Glaucoformer: Dual-Domain Global Transformer Network for Generalized Glaucoma Stage Classification.

Classification of glaucoma stages remains challenging due to substantial inter-stage similarities, the presence of irrelevant features, and subtle lesion size, shape, and color variations in fundus images. For this purpose, few efforts have recently been made using traditional machine learning and deep learning models, specifically convolutional neural networks (CNN). While the conventional CNN models capture local contextual features within fixed receptive fields, they fail to exploit global contextual dependencies. Transformers, on the other hand, are capable of modeling global contextual in

Domain generalization for voice-based cognitive impairment detection.

BACKGROUND: Voice biomarkers hold potential for early cognitive disorder detection, but variations in recording conditions across different environments present challenges for accurate diagnosis using artificial intelligence (AI) models. This study aims to develop a robust, generalizable model for reliably diagnosing cognitive impairments across varied datasets. METHODS: We implemented a domain generalization approach using an adapted Deep Domain-Adversarial Image Generation (DDAIG) framework. This method transforms input data to reduce center-specific characteristics and emphasizes domain-in

Natural language processing to identify suicidal ideation and anhedonia in major depressive disorder.

BACKGROUND: Anhedonia and suicidal ideation are symptoms of major depressive disorder (MDD) that are not regularly captured in structured scales but may be captured in unstructured clinical notes. Natural language processing (NLP) techniques may be used to extract longitudinal data on suicidal behaviors and anhedonia within unstructured clinical notes. This study assessed the accuracy of using NLP techniques on electronic health records (EHRs) to identify these symptoms among patients with MDD. METHODS: EHR-derived, de-identified data were used from the NeuroBlu Database (version 23R1), a lon

Early Screening of Autism in Toddlers via Express-Needs-With-Pointing Protocol.

The incidence of autism spectrum disorders (ASD), a neurodevelopmental condition associated with challenges in social communication, has witnessed a remarkable surge in recent years, with adverse effects on individuals, families, and society at large. Early screening for autism ensures timely access to interventions, yet screening lacks systematic and methodical approaches for objectively quantifying social behaviors. In response to this, we propose a protocol for early assistive screening, termed the Express-Needs-with-Pointing (ENP), which employs a multi-sensor platform to quantify the one

Machine learning-based stratification of mild cognitive impairment in Parkinson's disease: a multicenter cross-sectional analysis.

BACKGROUND: Cognitive impairment is a prominent non-motor manifestation of Parkinson's disease (PD) and is associated with reduced quality of life, increased mortality, and higher healthcare utilization. We aimed to develop and externally validate a machine-learning model, trained on Montreal Cognitive Assessment (MoCA)-based Movement Disorder Society (MDS) Level I labels, that estimates the contemporaneous probability of mild cognitive impairment in PD (PD-MCI) from routinely collected clinical variables, enabling clinicians to prioritize MoCA-normal patients with higher model-estimated proba

Contrastive Learning With Transformer to Predict the Chronicity of Children With Immune Thrombocytopenia.

Immune thrombocytopenia (ITP) is a typically self-limiting and immune-mediated bleeding disorder in children. Approximately 20% of children with ITP experience chronicity, leading to reduced quality of life and increased treatment burden. The accurate prediction of chronicity would enable clinicians to make personalized treatment plans at an early stage. However, due to the self-limiting nature of ITP and the scarcity of available children patients, the data presents two prominent issues: small data and imbalanced class, which are unfavorable for effectively training a deep learning model. To

Harmonic Wavelet Neural Network for Discovering Neuropathological Propagation Patterns in Alzheimer's Disease.

Emerging researchindicates that the degenerative biomarkers associated with Alzheimer's disease (AD) exhibit a non-random distribution within the cerebral cortex, instead following the structural brain network. The alterations in brain networks occur much earlier than the onset of clinical symptoms, thereby affecting the progression of brain disease. In this context, the utilization of computational methods to ascertain the propagation patterns of neuropathological events would contribute to the comprehension of the pathophysiological mechanism involved in the evolution of AD. Despite the enco

Enhancing Ultrasound-Based Diagnosis of Unilateral Diaphragmatic Paralysis With a Visual Transformer-Based Model.

This paper presents a novel methodology that combines a pre-trained Visual Transformer-Based Deep Model (ViT) with a custom denoising image filter for the diagnosis of Unilateral Diaphragmatic Paralysis (UDP) using Ultrasound (US) images. The ViT is employed to extract complex features from US images of 17 volunteers, capturing intricate patterns and details that are critical for accurate diagnosis. The extracted features are then fed into an ensemble learning model to determine the presence of UDP. The proposed framework achieves an average accuracy of 93.8% on a stratified 5-fold cross-valid

Predicting generalized anxiety disorder among Chinese depressed adolescents: an explainable machine learning approach.

BACKGROUND: The co-occurrence of depression and anxiety in adolescents is associated with a greater risk than the presence of depressive symptoms alone. Predicting anxiety disorders among depressed adolescents is critical for interventions and therapeutic tools. METHODS: We recruited 2316 depressed adolescents through the Chinese Adolescent Depression Cohort (CADC) and collected 34 predictive factors for model construction. The Light Gradient Boosting Machine (LightGBM) prediction model and Shapley Additive Explanations (SHAP) algorithm were implemented for in-depth interpretation of the pred

Dual-Scale Swin Transformer via Feature Alignment and Adversarial Discrimination for Retinopathy of Prematurity Diagnosis.

Retinopathy of prematurity (ROP) is a retinal vascular disease that primarily affects premature infants with low birth weight. It is a leading cause of childhood blindness worldwide, but it can often be effectively managed with appropriate and timely diagnosis and treatment. To address the impact of image style on model classification performance, this paper proposes a dual-scale Swin Transformer (DS-Swin-T) network for ROP. The network comprises three components: image synthesis (IS), feature alignment, and advanced adversarial learning. The IS module generates synthesis style images as an in

Breathing Cycle-Aware Segmentation for Patient-Ventilator Asynchrony Detection.

Patient-ventilator asynchrony (PVA) is a significant challenge in mechanical ventilation, affecting approximately 25% of intensive care unit patients and increasing the risk of lung and diaphragm injury. Segmenting breathing cycles from long ventilation waveforms is essential for the reliable detection of PVA events. However, existing segmentation methods present several limitations: manual annotation is time-consuming; fixed-length window and rule-based segmentation methods lack adaptability to varying respiratory patterns; and supervised deep learning (DL) segmentation methods require large

Self-Organized Prediction-Classification- Superposition of Longitudinal Cognitive Decline in Alzheimer's Disease: An Application to Novel Clinical Research Methodology.

Progressive cognitive decline spanning across decades is characteristic of Alzheimer's disease (AD). Various predictive models have been designed to realize its early onset and study the long-term trajectories of cognitive test scores across populations of interest. Research efforts have been geared towards superimposing patients' cognitive test scores with the long-term trajectory denoting gradual cognitive decline, while considering the heterogeneity of AD. Multiple trajectories representing cognitive assessment for the long-term have been developed based on various parameters, highlighting
