Development of a clinical prediction rule for the diagnosis of cubital tunnel syndrome in Thai wheelchair users.

PURPOSE: To develop and internally validate a clinical prediction rule (CPR) for diagnosing cubital tunnel syndrome (CuTS) in wheelchair users. To the authors' knowledge, no prior diagnostic CPR for CuTS was developed. METHODS: Participants were wheelchair users with spinal cord injuries/lesions aged ≥20 years. All underwent clinical examinations and electrodiagnostic studies. Clinical endpoint was a confirmed CuTS diagnosis; clinical symptoms and positive electrodiagnostic criteria. The CPR was developed using multivariable logistic regression with backward elimination. Coefficients of the s

Nonoperative and Operative Soft-Tissue, Cartilage, and Bony Regeneration and Orthopaedic Biologics of the Elbow and Upper Extremity: An Orthoregeneration Network Foundation Review.

Orthoregeneration is defined as a solution for orthopaedic conditions that harnesses the benefits of biology to improve healing, reduce pain, improve function, and, optimally, provide an environment for tissue regeneration. Options include drugs, surgical intervention, scaffolds, biologics as a product of cells, and physical and electromagnetic stimuli. The goal of regenerative medicine is to enhance the healing of tissue after musculoskeletal injuries as both isolated treatment and adjunct to surgical management, using novel therapies to improve recovery and outcomes. Various orthopaedic biol

Prediction of 7-Day Readmission Risk for Pediatric Trauma Patients.

BACKGROUND: Pediatric patients admitted for trauma may have unique risk factors of unplanned readmission and require condition-specific models to maximize accuracy of prediction. We used a multicenter data set on trauma admissions to study risk factors and predict unplanned 7-day readmissions with comparison to the 30-day metric. METHODS: Data from 28 hospitals in the United States consisting of 82,532 patients (95,158 encounters) were retrieved, and 75% of the data were used for building a random intercept, mixed-effects regression model, whereas the remaining were used for evaluating model

Evaluation of Biceps Tenotomy or Tenodesis on Fatty Infiltration of the Biceps Muscle.

Objective  The objective of the present study was to determine whether there is fatty infiltration (FI) of the biceps brachii muscle mass after tenotomy or tenodesis for the treatment of tendon injuries in the long head of the biceps and to establish a relationship between FI with changes in the length of muscle fibers. Methods  Clinical and imaging analysis of 2 groups of patients (biceps tenodesis [16 patients] and biceps tenotomy [15 patients]). In both groups, we compared the findings on the contralateral side of each patient (control group). All patients had undergone unilateral biceps te

Management of supracondylar fractures of the humerus in children.

Supracondylar fractures of the humerus are the most frequent fractures of the paediatric elbow, with a peak incidence at the ages of five to eight years.Extension-type fractures represent 97% to 99% of cases. Posteromedial displacement of the distal fragment is the most frequent; however, the radial and median nerves are equally affected. Flexion-type fractures are more commonly associated with ulnar nerve injuries.Concomitant upper-limb fractures should always be excluded. To manage the vascular status, distal pulse and hand perfusion should be monitored. Compartment syndrome should always be

The inter-teres approach to glenoid neck fractures: an alternative approach to glenoid fixation.

BACKGROUND: Scapula fractures are rare injuries that are generally treated nonoperatively. When surgery is performed, it is commonly undertaken through the posterior approach, which can be invasive and unforgiving on the soft tissues. We describe an alternative safe approach between teres major and minor that remains deep to a fascial sling formed by the combined infraspinatus and teres minor fasciae and deep to the primary nerve to teres minor, which is a terminal branch of the axillary nerve. METHODS: Between January 2008 and June 2014, there were 22 patients who underwent scapula fixation

Paediatric supracondylar humeral fractures: a technique for safe medial pin passage with zero incidence of iatrogenic ulnar nerve injury.

INTRODUCTION: The cross Kirschner wire (K-wire) configuration in closed reduction and percutaneous pinning of paediatric supracondylar humeral fracture affords superior stability. However, medial pin placement presents a risk of iatrogenic ulnar nerve injury. This study describes, in step-by-step detail, another safe method of percutaneous medial pin insertion. METHODS: The technique involved placing the patient's arm in external rotation, with elbow flexed no more than 45° after closed reduction. The surgeon held the K-wire close to its sharp end to pass it percutaneously onto the medial epi

Is an extension of the safe zone possible without jeopardizing the proximal radioulnar joint when performing a radial head plate osteosynthesis?

