A Case Report of Orbital Venolymphatic Malformation - A diagnostic dilemma.

INTRODUCTION: Intermittent proptosis is a transitory position-dependent protrusion of one or both eyeballs beyond orbit. The common causes of proptosis in young females are tumors, inflammation, trauma, thyroid eye disease and vascular lesions. The case report highlights the importance of imaging in prone position. CASE: We report a rare case of a 24-year-old female who recently developed intermittent proptosis while bending forwards. The patient was evaluated clinically. Ultrasonography B scan, Computerised axial tomography (CT), Magnetic resonance imaging (MRI) were advised to rule out orbi

TIMP3 c.319C>T, p.(Arg107Cys): Novel Sequence Variant In Sorsby Fundus Dystrophy.

INTRODUCTION: Sorsby fundus dystrophy is a rare autosomal dominant inherited retinal disease. The purpose of this case report is providing evidence to link the novel variant TIMP3 c.319C>T, p.(Arg107Cys), classified as variant of uncertain significance, to the clinical phenotype and to consider assignment of pathogenicity. CASE: Thorough history and comprehensive ophthalmological exam of a 51-year old female with presenile cataract and difficulty in night vision were conducted. Visual acuity was 0.15 logMAR and 0.05 logMAR in the right and left eye, respectively. OBSERVATIONS: The examinatio

Keratoconus associated with Fuchs' Corneal Endothelial Dystrophy in a Young Woman.

INTRODUCTION: Keratoconus has association with various ocular diseases. Here a case of the co-occurrence of keratoconus and Fuchs' corneal endothelial dystrophy (FCED), a rare entity has been reported. CASE: A 26-year-old woman presented with a sudden onset of blurring of vision of right eye (RE) three weeks back. OBSERVATION: The RE showed features of advanced keratoconus with apical scaring due to acute hydrops. Her left eye (LE) had myopic astigmatism with Vogt's striae. Careful examination revealed corneal guttate in both eyes. The co-existence of keratoconus and FCED was confirmed by to

Surgical Management of Glaucoma in Eyes with Ocular Cicatricial Pemphigoid.

INTRODUCTION: Glaucoma in ocular cicatricial pemphigoid (OCP) is an important blinding complication. Glaucoma in eyes with OCP is most often managed medically as surgical management is difficult in eyes with cicatrised conjunctiva and often end up with complications or scarring and failure. In some cases where the topical antiglaucoma medications are not tolerated or worsen the OCP and in eyes with medically uncontrolled glaucoma, surgical management would be needed. The glaucoma drainage devices (GDD) are more suitable in these eyes compared to trabeculectomy in these eyes. CASE: Here two ca

Use of Bedside Ultrasound for Ophthalmic Evaluation in a Case of Traumatic Brain Injury: A Case Report.

BACKGROUND: Ophthalmological examination is an important aspect of the neurological assessment in a patient with traumatic brain injury. However, significant periorbital swelling can make direct visualization of the pupils difficult. Ultrasonic examination can be a valuable bedside tool when direct visualization fails. CASE: A case of a seven-year female child who had presented with a history traumatic brain injury is reported here. OBSERVATIONS: The periorbital swelling prevented the direct visualization of the eyes. Bedside evaluation with a multipurpose ultrasound (USG) was used to monito

It involves the third and fourth decade of life as a gradual painless enlargement of the lacrimal gland. CASE: This is a case report of a 30-year-old female presenting with the forward bulging of the right eye causing the eye to be displaced medio-inferiorly over the course of one year. INTRODUCTION: Accurate refractive error assessment is the first and the most important step of ocular examination. A reliable screening method of refractive status is important for early detection and prevention of blinding complications of uncorrected refractive error. OBJECTIVES: To evaluate the comparability of astigmatism measured by an auto-refractometer (URk-800F) with the measurements of the gold standard anterior segment optical coherence tomography (AS-OCT). MATERIALS AND METHODS: This cross-sectional observational study was conducted in a tertiary eye centre in Kathmandu from Febru

INTRODUCTION: Renal diseases are on rise globally due to increased incidence of non-communicable diseases as well as primary-kidney diseases and frequent use of nephrotoxic drugs. Only definite treatment of End-Stage-RenalDisease (ESRD) is renal transplantation. Immuno-suppressive-drugs are prescribed lifelong after renal transplantation especially steroids which can lead to various sight-threatening complications. METHODS: This cross sectional, observational study included 62 eyes of 31 patients who had undergone renal transplantation, at least 3 months prior were referred from Nephrology De

Ocular Manifestations Leading to a Diagnosis of Joubert Syndrome Related Disorder.

INTRODUCTION: Joubert syndrome is an inherited disorder of rare occurrence usually presenting as developmental delay, hypotonia, hyperpnea and ataxia. The diagnosis is confirmed by characteristic findings in neuroimaging. Involvement of ocular, renal and hepatic systems can be present. Joubert syndrome presenting first to an ophthalmologist is very uncommon. CASE: A twenty-one-year female, with history of delayed milestones, infantile hemiplegia with hearing and visual impairment was referred for visual assessment. On systemic examination, ataxic gait was present. CT head showed hypoplasia of
