[Etiology and clinical course of urinary tract infections in infants less than 3 months-old].

BACKGROUND: Infants less than 3 months of age with urinary tract infection are usually hospitalized. Recent studies show that a less aggressive management for those patients aged ≥ 29 days may be feasible. OBJECTIVES: To determine the complication rate in infants<3 months of age with urinary tract infection, and to identify the causative agents and their antibiotic susceptibility. METHODS: A retrospective study was conducted on infants<3 months of age with positive urinalysis results, together with a positive urine culture from a catheterized specimen and seen in the Emergency Department fro

Current status and future of lung transplantation.

Lung transplantation has been performed successfully outside Japan since 1983 in patients with end-stage lung disease. More than 9,000 lung transplants have been reported in The Registry of the International Society for Heart and Lung Transplantation. In contrast, a transplant law became effective in Japan only recently, and four universities were designated as official lung transplant centers (Okayama, Osaka, Kyoto and Tohoku Universities). In October 1998, the first successful living-donor lobar lung transplantation was performed at Okayama University. Since then, seven lung transplants (fou

Analysis of time-dependent risks for infection, rejection, and death after pulmonary transplantation.

Infection and rejection remain the greatest threats to the survival of pulmonary allograft recipients. Furthermore, a relationship may exist between these events, because the occurrence of one may predispose to the other. By using multivariate analysis for repeated events, we analyzed the risk factors for bacterial, fungal, and viral infection, grade II or greater acute rejection, and death among 239 lung transplant recipients who received 250 allografts between January 1988 and September 1993. A total of 90 deaths, 491 episodes of acute rejection, and 542 infectious episodes occurred during a

Viruses associated with acute respiratory infections 1961-71.

Influenza-like illness, cold and sore throat was the diagnosis given in over 80% of 5177 acute respiratory illnesses in patients swabbed over a 10-year-period. A pathogenic organism was isolated twice as frequently from patients with a sore throat or an influenza-like illness as from those diagnosed as suffering from croup or laryngitis and bronchitis. A laboratory diagnosis was commoner in school children than in older or younger persons. Most of the organisms isolated were found in association with all types of acute respiratory illness but, with increasing age of the patient, one particular

D-Dimer in Viral Respiratory Infections: Established Evidence in COVID-19 and Emerging Evidence in Other Respiratory Viruses.

Viral respiratory infections remain a major cause of global morbidity and mortality, with severe disease increasingly recognized to involve profound interactions between inflammation, endothelial dysfunction, coagulation activation, and immunothrombosis. D-dimer, a fibrin degradation product and established biomarker of fibrin turnover, has gained considerable attention because of its strong association with adverse outcomes in coronavirus disease 2019 (COVID-19). This narrative review compares the evidence for D-dimer in COVID-19 with that available for other respiratory viruses and deliberat

Potential association of bronchoalveolar lavage lipid-laden macrophages with disease severity and corticosteroid response in interstitial lung disease: a pilot study.

BACKGROUND: Lipid-laden macrophages (LLMs) have been shown to be associated with lung inflammation and fibrosis. However, the clinical significance of LLM in interstitial lung disease (ILD) remains unclear. We conducted a retrospective study with the aim of evaluating whether the LLM index in bronchoalveolar lavage fluid (BALF) is a reliable biomarker reflecting disease severity and responsiveness to steroid treatment in patients with ILD. METHODS: Patients who underwent bronchoscopy with Oil Red O staining for the evaluation of ILD between 2022 and 2023 were consecutively recruited. ILD was

Prediction of Long-Term Mortality in Acute Hypercapnic Respiratory Failure with Use of Low-Flow Veno-Venous Extracorporeal CO2 Removal: A Retrospective Single-Center Study.

UNLABELLED: <p>Introduction: Hypercapnic respiratory failure is associated with high morbidity and mortality. Low-flow extracorporeal CO2 removal (ECCO2R) has been shown to facilitate lung-protective ventilation or spontaneous breathing. However, three multicenter randomized trials have failed to show benefit which could potentially be a result of patient selection. In this study, we aimed to characterize prognostic scores developed for extracorporeal membrane oxygenation therapy which could potentially assist with the selection of patients for ECCO2R. METHODS: A total of 70 patients admitted

Airway protease availability governs a reversible shift between syncytial and cell-free spread.

