The authors presented an outline of modern knowledge about anthrax. Some aspects of the use of Bacillus anthracis as a biological terrorist agent are also described.

HISTORY: Anthrax has been known since ancient times. Besides some references in the Old Testament, there is evidence of plagues in ancient Egypt, as well as descriptions of the disease by the Roman poet Virgil.
ETIOLOGY: Anthrax is caused by Bacillus anthracis, unmovable, aerobic, gram-positive rods. It forms spores, which can survive for years in the environment.
PATHOGENESIS: Capsular polypeptide and anthrax toxin are the principal virulence factors of Bacillus anthracis. Anthrax toxin consists of three proteins called protective antigen, edema factor, and lethal factor. It is thought that…

A human case of anthrax was identified through surveillance of knackery workers who had been exposed to infected cattle. The outbreak in cattle has affected 38 herds in the Stanhope/Tatura area of central northern Victoria. The human case, a 39 year old male, was treated in hospital and is recovering. Surveillance of other knackery workers has now been completed, and no other cases were found. Public health measures are in place to prevent further human cases.

The paper reports group cases of the disease running as anthrax. The disease was not identified etiologically. The authors hold that it is important to make differential diagnosis of anthrax with necrobacillosis, pasteurellosis and contact pustular viral dermatitis. Laboratory and clinical diagnostic techniques are specified.

In April and May 1979, an unusual anthrax epidemic occurred in Sverdlovsk, Union of Soviet Socialist Republics. Soviet officials attributed it to consumption of contaminated meat. U.S. agencies attributed it to inhalation of spores accidentally released at a military microbiology facility in the city. Epidemiological data show that most victims worked or lived in a narrow zone extending from the military facility to the southern city limit. Farther south, livestock died of anthrax along the zone's extended axis. The zone paralleled the northerly wind that prevailed shortly before the outbreak.…

Gram-positive bacilli isolated during epidemiological investigations which, on the basis of conventional tests, resemble Bacillus anthracis but which fail to produce the capsule or to induce anthrax in test animals have long been dismissed in clinical and veterinary laboratories as B. cereus or simply as unidentified Bacillus spp. and thereupon discarded as inconsequential. In this study, the application of newly available DNA probe, polymerase chain reaction and specific toxin antigen detection technology has revealed that a proportion of such strains are B. anthracis which lack the plasmid c…

Twenty-five cases of human anthrax, occurring after contact with a Jercy cow infected with B. anthracis and consumption of its improperly cooked meat after death, were reported. Cutaneous anthrax was the predominant variety in 18 persons, 7 had intestinal anthrax, out of them 2 children died, 5 adults survived and one among them developed associated cutaneous anthrax also. All the persons with cutaneous anthrax recovered with treatment.
