Selective Tissue Penetration of the Corneal Layers of Cyclosporin 2% Associated with Miglyol in Rabbits.

Purpose: Cyclosporin A (CsA) is a drug used to prevent immune rejection in corneal transplantation. Most grafts performed today are endothelial grafts which are complicated with poor penetration of CsA into the endothelium due to its hydrophobicity. To improve CsA penetration into the corneal a new ocular formulation of CsA 2% with Miglyol was developed and is commercially available. The purpose of this pharmacokinetic study was to determine the concentrations of CsA in all layers of the cornea, in particular in the endothelium after topical administration of CsA 2%-Miglyol in rabbits. Methods

Impact of topical insulin as a therapeutic intervention for corneal wounds: A comprehensive systematic review and meta-analysis.

BackgroundPersistent epithelial defects (PEDs) are challenging corneal conditions with prolonged non-healing. Commonly associated with dry eye disease and neurotrophic keratopathy, PEDs can lead to severe complications. Topical insulin has emerged as a therapeutic option, promoting epithelial healing.MethodsWe searched Medline, Embase, Web of Science, and Cochrane Library databases following PRISMA guidelines for studies reporting topical insulin for PEDs of any etiology and corneal epithelial defect (CEDs) post-vitrectomy. Healing time, complete wound healing (CWH), epithelization failure (EF

Ophthalmic formulation of methotrexate: a strategy of using the self-assembled LacAC4A nanoparticles for non-invasive drug delivery to the ocular posterior segment.

Drug delivery to ocular posterior segment remains difficult due to the challenges imposed by dynamic and static ocular barriers, lesion point targeting, and off-target effect. In this study, a novel approach is demonstrated for non-invasive drug delivery to the ocular posterior segments using lactose-modified azocalix[4] arene (LacAC4A) as a supramolecular ocular drug delivery platform. LacAC4A contains azo groups and is covalently modified by lactose groups, which confers active targeting to the retina, and induces a hypoxic response. The immunomodulator methotrexate (MTX), which is commonly

Recent advances and future challenges in nanosystems for ocular drug delivery.

Ocular drug delivery faces significant challenges because of the eye's complexity and anatomical barriers, such as the cornea, conjunctiva, and blood-retinal barrier, which limit drug penetration and bioavailability. Recent advances in nanotechnology-based drug delivery have led to the development of innovative delivery platforms, enabling targeted, sustained, and minimally invasive delivery for ocular diseases and injuries. This review outlines the recent advances in nanosystems-based ocular drug delivery and highlights the latest progress in targeting technologies based mainly on preclinical

Urinary bladder retention secondary to intranasal administration of oxymetazoline "Afrin" for intra-arterial chemotherapy.

Selective ophthalmic artery infusion of chemotherapy (SOAIC) has emerged as the standard of care for retinoblastoma (RB). Intranasal oxymetazoline (INO), Afrin, is often intraoperatively administered adjunctively to optimize flow to the orbit. There has been one report to date that suggests the adjunctive use of INO has led to systemic side effects. To our knowledge, this is the first documented case of INO causing urinary retention in a patient undergoing SOAIC, and the recommended treatment.

Topical nonsteroidal anti-inflammatory therapy in ophthalmology.

Topically applied nonsteroidal anti-inflammatory drugs (NSAIDs) are commonly used in the management and prevention of ocular inflammation and cystoid macular edema related to cataract surgery and the maintenance of mydriasis during cataract surgery. Other common uses are the reduction of discomfort after refractive surgery or in allergic conjunctivitis. NSAIDs primarily act as cyclooxygenase inhibitors and thus reduce the formation of endogenous PGs. Today, several NSAIDs are commercially available: diclofenac, flurbiprofen, indomethacin, ketorolac and suprofen. At present the ophthalmologist

Anesthesia and preoperative and postoperative medications.

Retrobulbar blocks, although widely used, still have potentially serious complications. Topical anesthesia presents less risk of injury to the globe and less pain but requires careful usage and an experienced surgeon. New techniques, however, allow for an increase in the percentage of patients able to have topical anesthesia. Preoperatively, 2.5% phenylephrine is found to be just as effective as 10% phenylephrine, and, when compared with wound closure and surgeon's experience, the effect of prophylactic medications was found to be negated. Postoperatively, diclofenac is found to be as effectiv

Extemporaneous preparation of antibiotic ophthalmic solutions.

Until the introduction of ocular ciprofloxacin, the standard of practice in treating bacterial corneal ulcers required topical antibiotic ophthalmic solutions that were either not commercially available or not available in the higher concentrations needed. A survey was mailed to 125 hospital pharmacies and 125 community pharmacies in Oklahoma to determine the availability of extemporaneously prepared antibiotic ophthalmic solutions. Of 72 hospital pharmacies and 60 community pharmacies that responded, 17 hospital and 2 community pharmacies reported that they do compound these solutions. The ma

Corneoscleral ulceration and corneal perforation after pterygium excision and topical mitomycin C therapy.

A 78-year-old woman complained of ocular pain in the right eye. Thirty-three months before, the patient had a pterygium excised and mitomycin C, 0.04%, was instilled 3 times a day postoperatively for 10 days in the right eye. The same surgery was performed and the same drug was used for a course of 5 days in the left eye. Corneoscleral ulceration and corneal performation occurred in the right eye. The lesions were successfully treated with a lamellar patch graft with reserved corneosclera. We believe that it may not be safe to instill 0.04% mitomycin C 3 times a day postoperatively for 10 days