BACKGROUND: Proximal radial fractures are common elbow injuries. Because of the fracture pattern, stability criteria, or plate configuration, a plate position outside the "safe zone" (SZ) may be required in some cases when performing a radial head plate osteosynthesis. We examined the gross anatomy of the radial head and analyzed different so-called low-profile and precontoured radial head and neck plates with respect to the SZ. MATERIALS AND METHODS: Macroscopic measurements of the radial head and neck of 22 formalin-fixed human cadaveric upper extremities were obtained. The SZ was determine…

Effects of additional external weight on posture and movement adaptations to fatigue induced by a repetitive pointing task.

Fatigue and additional weight are risk factors of injuries by falls. Posture and trunk movement changes occur with fatigue induced by a repetitive pointing task. These changes facilitate arm movement but they may also jeopardize postural stability. When equilibrium is challenged, e.g. with additional weight, strategies that represent less postural threat could develop with fatigue. Nineteen participants performed two sessions (without, with 20% body weight added load (Load)) of a repetitive pointing task until shoulder fatigue (8 on Borg CR-10). There was no difference in time to fatigue betwe

A Brain Motor Control Assessment (BMCA) protocol for upper limb function.

UNLABELLED: The Brain Motor Control Assessment (BMCA) protocol is a surface electromyography (sEMG)-based measure of motor output from central nervous system during a variety of reflex and voluntary motor tasks performed under strictly controlled conditions. The aim of this study was to evaluate the BMCA protocol for upper limb with the addition of shoulder voluntary tasks. The voluntary response index (VRI) was calculated from quantitative analysis of sEMG data during defined voluntary movement in neurologically intact people for comparison with that of patients after neurological injuries. Treatment of diaphyseal forearm nonunions with interlocking intramedullary nails.

UNLABELLED: Fifteen patients with 26 nonunions of diaphyseal forearm fractures were treated with interlocking intramedullary nails and iliac bone grafts. The purpose of our study was to evaluate the rate of successful healing and complications that developed after using this technique. The mean followup was 31 months (range, 25-38 months). The average length of bone loss after freshening the bone ends was 20 mm (range, 10-30 mm). Radiographic union was achieved in 14 patients (96%). Compared with the uninjured contralateral arm, the mean loss of flexion and extension motion at the wrist was 27

The effect of tennis racket grip size on forearm muscle firing patterns.

BACKGROUND: Inappropriately sized tennis racket grip is often cited in the popular media as a risk factor for overuse injuries about the forearm and elbow. Currently, a hand measurement technique developed by Nirschl is commonly used by tennis racket manufacturing companies as the method for determining a player's "recommended" grip size. HYPOTHESIS: Quarter-inch changes from that recommended by Nirschl in tennis racket grip size will have no significant effect on forearm muscle firing patterns. STUDY DESIGN: Controlled laboratory study. METHODS: Sixteen asymptomatic Division I and II colle

Hybrid functional electrical stimulation orthosis system for the upper limb: effects on spasticity in chronic stable hemiplegia.

A new hybrid functional electrical stimulation orthosis system for the upper limb has been designed to allow for ease of use in the home as a daily treatment modality, as well as offer the opportunity for function enhancement. In a pilot study, the system was used by ten patients with chronic stable hemiparesis secondary to cerebral vascular accident and head injuries. The patients were referred by their treating physicians or therapists after meeting the inclusion criteria of good general health, being greater than one year after head injury, or being ten months post-stroke, with no observed

The musculocutaneous nerve and its branches to the biceps and brachialis muscles.

The musculocutaneous nerve and its motor branches to the biceps and brachialis were dissected and studied under the operating microscope in 24 fresh-frozen cadaveric specimens. The motor branch to the biceps exits from the musculocutaneous nerve at 119 mm distal to the coracoid process. Anatomic variations were seen in the innervation of the two heads of the biceps. A common primary motor branch that bifurcates to supply the two heads was seen in 20 specimens (type I). Two specimens were observed to have two separate primary branches originating from the main musculocutaneous nerve trunk to in
