Impact of Adenotonsillectomy on Quality of Life in Pediatric Obstructive Sleep Apnoea (OSA): Insights from the OSA-18 Questionnaire.

PURPOSE: Hypertrophy of the adenoids and tonsils is a common cause of obstructive sleep apnoea (OSA) in children, with adenotonsillectomy as the primary treatment. This study aimed to assess the efficacy of surgical options (adenotonsillectomy, adenoidectomy, and tonsillectomy) in managing pediatric OSA and their impact on quality of life, using the OSA-18 questionnaire. METHODS: This retrospective cohort study analyzed data from parents of 196 children who underwent adenoidectomy, tonsillectomy, or both. The OSA-18 questionnaire was administered online via Google Form to assess quality-of-li…

Values of mean platelet volume in patients with chronic tonsillitis and adenoid hypertrophy.

OBJECTIVES: Chronic tonsillitis (CT)-adenoid hypertrophy (AH) is the most common cause of obstructive sleep apnea (OSA), which is one of the most common reasons of nocturnal hypoxia in children. However, there is limited information about the relationship between childhood OSA and atherosclerosis or cardiac diseases. In the present study, we evaluated the relationship between mean platelet volume (MPV) and CT-AH which is the most frequent cause leading OSA in children. Methodology : The medical records of 200 children, who underwent adenoidectomy or adenotonsillectomy with a diagnosis of adeno…

A diagnostic approach to suspected obstructive sleep apnea in children.

Most children with obstructive sleep apnea will benefit from tonsillectomy and adenoidectomy. Although polygraphic monitoring remains the definitive diagnostic technique, we wondered if all children suspected of having OSA require such evaluation. We therefore administered a standardized questionnaire to the parents of 23 children with polygraphically proved OSA related to adenotonsillar hypertrophy, 46 age- and sex-matched normal children, and 23 children subsequently referred because of possible OSA. Significantly increased frequencies of the following symptoms were found in the OSA group co…

[The catarrhal child].

The first seven years of a child's development are often described as the catarrhal stage. This is because of the increased incidence of upper respiratory tract infections. Nasal obstruction in the children is common and symptoms are often distressing. The authors report a rational approach to this problem to produce effective treatment of recurrent nasal obstruction. In particular clinical features, diagnosis, and therapy of recurrent rhinitis, adenoid hypertrophy, and tonsillar obstruction are described. The indications for adenoidectomy, tonsillectomy or adenotonsillectomy has been question…

Temporal bone pneumatization. A planimetric study.

This present study is based on the results of a planimetric investigation of temporal bone pneumatization in 73 children (with and without middle ear pathology). Radiologic and clinical follow-up studies indicate a definite inhibition of pneumatization due to chronic secretory or recurrent suppurative otitis media. This process is partially reversible if the underlying pathologic ventilation of the middle ear spaces is cured by an adequate treatment, such as direct adenoidectomy and/or long-term middle ear ventilation (6 months indispensable).

[Grisel's syndrome after otoplasty].

Grisel's syndrome is known as a very rare complication of ENT surgery. It is described as non-traumatic atlantoaxial rotatory subluxation, often seen after tonsillectomy or adenoidectomy in children. Therapy is staged according to the Fielding classification. We report the case of a 9-year-old female patient with Grisel's syndrome after otoplasty. The diagnosis was confirmed by CT scan. Manual reposition was performed under general anaesthesia, followed by temporary immobilization with a Minerva orthesis.

Successful use of nasal-CPAP for obstructive sleep apnea in Hunter syndrome with diffuse airway involvement.

A patient with Hunter syndrome and diffuse airway obstruction had daytime hypersomnolence, snoring, and alveolar hypoventilation. Polysomnography showed severe obstructive sleep apnea. In the past, all reported cases of sleep apnea in patients with mucopolysaccharidoses had been treated with tonsillectomy/adenoidectomy or tracheostomy. This patient, in whom tracheostomy would have been very difficult due to the diffuse nature of his airway involvement, was successfully treated with high pressure nasal CPAP and supplemental oxygen.

Nasopharyngeal radium irradiation: The lessons of history.

In the Netherlands, nasopharyngeal radium irradiation was started in 1945. The indications included refractory symptoms of otitis media with effusion and other adenoid-related disorders after adenoidectomy. It was considered a safe and effective therapy. Its use decreased sharply in 1958, following a worldwide media avalanche around the dramatic events in the treatment of a 5-year-old child in Utrecht, enhancing the widespread fear of radioactivity. This case history illustrates the powerful role of the media in medical decision-making.

Pediatric lingual tonsillectomy.

Tonsillectomy and adenoidectomy are an effective surgical treatment of pediatric obstructive sleep apnea; however, up to 20% of these patients can have persistent disease. In this select patient population, the lingual tonsil may be an occult source of obstruction. Lingual tonsillectomy can be a challenging procedure due to poor access and visualization, airway edema, postoperative pain and hemostasis during tissue removal. In this chapter, we describe our preferred technique for lingual tonsillectomy including surgical pearls for success.

Tracheostomy in children with nemaline core myopathy.

Two children with nemaline core myopathy (NM) who required tracheostomy are reported. One had a severe neonatal form requiring tracheostomy in the neonatal period for continuous mechanical ventilation. The other had a milder form with obstructive sleep apnoea, who underwent tracheostomy at age 22 months after adenoidectomy and a trial of nocturnal nasal continuous positive airway pressure ventilation. These cases highlight the clinical heterogeneity of the condition and the important role of the otolaryngologist in its management.

Efficacy of medical treatment as an adjunct to surgery in the treatment of secretory otitis media.

In a pilot controlled randomised trial of 38 children who had bilateral secretory otitis media, with effusion demonstrated at operation, we compared the efficacy of a six-week course of an oral decongestant--antihistamine combination and a mucolytic preparation with a control group in preventing the presence of middle-ear effusion six weeks after myringotomy and adenoidectomy. The mucolytic preparation decreased the presence of middle-ear effusion when compared to the decongestant-antihistamine combination and the control group (p = 0.06).

Comparison of Ibuprofen vs Acetaminophen and Severe Bleeding Risk After Pediatric Tonsillectomy: A Noninferiority Randomized Clinical Trial.

IMPORTANCE: Ibuprofen is an effective analgesic after tonsillectomy alone or tonsillectomy with adenoidectomy, but concerns remain about whether it increases postoperative hemorrhage. OBJECTIVE: To investigate the effect of ibuprofen compared with acetaminophen on posttonsillectomy bleeding (PTB) requiring surgical intervention in children. DESIGN, SETTING, AND PARTICIPANTS: A multicenter, randomized, double-blind noninferiority trial was conducted at 4 tertiary medical centers (Massachusetts Eye and Ear Infirmary, Boston; Naval Medical Center, San Diego, California; Naval Medical Center, Po…

Tonsillotomy versus tonsillectomy on young children: 2 year post surgery follow-up.

ObjectivesTo study the long-term effect of tonsillotomy and tonsillectomy in young children after two years in comparison to the results after six months.MethodChildren, age 4-5 with Sleep Disordered Breathing (SDB) and tonsil hyperplasia, were randomized to TE (32) or TT (35). TT was performed ad modum Hultcrantz with radiofrequency technique (Ellman). An adenoidectomy with cold steel was performed in the same session for 80% of cases. The patients were assessed prior to surgery, at six and 24 months postoperatively. Effects of surgery were evaluated clinically, through questionnaire (general…
