The authors address the multifactored origins of renovascular hypertension following renal transplantation and present the experience with digital subtraction angiography and percutaneous transluminal angioplasty as well as standard angiography and surgical repair in the diagnosis and treatment of transplant renal artery stenosis. The roles of the renin-angiotensin system of the native kidney and of the allograft in sustaining hypertension after transplantation are reviewed in detail.

Dural arteriovenous fistula is an acquired vascular anomaly that can cause various symptoms. Here, we report a rare case of Borden type I sigmoid sinus dural arteriovenous fistula presenting as subarachnoid hemorrhage. Bleeding occurred from a side-wall aneurysm in the lateral pontomedullary segment of the anterior inferior cerebellar artery, which was a minor pial feeder. Features on imaging modalities, including brain CT, CT angiography, MR imaging/angiography and digital subtraction angiography, are described with a literature review.

A 62-year-old woman had sudden-onset headache and posterior neck pain, and a subarachnoid hemorrhage was revealed by unenhanced CT. Both multi-detector CT angiography and digital subtraction angiography were performed and revealed a small intracanalicular aneurysm of the left anterior inferior cerebellar artery. The patient underwent successful retrosigmoid craniectomy and trapping of the aneurysm. This case shows the ability of multi-detector CT angiography to indicate bony landmarks that can alter the surgical approach.

The anatomy of the gastric veins and gastric varices is complex, and is difficult to understand even with digital subtraction angiography. In balloon-occluded retrograde transvenous obliteration (BRTO) of gastric varices, accurate assessment of the vascular anatomy is essential for successful treatment. Here, we report the case of a patient with gastric varices who was successfully treated with BRTO using three-dimensional rotational angiography.

We report a case of postoperative slippage of a titanium clip that was used for a small ruptured anterior communicating artery aneurysm. Clipping was successful with no remnant in the initial operation. Progressive slippage of the clip was observed in follow-up brain computed tomography angiography studies. Digital subtraction angiography confirmed this finding. Reoperation was performed 19 days after the initial craniotomy. The clinical course after reoperation was uneventful.

Vertebral artery tortuosity and loop formation are rare causes of cervical radiculopathy. The criterion standard for the detection of vertebral artery loop formation is digital subtraction angiography. Computed tomography (CT) and magnetic resonance imaging provide good accuracy in the evaluation of vertebral arteries. A case of vertebral artery loop formation is reported. The diagnosis was confirmed by 16-row multisection CT angiography findings of vertebral artery loop formation.

We report a case of secondary antiphospholipid syndrome (APS) occurring in a progressive systemic sclerosis (PSS) patient who took herbal medication. Clinical findings compatible with APS included positive IgM anticardiolipin antibody (ACL), thrombocytopenia, and obstruction of the left radial artery on digital subtraction angiography (DSA). Clinical findings compatible with PSS included sclerodactyly and digital ulcers, Raynaud's phenomenon, pulmonary fibrosis and pulmonary hypertension, proteinuria and renal mesangial reaction, and myocarditis.

The purpose of this study was to evaluate the utility of 16-slice multidetector computed tomographic (MDCT) angiography for identifying anatomic features of aberrant subclavian arteries. Seventeen patients with aberrant subclavian arteries were assessed by MDCT angiography. The aortic arch position, the presence of a Kommerell's diverticulum, aneurysm, vascular compression of trachea and oesophagus and associated cardiovascular abnormalities were evaluated. MDCT findings were confirmed by surgery in eight patients but in the other nine patients no further evaluation or management was warranted
