A miRNome analysis of drug-free manic psychotic bipolar patients versus healthy controls.

The lifetime presence of psychotic symptoms is associated with more clinical severity, poorer outcome and biological changes in patients affected by bipolar disorder (BD). Epigenetic mechanisms have been evoked to explain the onset of psychotic symptoms in BD as well as the associated biological changes. The main objective of the present study was to evaluate the expression profiles of circulating microRNAs (miRNAs) in drug-free manic psychotic bipolar patients versus healthy controls (HC), to identify possible non-invasive molecular markers of the disorder. 15 drug-free manic psychotic bipola

Evidence for the Risks and Consequences of Adolescent Cannabis Exposure.

OBJECTIVE: This review of the scientific literature examines the potential adult sequelae of exposure to cannabis and related synthetic cannabinoids in adolescence. We examine the four neuropsychiatric outcomes that are likely most vulnerable to alteration by early cannabinoid use, as identified within both the clinical and preclinical research: cognition, emotional functioning, risk for psychosis, and addiction. METHOD: A literature search was conducted through PubMed, PsychInfo, and Google Scholar with no publication date restrictions. The search terms used were "adolescent" and "adult," an

Substance use disorder and the early course of illness in schizophrenia and affective psychosis.

The relationship between a history of substance use disorder and the early course of psychotic illness was examined in 96 subjects with schizophrenia and 106 subjects with affective psychosis followed in the Suffolk County Mental Health Project, a longitudinal study of first-admission psychosis. Subjects received a structured diagnostic interview and clinical ratings at baseline assessment and again 6 months later. The 6-month assessment included information about treatment received during the interval. A lifetime history of substance use disorder was associated with worse clinical functioning

Tricyclic-induced mania and MHPG excretion.

In order to evaluate the presumed involvement of altered noradrenergic receptor sensitivity in the switch process from depression into mania, we explored the relationship between pretreatment 3-methoxy-4-hydroxy-phenylglycol (MHPG) and tricyclic-induced mania or hypomania in bipolar depressed patients. Within the group of patients developing mania or hypomania on tricyclics, there was a strong positive correlation between pretreatment 24-hour urinary MHPG and the latency of onset of the episode. This finding is consistent with both the reported differences in MHPG excretion between unipolar an

Long-term clinical course of patients with acute polymorphic psychotic disorder without symptoms of schizophrenia.

Few studies have investigated the long-term course of patients with acute polymorphic psychotic disorder, despite the clinical relevance. The present study focused on the frequency of acute polymorphic psychotic disorder without symptoms of schizophrenia (F23.0) and the prognosis of patients with the disorder over a long period. A total of 388 inpatients with nonaffective psychosis were examined and cases with F23.0 were retrospectively investigated regarding clinical course over 12 years with reference to clinical charts. A total of 16 cases were diagnosed with F23.0 according to first episod

Association of Magnetoencephalographically Measured High-Frequency Oscillations in Visual Cortex With Circuit Dysfunctions in Local and Large-scale Networks During Emerging Psychosis.

IMPORTANCE: Psychotic disorders are characterized by impairments in neural oscillations, but the nature of the deficit, the trajectory across illness stages, and functional relevance remain unclear. OBJECTIVES: To examine whether changes in spectral power, phase locking, and functional connectivity in visual cortex are present during emerging psychosis and whether these abnormalities are associated with clinical outcomes. DESIGN, SETTING, AND PARTICIPANTS: In this cross-sectional study, participants meeting clinical high-risk criteria for psychosis, participants with first-episode psychosis,

Race bias and gender bias in the diagnosis of psychological disorders.

Bias is said to occur when validity is better for one group than another (e.g., when diagnoses are more valid for male or female clients). This article provides (a) a methodological critique of studies on race bias and gender bias in diagnosis and (b) a narrative review of results from studies with good internal validity. The results suggest that race bias occurs for the diagnosis of conduct disorder, antisocial personality disorder, comorbid substance abuse and mood disorders, eating disorders, posttraumatic stress disorder, and the differential diagnosis of schizophrenia and psychotic affect

Annotation: velo-cardio-facial syndrome.

BACKGROUND: Velo-cardio-facial syndrome (VCFS), the most frequent known interstitial deletion identified in man, is associated with chromosomal microdeletions in the q11 band of chromosome 22. Individuals with VCFS are reported to have a characteristic behavioural phenotype with high rates of behavioural, psychiatric, neuropsychological and linguistic disorders. METHODS: A selective literature review was undertaken. RESULTS: Children and adults with VCFS have high rates of behavioural, psychiatric and communication disorders. While VCFS children have high rates of ADHD, anxiety and affective

Delusional (paranoid) disorders: etiologic and taxonomic considerations. II. A possible relationship between delusional and affective disorders.

Paranoid (delusional) disorders are usually thought to overlap with schizophrenic disorders, and there may be a continuum, especially with paranoid schizophrenia. There is also some recent evidence of an overlap with affective disorders. This article refers to the author's series of monodelusional disorders, emphasizing certain mood concomitants, and discussing the implications of these for delusional disorders in general.

