DE QUERVAIN'S DISEASE: STENOSING TENOVAGINITIS AT THE RADIAL STYLOID PROCESS.

Twelve cases of stenosing tenovaginitis at the radial styloid process (de Quervain's disease), including two bilateral cases, are reviewed. These patients presented the typical symptoms of pain just proximal to the radial styloid process accompanied by limitation of abduction of the thumb. Many previous reports have emphasized the frequency of aberrant tendons in this region and have implied that such anomalies may play a part in the etiology of this disorder. In our cases aberrant tendons were not found. Pronounced thickening of the tendon sheaths was present at operation and accumulations of

Bony adaptation signs are predictive of anterior head-neck offset remodeling after internal fixation for slipped capital femoral epiphysis: a multicenter study on 217 patients (228 hips) with follow-up until end of growth.

BACKGROUND AND PURPOSE: We aimed to evaluate the prognostic value of radiographic factors in predicting femoral head-neck remodeling, cam deformity, and local growth disturbances in slipped capital femoral epiphysis (SCFE) treated with internal fixation (IF) with or without simultaneous closed reduction. METHODS: A retrospective multicenter study on 217 SCFE patients (228 hips) treated with IF in 10 institutions was performed. Hip morphology was assessed using preoperative, postoperative, and end of growth (EG) radiographs and radial MRI scans. Evaluated parameters included: epi-metaphyseal d

Is Subcapital Realignment for Slipped Capital Femoral Epiphysis a Reproducible Technique? Midterm Results.

BACKGROUND: The modified Dunn procedure, which is based on the development of an extended retinacular flap containing the blood supply for the femoral head, allows anatomic reestablishment in patients with moderate to severe slipped capital femoral epiphysis (SCFE). Some controversy exists regarding the short-term to midterm risk of avascular necrosis (AVN) and other complications resulting from the surgical technique. QUESTIONS/PURPOSES: (1) What percentage of patients treated with an extended retinacular flap during the modified Dunn procedure for SCFE with a moderate (slip angle from 30° t

A retrospective review of revision proximal humeral allograft-prosthetic composite procedures: an analysis of proximal humeral bone stock restoration.

BACKGROUND: Allograft-prosthetic composite (APC) reconstruction of the proximal humerus is a technique for reconstruction of large bone deficits, provides improvement in pain and function, and is thought to restore bone if revision surgery is needed. The purpose of this study was to evaluate the ability of proximal humeral APCs to restore usable bone at the time of revision surgery. METHODS: Two institutional electronic medical records were reviewed to evaluate proximal humeral APC procedures performed between 1970 and 2018. We identified 115 cases, of which 14 underwent revision for aseptic

Slipped Capital Femoral Epiphysis in Children without Obesity.

OBJECTIVE: To evaluate rates and characteristics of slipped capital femoral epiphysis (SCFE) in children who are not obese to prevent missed diagnoses and subsequent complications. STUDY DESIGN: A multicenter, retrospective review identified all patients with SCFE from January 1, 2003 to December 31, 2012. Patients were excluded if they received previous surgery at an outside institution, had no recorded height and weight, or had medical co-morbidity associated with increased risk of SCFE. Body mass index (BMI) percentile for age was calculated and categorized for each patient (patients witho

Relevance of MRI for management of non-displaced lateral humeral condyle fractures in children.

INTRODUCTION: The treatment for non-displaced (<2 mm displacement) fractures of the lateral humeral condyle in children is controversial. Most studies recommend non-surgical treatment. However, plain radiographs are not sufficient to evaluate extension of the fracture line through the articular cartilage. This explains the high frequency of secondary displacements and non-unions, despite well-conducted conservative treatment. We hypothesized that MRI could be used to analyse whether the fracture is complete or incomplete. This could help to determine whether surgical or conservative treatment

The utility of the radiographic condylar cut-off sign in children and adolescents with complete discoid lateral meniscus.

PURPOSE: The purpose of this study was to determine the utility of the radiographic 'condylar cut-off sign' to detect the presence of a complete discoid lateral meniscus in children and adolescents. METHODS: The radiographs of 166 subjects younger than 17 with complete discoid lateral meniscus as well as 153 age- and sex-matched subjects with normal meniscus were reviewed. The condylar cut-off sign, judged by the prominence ratio of the medial and lateral femoral condyles in tunnel view radiographs, was assessed. The prominence ratio was compared between the complete discoid and the normal gr

Correction of lower extremity angular deformities in skeletal dysplasia with hemiepiphysiodesis: a preliminary report.

