Complicated iatrogenic Cushing's syndrome induced by topical clobetasol propionate in a child with psoriasis: a case report and review of the literature.

BACKGROUND: Long-term use of oral or parenteral corticosteroids is the most common cause of hypothalamic-pituitary-adrenal axis suppression and iatrogenic Cushing's syndrome. Still, iatrogenic Cushing's syndrome occurs rarely following the administration of topical corticosteroids. CASE PRESENTATION: This case study discusses the misuse of a high-potency corticosteroid cream by an Iranian 5-year-old male with plaque-form psoriasis, resulting in Cushingoid symptoms including moon face, buffalo hump, red striae, and weight gain. The child experienced different complications following iatrogenic

Topical use of cidofovir induced acute renal failure.

BACKGROUND: Cidofovir has antiviral activity against a wide spectrum of DNA viruses. Several small studies have focused on the efficacy of topical cidofovir in various viral-induced diseases. We report a systemic complication of such therapy. CASE REPORT: A bone marrow transplant recipient with chronic renal failure developed genital condylomas resistant to standard therapy. After topical cidofovir application (1% once daily for 5 days, then 4% for 12 days), the lesions improved while local erosions appeared. Acute renal failure with features of tubular acidosis occurred at day 19. Spontaneou

Diphenhydramine toxicity in three children with varicella-zoster infection.

Diphenhydramine hydrochloride is an antihistamine with anticholinergic properties that is frequently used both orally and topically for the temporary relief of pruritus. Significant systemic absorption may occur following topical administration of diphenhydramine in patients with varicella-zoster lesions. We describe three children with varicella-zoster infection (VZI) who developed bizarre behavior as well as visual and auditory hallucinations following topical applications of large amounts of diphenhydramine to the majority of skin surfaces. In two cases, oral diphenhydramine was also admini

Corneal Melt and Perforation Associated With Prolonged Oral Non-Steroidal Anti-Inflammatory Drug (NSAID) Use: A Case Report.

Corneal melt and perforation can arise from various etiologies, including the use of toxic topical drops, particularly topical non-steroidal anti-inflammatory drugs (NSAIDs). The literature has frequently documented the association between the use of topical NSAIDs and the subsequent development of corneal ulcers. More recently, reports have emerged linking the use of oral NSAIDs and colchicine to impaired corneal wound healing and corneal perforation. This case report presents an instance of corneal melting and subsequent perforation in a medically unburdened patient who had been self-adminis

Multidisciplinary Guidelines for the Rational Use of Topical Non-Steroidal Anti-Inflammatory Drugs for Musculoskeletal Pain (2022).

(1) Background: Topical non-steroidal anti-inflammatory drugs (NSAIDs) are one of the primary drugs for treating musculoskeletal pain. However, there are currently no evidence-based recommendations about drug selection, drug administration, drug interactions, and use in special populations or other pharmacology-related content of such medications. To this end, the Chinese Pharmaceutical Association Hospital Pharmacy Professional Committee developed multidisciplinary guidelines on using topical NSAIDs to treat musculoskeletal pain. (2) Methods: The guidelines development process followed the Wo

Adrenal Insufficiency Secondary to Abrupt Dose Reduction of Topical Corticosteroid Therapy after Starting Brodalumab for Psoriasis: A Case Report.

The risk of treating psoriasis with biologic drugs in patients treated with topical corticosteroids over prolonged periods requires careful attention to their underlying adrenal insufficiency because the development of adrenal insufficiency symptoms frequently occurs after cessation of the topical corticosteroids: the dose and duration of topical corticosteroid therapy and etretinate use correlate with risk. In this case report, we present a 65-year-old man with psoriatic erythroderma who developed arthralgia, joint pain, muscle pain, fatigue, and headache after starting brodalumab and a reduc

Gene Delivery into the Inner Ear and Its Clinical Implications for Hearing and Balance.

The inner ear contains many types of cell, including sensory hair cells and neurons. If these cells are damaged, they do not regenerate. Inner ear disorders have various etiologies. Some are related to aging or are idiopathic, as in sudden deafness. Others occur due to acoustic trauma, exposure to ototoxic drugs, viral infections, immune responses, or endolymphatic hydrops (Meniere's disease). For these disorders, inner ear regeneration therapy is expected to be a feasible alternative to cochlear implants for hearing recovery. Recently, the mechanisms underlying inner ear regeneration have bee

Use of Topical Tazarotene for the Treatment of Acne Vulgaris in Pregnancy: A Literature Review.

A concern with the increasing use of prescription drugs during pregnancy is teratogenic risk. This risk is undetermined for most drugs approved in the United States (US) from 2000 to 2010. Acne and psoriasis are chronic diseases that typically occur during the child-bearing years, and as topical retinoids are recommended for both acne and psoriasis treatment, is it possible for women to be exposed to a topical retinoid during pregnancy. Pharmacokinetic studies show relatively low systemic exposure from topical retinoids, but the exposure levels that could lead to teratogenicity in humans are u

A Comprehensive Review of Topical Therapies for Distal Ulcerative Colitis.

Patients with ulcerative colitis (UC) limited to distal segments of the colon and rectum are often poorly represented in large clinical therapeutic trials, yet they constitute up to two-thirds of all UC patients. The propensity of UC to be most severe distally has also resulted in many oral or systemic therapies with lower levels of therapeutic success and mucosal healing in the distal regions of the colon. Topically administered mesalamine and corticosteroid agents have been utilized for decades in patients with distal UC but are often poorly accepted by patients and their prescribing physici

Sodium bicarbonate attenuates myocardial ischemia-reperfusion injury via activation of the splenic cholinergic anti-inflammatory pathway.

OBJECTIVE: The spleen plays a central role in mediating inflammatory responses during myocardial ischemia-reperfusion injury (IRI) via a neuroimmune axis involving vagal signaling and cholinergic modulation. Sodium bicarbonate (NaHCO3) has been implicated in the activation of a splenic cholinergic anti-inflammatory pathway, but its role in myocardial IRI remains incompletely defined. The present study aims to determine whether NaHCO3 administration before reperfusion attenuates myocardial injury by modulating splenic inflammatory signaling. METHODS: C57BL/6 mice underwent 40 minutes of left c
