A before (n = 8124) and after (n = 78 500) design was used for implementation.

[Current developments and perspectives on the diagnosis and treatment of urinary incontinence and genital prolapse in women].

As a key area of gynaecology, urogynaecology has undergone impressive changes in the past few years. Together with the high prevalence of functional pelvic floor disorders, modern anaesthesia procedures and the introduction of new, innovative minimally invasive operation techniques have led to a dramatic increase in the number of operations for incontinence and prolapses. The increasingly subtle diagnostic options, such as, e. g., 2D and 3D sonography of the pelvic floor provide unambiguous findings and facilitate decision making. Community partners as co-teachers in resident continuity clinics.

Standard approaches to teaching the management of psychosocial issues in pediatrics--visits to community-based organizations and stand-alone block rotations in developmental-behavioral pediatrics and community pediatrics--neither expose residents to models of interdisciplinary collaboration between faculty preceptors and community providers nor take advantage of the efficacy of learning in continuity clinics. The authors describe their project, developed from an existing Community Pediatrics Training Initiative with long-standing relationships with a domestic violence shelter, a community cent

Dermatology - a surgical discipline - as exemplified by surgical procedures at the Hospital of Munich Thalkirchner Strasse.

BACKGROUND: Despite the enormous progress in dermatosurgery during the last 35 years, this achievement is not always appropriately acknowledged by other surgical specialties. We outline the significance of dermatosurgery by looking at the development of dermatological operations at the Department of Dermatology,University of Munich/Hospital of Munich Thalkirchner Strasse. PATIENTS AND METHODS: Data were collected and analyzed from surgery records, tables and from the SAP ISH-Med.The survey covered the time frame from 1971 to 2006. RESULTS: There were 101,103 inpatient operations. The number o

The development of psychiatric medicine at Istanbul Faculty of Medicine and evaluation of 889 psychiatric referrals.

A clinical analysis of the psychiatric referrals (N = 889) during 1989-1991 is presented. The most prevalent psychiatric syndromes seen in both inpatients and outpatients are compared with reference to those areas of psychopathology that are less frequently identified by nonpsychiatrist physicians. The distribution of psychiatric referrals according to the various clinics in order of frequency was as follows: internal medicine, surgery, dermatology, and neurology. The most frequent reasons for consultation requests were (in order of frequency) the referring physician's difficulty in making a d

Holding and Mobility of Pediatric Patients With Transthoracic Intracardiac Catheters.

BACKGROUND: Nursing care of pediatric patients after cardiac surgery consists of close hemodynamic monitoring, often through transthoracic intracardiac catheters, requiring patients to remain on bed rest and limiting holding and mobility. OBJECTIVES: The primary aim of this quality improvement project was to determine the feasibility of safely mobilizing pediatric patients with transthoracic intracardiac catheters out of bed. Once feasibility was established, the secondary aim was to increase the number of days such patients were out of bed. METHODS AND INTERVENTIONS: New standards and proce

Method for the evidence-based reviews on occupational therapy and stroke.

Evidence-based reviews of the literature relevant to adults with stroke are important to the practice of occupational therapy. We describe the four questions that served as the focus for the evidence-based reviews of the effectiveness of occupational therapy interventions for adults with stroke. The questions include occupation- and activity-based interventions to improve occupational performance and social participation after stroke, as well as interventions for motor, cognitive, and psychological and emotional impairments after stroke. We include the background for the reviews; the process f

Germ simulation: a novel approach for raising medical students awareness toward asepsis.

INTRODUCTION: Medical student procedural training has become increasingly limited amid concerns for patient safety. Because simulation education has been shown to be effective for early skills training, we used germ simulation for teaching hand hygiene principles and aseptic technique in urinary catheterization (UC). We hypothesized that posttraining, students would demonstrate proficiency comparable to surgery residents. METHODS: Seventy-five clinical clerks, second-year medical students, underwent the simulation training. Sixty-four volunteered to have their asepsis scores compared with 21