Gender dysphoria symptoms in schizophrenia.

Gender dysphoria in individuals with schizophrenia may result from the delusionally changed gender identity or appear regardless of psychotic process. Distinguishing between these situations is not only a diagnostic challenge, but also affects the therapeutic decisionmaking. The review of the literature shows that different delusional beliefs regarding belonging to another gender, anatomy or changes within the genitals affect about one-fourth of patients with schizophrenia. Contemporary classifications of disorders are moving towards the elimination of psychotic disorders as a disqualifying cr

[Kurt Schneider's concept of cyclothymic mania in the light of a research-oriented catatonic syndrome].

Catatonic motor signs were specifically sought out in the case records of 89 episodes of Schneider-oriented mania. Only some so-called "minor" catatonic motor phenomena were found and "major" catatonic motor disturbances, recently reported to frequently occur in some American samples of mania, were completely absent. Indeed, the only motor abnormalities seen in Schneider-oriented mania appeared to be of a kind and degree typically associated with Kraepelins "hypomania". These findings are then primarily discussed in historical perspective and their relationships to some important modern resear

Structural brain networks in remitted psychotic depression.

Major depressive disorder with psychotic features (psychotic depression) is a severe disorder. Compared with other psychotic disorders such as schizophrenia, relatively few studies on the neurobiology of psychotic depression have been pursued. Neuroimaging studies investigating psychotic depression have provided evidence for distributed structural brain abnormalities implicating the insular cortex and limbic system. We examined structural brain networks in participants (N = 245) using magnetic resonance imaging. This sample included healthy controls (n = 159) and the largest cross-sectional sa

Psychotic symptoms in an urban general medicine practice.

OBJECTIVE: The authors' goals were to estimate the prevalence of psychotic symptoms among adults attending an urban general medical practice that serves a low-income population and to describe the mental health, social and occupational functioning, and mental health treatment of these patients. METHOD: Data were drawn from a recent study of adult primary care patients (N=1,005) in a large, urban, university-affiliated general medicine practice. During a medical visit, patients completed the psychotic disorders section of the Mini International Neuropsychiatric Interview, the Primary Care Eval

Understanding Psychotic Depression.

Major depression with psychotic symptoms (ICD-10) or psychotic features (DSM-IV) is relatively common and probably underdiagnosed. In recent years, there has been a renewed interest in better understanding the psychopathology of this disorder and in proposing more specific approaches. The DSM-5 provides a definition of psychotic symptoms that is independent of any concept of psychosis. In depressed patients, the presence of hallucinations or delusions, whatever their clinical form, generally leads to a diagnosis of psychotic depression and the prescription of neuroleptics. Psychotic symptoms a

The late-onset psychoses. Clinical and diagnostic features.

Psychoses of late onset are poorly understood due to a limited number of inconsistent studies. The authors conducted this study to determine the clinical characteristics of a clearly defined group of patients with onset of psychosis after age 65 years and to test the usefulness of DSM-III criteria in diagnosing the condition of these patients. Late-onset psychosis occurred in 8% of the patients admitted to the geropsychiatry unit during the study period. More than three quarters of these patients suffered from either an organic mental disorder or major affective disorder, the remainder having

A Systematic Review of Women's Cancer Screening and Mortality in Women with Severe Mental Illness.

BACKGROUND AND HYPOTHESIS: Breast and cervical cancer remain a significant cause of morbidity and mortality for women worldwide. To combat these high mortality rates, numerous countries have implemented national screening programs. While the relationship between mental illness and screening participation has been examined in past systematic reviews, this is the first systematic review specifically on women with severe mental illness (SMI), referring to women experiencing bipolar affective disorder (BPAD), schizophrenia (SCZ), or associated psychotic disorders. We hypothesized that in compariso

Categorical vs dimensional classifications of psychotic disorders.

OBJECTIVE: Both categorical and dimensional methods appear relevant to classifying psychotic disorders; however, there is no clear consensus on the most appropriate categories and dimensions or on the best approach for constructing nosologic criteria that integrate these 2 methods. This review examines the evidence on specific dimensions and categories that would best characterize psychoses. METHOD: Entries in the MEDLINE database between 1980 and 2011 were searched for studies of the dimensional and/or categorical structure of psychosis. Studies were included if samples represented a spectru

The Intersection Between Childhood Trauma, the COVID-19 Pandemic, and Trauma-related and Psychotic Symptoms in People With Psychotic Disorders.

INTRODUCTION: People with psychotic disorders may be disproportionately affected by the traumatic effects of the COVID-19 pandemic. Childhood trauma, which also increases vulnerability to subsequent stressors, is common in individuals with psychosis. In this study, we investigated the intersection of the pandemic, childhood trauma, and psychotic and trauma-related symptoms in individuals with psychotic disorders. METHODS: We administered a cross-sectional survey to 151 participants [47 schizophrenia (SZ), 53 psychotic bipolar disorder (BP)], 51 healthy control (HC)] during the COVID-19 pandem
