[Acute ankle sprain: which immobilization?].

Acute ankle sprain is the most frequent orthopaedic injury. Up to 4% of patients admitted to the emergency room will present with an acute ankle sprain. The lateral ligaments are involved at various degrees (anterior talo-fibular and calcaneo-fibular ligaments). Grade I acute ankle sprains are well treated with a compression bandage or an Aircast brace. Grade II and III acute ankle sprains are best treated with a below-knee cast. The Aircast brace is the next best treatment for such lesions (grades II and III) and will provide satisfactory support, but has been shown to be not as effective as …

Valgus ankle deformity and arthritis.

This article describes several conditions that may result in valgus ankle arthritis. The emphasis is on correction of pathology or deformity to prevent valgus arthritis from developing. The surgical techniques available for the treatment of this form of ankle arthritis, once it develops, are described also.

Acute ankle sprain: an update.

Acute ankle injury, a common musculoskeletal injury, can cause ankle sprains. Some evidence suggests that previous injuries or limited joint flexibility may contribute to ankle sprains. The initial assessment of an acute ankle injury should include questions about the timing and mechanism of the injury. The Ottawa Ankle and Foot Rules provide clinical guidelines for excluding a fracture in adults and children and determining if radiography is indicated at the time of injury. Reexamination three to five days after injury, when pain and swelling have improved, may help with the diagnosis. Therap…

Revision of failed ankle arthrodeses.

OBJECTIVE: Ankle arthrodesis with the foot at 90 degrees with minimal as possible leg shortening. Regain of a pain-free use of the limb. INDICATIONS: Failure of arthrodesis, septic or aseptic in origin, accompanied by pain interfering with weight bearing. CONTRAINDICATIONS: General surgical or anesthesiologic risks. Acute reflex sympathetic dystrophy. Significant arterial circulatory disturbances or extensive neurologic deficits. Extensive bone or soft-tissue defects after previous surgeries. SURGICAL TECHNIQUE: Approach using existing scars. Resection of nonunion making allowance for the p…

Ankle problems masquerading as sprains.

The most common ankle injury is a ligamentous sprain, usually involving the lateral side. This is usually treated very successfully with conservative management. Those patients who have continued complaints despite such management should be suspected of having something more than a simple sprain. The diagnostic entities discussed herein should always be part of the differential diagnosis of a sprained ankle, so that when one of these injuries is initially missed, the examining physician will be able to re-examine and eventually diagnose and treat these injuries properly. They should be managed…

Ankle dorsiflexion in adolescent athletes.

Ankle equinus has been proposed to be associated with lower-extremity pathology. Physiologically normal measurements have been quantified in various populations. Forty high-school athletes (16 girls and 24 boys) without a history of ankle injury had ankle dorsiflexion measured with the knee extended and flexed by an experienced evaluator using a goniometer with the subjects supine. The group mean +/- SD dorsiflexion for the right ankle was 0.35 degrees +/- 2.2 degrees with the knee extended and 4.88 degrees +/- 3.23 degrees with the knee flexed. The values for the left ankle were -0.6 degrees …

Peroneal tenosynovitis following ankle sprains.

Peroneal tenosynovitis has a variety of causes which include inversion injury of the ankle. It is rarely reported and frequently misdiagnosed. We report the operative findings and outcome of surgical decompression in 19 patients who clinically presented following inversion injury with features suggestive of peroneal tenosynovitis. A review of the literature of this pathology is presented.

Primary Treatment of Ankle Trauma.

In brief Decreasing inflammation and pain associated with an ankle injury is the first goal of treatment. Then success depends on accurately assessing the extent of the injury. With sprains and strains, options include bracing, taping, and range-of-motion exercises. For fractures, the key is knowing when to treat conservatively and when to refer. With any ankle injury, aggressive rehabilitation often produces good results.

Immediate Diagnosis of Ankle Trauma.

In brief Traumatic Injuries to the ankle vary greatly in severity, from minor contusions to displaced intra-articular fractures. A systematic approach to their evaluation can lead to more accurate diagnoses and help simplify treatment options. Primary care physicians can successfully manage most sports-related ankle trauma, but knowing when to obtain orthopedic consultation is equally important.

[Laser therapy of ankle sprain].

The effect of low-power laser therapy on acute ankle sprains was evaluated in a double-blind randomised clinical study consisting of 40 patients from the casualty ward. All patients received treatment until their ankle joint was painless. No statistically significant differences regarding discolouring, pain, oedema, and use of analgetics were observed between patients treated with laser and placebo. The patients treated with active laser had a significantly longer sick leave.

Ipsilateral tibia and ankle fractures.

This study reports on 18 cases of diaphyseal fractures of the tibia associated with ipsilateral but not contiguous ankle fractures. Thirteen of these complex fractures resulted from low-energy torsional forces, while five were caused by high-energy impact. Definitive treatment consisted solely of cast immobilization in four; 14 had operative stabilization of each fracture component. Of these 14, four patients were treated initially in long leg casts and one in a plaster splint, but each required surgical intervention to control fracture alignment. The average follow-up time was 21.2 months. In…

Current concepts in ankle arthroscopy.

Ankle arthroscopy has rapidly become an important diagnostic and therapeutic procedure. Currently, indications for operative arthroscopy include transchondral talar dome fractures, acute articular fractures with hemarthrosis, posttraumatic synovitis, loose bodies, inflammatory synovitis, degenerative joint disease, and soft tissue impingement. Diagnostic arthroscopy is indicated for the patient with a chronically painful, symptomatic ankle when nonoperative treatment has failed and other measures have failed to produce a diagnosis. Three standard portals are used for routine ankle arthroscopy …

Chronic and recurrent ankle sprains.

Persistent symptoms such as pain, swelling, or giving way after lateral ligament injuries of the ankle are not uncommon. The treating orthopedist must first decide whether the symptoms are related to the ankle instability. Intra-articular or extra-articular injury may occur concomitantly with either a stable or unstable ankle, and careful diagnostic evaluation must be undertaken. Arthroscopy of the unstable ankle in which there is significant pain without demonstrable intra-articular injury by bone scans, CT scans, and MRI studies is indicated, as intra-articular lesions do occur that may resu…

Ball-and-socket ankle joint.

The ball-and-socket ankle joint is a malformation of the ankle in which the articular surface of the talus is hemispherical in both the anteroposterior and lateral projections and has a congruent, concave tibial articular surface. Fourteen patients with this condition were identified retrospectively. Thirteen patients were thought to have the congenital type of ball-and-socket ankle joint which in many was associated with tarsal coalition, short limb, and ray fusion and deletion anomalies. One case of the acquired type, demonstrating less geometric rounding of the talar margins, was seen in a …

Ankle arthrodesis. Problems and pitfalls.

Compression ankle arthrodesis remains a widely accepted surgical procedure. However, uniform efficacious results are not always obtainable. Seventeen patients treated by compression ankle arthrodesis showed 11 fusions, or a 65% union rate, with an average immobilization period of five months. The nonunion group of six patients (35%), with an average of ten months of immobilization, included two patients who were treated by below-knee amputations. The failures demonstrated basic violations of surgical, roentgenographic, and biomechanical tenets. In particular, the commonly used external fixatio…
