Receipt of pharmacotherapy for alcohol use disorder by justice-involved women in the Veterans Health Administration.

BACKGROUND: Alcohol use disorder (AUD) and unhealthy drinking are prevalent among women involved in the criminal justice system and women military veterans. Pharmacotherapy-including naltrexone, topiramate, acamprosate, and disulfiram-for AUD is one form of effective treatment that is associated with better health and criminal justice outcomes. The current study examined the association of justice involvement with receipt of pharmacotherapy for AUD, as well as other patient factors that may explain variation in receipt of pharmacotherapy for AUD among women veterans who receive care at Veteran…

Does family history of alcoholism moderate naltrexone's effects on alcohol use?

OBJECTIVE: Primary outcomes from the COMBINE Study indicated support for naltrexone (Revia) on measures of abstinence and time to heavy drinking; however, effect sizes were modest. The delineation of individual difference variables that qualify these results could aid efforts to target treatment approaches appropriately. Laboratory and clinical studies have found greater effectiveness of naltrexone among men and those with familial alcoholism. The present study used multilevel modeling to investigate family history of alcoholism (FHA) based on first-degree relatives and gender as moderators of

[Recent results in relaps prevention of alcoholism with Disulfiram].

OBJECTIVE: For more than 50 years, disulfiram has been approved for the treatment of chronic alcohol dependence. In the last years there has been observed an increase in the prescription of disulfiram in germany. It acts as a psychological deterrence of a possible disulfiram-alcohol reaction. This paper describes the current clinical impact and possible future of disulfiram. METHODS: Clinical trials using disulfiram for the treatment of alcohol dependence were discussed. Furthermore, the options of combining disulfiram with novel anti-craving agents were considered. Moreover, experiences and

Therapeutics for alcoholism: what's the future?

As with other addictions, human alcoholism is characterised as a chronically relapsing condition. Consequently, the therapeutic goal is the development of clinically effective, safe drugs that promote high adherence rates and prevent relapse. These products can then be used in conjunction with psychosocial approaches. In this review, preclinical studies are highlighted that indicate the mechanism of action of currently used anti-craving medications or demonstrate the potential of novel pharmacological agents for the treatment of alcohol use disorders. While current pharmacological strategies a

NEUROBIOLOGICAL BASES OF ALCOHOL ADDICTION.

Alcohol addiction is a heterogeneous psychiatric disorder according to both phenotype and etiology. Difference in phenotype characteristics manifests in the manner the addiction arises, history of the alcoholic and history of drinking, comorbid disorders, and the phenomenon of abstinence difficulties. Concerning the etiology of alcoholism, the disease itself is considered to be a consequence of an interactive influence of the environment and genetic factors. Numerous researches conducted in the last decades discovered many aspects of the biochemical, cell and molecular bases of alcohol addicti

Boosted Regeneration and Reduced Denervated Muscle Atrophy by NeuroHeal in a Pre-clinical Model of Lumbar Root Avulsion with Delayed Reimplantation.

The "gold standard" treatment of patients with spinal root injuries consists of delayed surgical reconnection of nerves. The sooner, the better, but problems such as injury-induced motor neuronal death and muscle atrophy due to long-term denervation mean that normal movement is not restored. Herein we describe a preclinical model of root avulsion with delayed reimplantation of lumbar roots that was used to establish a new adjuvant pharmacological treatment. Chronic treatment (up to 6 months) with NeuroHeal, a new combination drug therapy identified using a systems biology approach, exerted lon

Alcohol dependence: analysis of factors associated with retention of patients in outpatient treatment.

AIMS: To identify factors associated with retention in treatment of alcohol-dependent individuals and to compare treatment retention between men and women. METHODS: Analysis of the treatment attendance records and baseline characteristics of 833 men and 218 women who undertook to attend follow-up treatment in an alcoholism treatment centre. RESULTS: Retention after 4 weeks of treatment is more likely to occur among those using adjuvant medication (the most frequent of which was disulfiram), those presenting severe alcoholism and those who are older and tend to be frequent drinkers. There was

Animal models of craving for ethanol.

Craving has various meanings but can be defined generally in terms of a desire for the previously experienced effects of ethanol. Animal models provide a means by which to study the underlying mechanisms associated with craving and are most useful when they fulfill the requirements for predictive validity and reliability. Craving is a key part of the process of addiction that can lead to relapse and is conceptualized as having at least three components: preoccupation/anticipation, binge/intoxication and withdrawal/negative affect. Animal models of craving are hypothesized at this time to invol

COMBINE genetics study: the pharmacogenetics of alcoholism treatment response: genes and mechanisms.

OBJECTIVE: Partial efficacy of treatment and differences in adverse events across individuals are a challenge and an opportunity in the treatment of alcoholism. Individuation of therapy and understanding origins of differential treatment response may require identification of inherited functional variants of genes. The neurobiology of reward, executive cognitive function, anxiety and dysphoria have been identified as critical domains that may have a genetic basis that could predict treatment response. METHOD: The COMBINE Study presents a unique opportunity to evaluate specific genetic loci (m

Effect of neramexane on ethanol dependence and reinforcement.

It has been suggested that drugs modulating the glutamate/N-methyl-D-aspartate (NMDA) receptor system may be useful in the treatment of alcohol dependence. The effect of neramexane, a low-to-moderate affinity uncompetitive NMDA receptor antagonist, was examined on the development and expression of ethanol dependence (withdrawal-associated audiogenic seizures) and ethanol-induced conditioned place preference. Neramexane hydrochloride (3.5 mg/kg and higher) inhibited both the development and expression of ethanol dependence. Neramexane hydrochloride also inhibited the acquisition (1.75 mg/kg and

Addiction medicine for the primary care physician.

Addiction and the consequences including medical, social, and legal are seen almost daily in a primary care office. To effectively deal with these problems it is imperative that the physician view addiction as a disease. This article explores the disease concept and the basis for the view as well as providing a guide to diagnosis and assessment. Useful screening tools are also provided as well as a discussion of biochemical markers currently available. A guideline for delivering the diagnosis is also discussed. An overview of the stages of treatment is provided as well, with particular emphasi

What happens when people discontinue taking medications? Lessons from COMBINE.

AIMS: We use intensive longitudinal data methods to illuminate processes affecting patients' drinking in relation to the discontinuation of medications within an alcohol treatment study. Although previous work has focused on broad measures of medication adherence, we focus on dynamic changes in drinking both before and after patients discontinue. DESIGN: We conducted secondary data analyses using the COMBINE (Combined Pharmacotherapies and Behavioral Interventions for Alcohol Dependence) study, focused on participants who discontinued medications prior to the planned end of treatment.
