Estimation of median effective doses and 95% effective doses of oliceridine combined with propofol for day-case hysteroscopy under total intravenous anesthesia with preserved spontaneous breathing: an up-and-down sequential allocation trial.

BACKGROUND: Oliceridine is a novel developed G protein-biased µ-opioid receptor agonist, offering a significant reduction in the risk of opioid-related respiratory depression and a lower incidence of gastrointestinal adverse effects. This sequential dose-finding study was conducted to determine the median effective doses (ED₅₀) and 95% effective doses (ED₉₅) of oliceridine coadministered with propofol during day-case hysteroscopy under total intravenous anesthesia (TIVA) with preserved spontaneous breathing (PSB), thereby supporting the optimization of clinical anesthesia protocols. METHODS: I

Comparison of subanaesthetic ketamine and lidocaine as adjuvants to propofol-based total intravenous anesthesia on propofol requirement during emergency decompressive craniectomy for traumatic brain injury: a prospective, double-blind, three-arm randomized controlled trial.

BACKGROUND: Prevention of secondary brain injury remains a key objective during emergency decompressive craniectomy for traumatic brain injury (TBI). Propofol-based total intravenous anesthesia (TIVA) is widely used in TBI because of its favorable cerebral physiological effects but may cause dose-dependent hypotension and compromise cerebral perfusion. Ketamine and lidocaine have been proposed as adjuncts that may reduce anesthetic requirements while preserving intracranial homeostasis. This study compared the effects of ketamine and lidocaine versus placebo on propofol consumption and periope

The effect of isoflurane versus propofol-based total intravenous anesthesia on hemodynamic stability, intraoperative blood loss, surgical field quality, recovery characteristics, and complications in patients undergoing posterior lumbar fusion surgery: a randomized clinical trial.

INTRODUCTION: General anesthesia is commonly used for spinal surgeries, achieved through inhalational methods or total intravenous anesthesia (TIVA). This study compared propofol-based TIVA with isoflurane anesthesia in patients undergoing posterior lumbar fusion surgery. Primary outcomes - intraoperative blood loss, surgical field quality, and hemodynamic stability - were selected for their direct impact on patient safety and surgical outcomes. Secondary outcomes evaluated recovery characteristics (extubation time, eye-opening, response to commands, and post-anesthesia recovery quality) and p

Assessment of feasibility of opioid-free anesthesia combined with preoperative thoracic paravertebral block and postoperative intravenous patient-controlled analgesia oxycodone with non-opioid analgesics in the perioperative anesthetic management for video-assisted thoracic surgery.

INTRODUCTION: This study, conducted between December 2015 and March 2018 at a single university hospital, explored the feasibility and safety of opioid-free anesthesia combined with preoperative thoracic paravertebral block (ThPVB) for patients undergoing elective video-assisted thoracoscopic surgery (VATS). The aim was to assess the impact of this approach on postoperative pain levels and opioid consumption. MATERIAL AND METHODS: Sixty-four patients scheduled for elective VATS were randomly assigned to either the intervention group, receiving opioid-free anesthesia with ThPVB, or the control

Comparative Study of Efficacy of Preemptive Use of Opioid-free Anesthesia Using Paracetamol, Magnesium Sulfate, Lignocaine, Dexamethasone versus Intravenous Fentanyl for Attenuating Hemodynamic Response, Perioperative Analgesia, and Postoperative Recovery Profile during Laparoscopic Cholecystectomy: A Randomized Parallel Group Trial.

BACKGROUND: Opioids such as fentanyl are widely used in general anesthesia for intraoperative analgesia; however, their use is associated with adverse effects, including respiratory depression, postoperative nausea and vomiting (PONV), ileus, and risk of opioid-related hyperalgesia. Opioid-free anesthesia (OFA) has emerged as an alternative approach utilizing nonopioid agents to provide adequate analgesia while minimizing opioid-related complications. This study aimed to compare the efficacy and safety of OFA with fentanyl-based anesthesia in patients undergoing laparoscopic cholecystectomy.

Implementation of an electroencephalogram-guided propofol anesthesia practice in a large academic pediatric hospital: A quality improvement project.

BACKGROUND: Propofol-based total intravenous anesthesia is gaining popularity in pediatric anesthesia. Electroencephalogram can be used to guide propofol dosing to the individual patient to mitigate against overdosing and adverse events. However, electroencephalogram interpretation and propofol pharmacokinetics are not sufficiently taught in training programs to confidently deploy electroencephalogram-guided total intravenous anesthesia. AIMS: We conducted a quality improvement project with the smart aim of increasing the percentage of electroencephalogram-guided total intravenous anesthesia

Perioperative considerations in adult mitochondrial disease: A case series and a review of 111 cases.

Mitochondrial disease has been uncommon conditions, still results in death during childhood in many cases. The ideal anesthetic pharmacological management strategy for adult patients with mitochondrial disease is currently unclear. In this study, we presented features of the anesthesia methods employed and the perioperative complications of patients in our institution and in previously published case reports. We report the use of general anesthesia 7 times in 6 adult patients with mitochondrial disease during 2004-2014. All cases were performed with maintained intravenous anesthesia. One case

Consistency of remifentanil concentrations in propofol-Remifentanil infusions. A laboratory-based study.

BACKGROUND: There is increasing interest in two-agent single-pump intravenous infusions for anesthesia and sedation in pediatric patients. Propofol-remifentanil is one such mixture. The poor miscibility of such admixtures when remifentanil is added in very high concentrations and when the admixtures are maintained in static conditions has been demonstrated; however, these physiochemical properties have not been examined in clinically relevant concentrations or settings. AIM: To examine if propofol-remifentanil admixtures maintain consistent remifentanil delivery when mixed in clinically relev

Depth of anesthesia by Narcotrend® and postoperative characteristics in children undergoing cardiac surgery under extracorporeal circulation: a retrospective comparison of two anesthetic regimens.

BACKGROUND: Depth of anesthesia may be insufficient in pediatric cardiac anesthesia if a total intravenous anesthetic regimen with opioids and midazolam is used during cardiopulmonary bypass. The advantages of sevoflurane-based balanced anesthesia may be (1) a more graduated regulation of the depth of anesthesia during cardiopulmonary bypass and (2) a reduction in postoperative ventilation time for children in comparison with total intravenous anesthesia. AIM: To evaluate a possibly positive effect of sevoflurane-based balanced anesthesia in children undergoing cardiac surgery we analyzed whe