UNLABELLED: Paramyxoviruses such as respiratory syncytial virus (RSV) and human parainfluenza virus 3 (HPIV3) spread in the airway by releasing virions and by forming syncytia. Spread phenotypes are typically attributed to the fusion protein cleavage site, yet it remains unclear whether, and if so, how, host protease context biases syncytial versus cell-free spread. We compared RSV and HPIV3-two closely related viruses with overlapping epithelial tropism but distinct F cleavage-site composition (polybasic, furin-type for RSV versus a distinct, less polybasic cleavage-site motif for HPIV3)-in r

Inhalation Injury: Update on Pathogenesis, Emerging Hazards, and Management.

Inhalation injury, the pulmonary and systemic damage caused by a single or brief exposure to toxic gases, vapors, aerosols, or particulate matter, encompasses exposure categories ranging from structural-fire smoke and industrial chemical releases to wildfire smoke, earthquake-generated dust, and novel consumer products. Relevant exposures differ markedly in timescale, and four entities are distinguished: acute inhalation injury; subacute toxic pneumonitis, which follows repeated exposure peaks or sustained exposure over weeks to months; chronic inhalational disease; and population-level enviro

Long-term outcomes of the international EXPAND trial of Organ Care System (OCS) Lung preservation for lung transplantation.

BACKGROUND: Portable ex vivo lung perfusion and ventilation with the Organ Care System (OCS) Lung system is a safe, effective method for preserving extended criteria donor (ECD) organs before transplant. Although this technology is increasingly used in the United States, no published data describe its effects on long-term graft function and patient outcomes. This study assessed long-term clinical outcomes after transplantation of ECD lungs that were preserved, recruited, and assessed with the OCS Lung. METHODS: The EXPAND Lung Trial was a prospective, single-arm, multicenter, international tr

Epidemiology of respiratory syncytial virus in Central Queensland, Australia.

Respiratory syncytial virus (RSV) is the leading cause of bronchiolitis and pneumonia in infants. Little is known about the epidemiology, burden, and seasonality of RSV in subtropical regions of Australia like Central Queensland. This information is important to plan prevention strategies, including therapeutics, future vaccines, and health system preparedness. We collected data on laboratory-confirmed RSV cases and admissions in Central Queensland for the period 1 July 2021 to 31 December 2022. From July 2021, RSV was listed as a nationally notifiable condition on laboratory-confirmed diagnos

The Effects of Vitamin D Supplementation on Respiratory Infections in Children under 6 Years Old: A Systematic Review.

Childhood respiratory tract infections (RTIs) pose a significant health burden, especially in children under six years old. The main objective of this systematic review was to assess the effectiveness of vitamin D supplementation in the prevention of RTI in this population while also exploring potential effect modifiers such as age, baseline vitamin D status, and type of respiratory infection. A systematic review of the literature published up to February 2023 was conducted according to PRISMA guidelines, searching PubMed, Web of Science, Cochrane, and Scopus databases. Eight studies met the i

A Case of Vaping TCH Oil Leading to Vaping Associated Pulmonary Injury: Our Approach to Its Diagnosis, Management, and Recommendations.

Vaping's popularity has grown exponentially since its introduction to the US market in 2003. Its use has sky-rocketed since the unveiling of the vaping pods in 2017 which may account for the advent of the vaping related illnesses we are now seeing. Substances such as nicotine solution, tetrahydrocannabinol (THC) oil, cannabidiol (CBD) oil, and butane hash oil (BHC) packaged in cartridges available in various flavors and concentrations are aerosolized by the heating of metal coils in the e-cigarette/vaping devices. Cases from all over the country have recently been coming to light in which vapi

Immunomodulation by hyperimmunoglobulins after solid organ transplantation: Beyond prevention of viral infection.

Hyperimmunoglobulins are pharmaceutical formulations of human IgG which contain high titers of antibodies against specific viruses. They have been successfully used in solid organ transplantation (SOT) to prevent Cytomegalovirus (CMV) and Hepatitis B Virus (HBV) infection. The introduction of effective and cheaper antiviral drugs has resulted in decreasing usage of hyperimmunoglobulins in SOT. However, it may still be attractive to combine antiviral drug therapy with hyperimmunoglobulins after SOT, as there is some evidence that hyperimmunoglobulins, similar to high doses of intravenous immuno