BACKGROUND: Lower extremity angular deformities are common in children with skeletal dysplasia and can be treated with various surgical options. Both acute correction by osteotomy with internal fixation and gradual correction by external fixation have been used with acceptable results. Recently, the Guided Growth concept using temporary hemiepiphysiodesis for correction of angular deformities in the growing child has been proposed. This study presents the results of temporary hemiepiphysiodesis using eight-Plates and medial malleolus transphyseal screws in children with skeletal dysplasia with

Applications of musculoskeletal ultrasonography in pediatric patients.

Ultrasonography is an excellent adjunct to other musculoskeletal imaging tools utilized in the pediatric population and in some instances offers advantages over CT and MRI. It permits dynamic examination of anatomic structures and assists in guiding minimally invasive procedures. In the lower extremity, ultrasonography assists in screening for such disorders as developmental dysplasia of the hip and in detecting slipped capital femoral epiphysis and femoral acetabular impingement. In the neonatal spine, ultrasonography can identify unossified vertebral arches. Among other applications in the u

Physeal fractures of the distal tibia and fibula (Salter-Harris Type I, II, III, and IV fractures).

Physeal fractures of the distal tibia and fibula are common and can be seen at any age, although most are seen in the adolescent. An understanding of the unique anatomy of the skeletally immature ankle in relation to the mechanism of injury will help one understand the injury patterns seen in this population. A thorough clinical exam is critical to the diagnosis and treatment of these injuries and the avoidance of potentially catastrophic complications. Nondisplaced physeal fractures of the distal tibia and fibula can be safely treated nonoperatively. Displaced fractures should undergo a gentl

Slipped capital femoral epiphysis following fracture of the femoral neck: a case report.

Reports on cases of femoral neck fracture complicated by a slipped capital femoral epiphysis associated with avascular necrosis and coxa vara deformity in children are extremely rare. In this case report, we describe a patient who had complications of a slipped capital femoral epiphysis with avascular necrosis and coxa vara deformity after a Delbet type III left femoral neck fracture. We also describe the surgical treatment to overcome these complications.

Total hip arthroplasty in young adults, with focus on Perthes' disease and slipped capital femoral epiphysis: follow-up of 540 subjects reported to the Norwegian Arthroplasty Register during 1987-2007.

BACKGROUND AND PURPOSE: Pediatric hip diseases account for 9% of all primary hip arthroplasties in the Norwegian Arthroplasty Register. We wanted to validate the diagnosis as reported to the register and to assess the quality of life of these patients after hip replacement. PATIENTS AND METHODS: 540 patients accepted to participate in this follow-up study (634 hips). All were less than 40 years of age and had been reported to the Norwegian Arthroplasty Register as having undergone a primary total hip arthroplasty (THA) between 1987 and 2007. The underlying diagnosis, age at diagnosis, and typ

[Surgical treatment of patients with fractures of distal metaepiphysis of the radial bone].

Traumatologic emergency to patients with primary unstable, comminuted, intraarticular fractures of distal metaepiphysis of the radial bone (DMRB) should be provided in steady-state conditions, using reliable methods of reposition and retention of the fixed bone fragments. The strategy of treatments of fractures of DMRB first consisting in closed reposition and in surgery in case of failure or secondary displacement is thought to be outdated. In patients with symptoms of unstable fractures active surgical strategy should be used before complications appear.

Femoral morphology due to impingement influences the range of motion in slipped capital femoral epiphysis.

Femoroacetabular impingement due to metaphyseal prominence is associated with the slippage in patients with slipped capital femoral epiphysis (SCFE), but it is unclear whether the changes in femoral metaphysis morphology are associated with range of motion (ROM) changes or type of impingement. We asked whether the femoral head-neck junction morphology influences ROM analysis and type of impingement in addition to the slip angle and the acetabular version. We analyzed in 31 patients with SCFE the relationship between the proximal femoral morphology and limitation in ROM due to impingement based
