We present a technique for the surgical correction of aphakia that allows intrascleral fixation of a posterior chamber intraocular lens (IOL) without sutures. The technique is useful in situations in which one haptic has to be fixated and capsule support is adequate for fixation of the second haptic. The haptic is externalized with a 25-gauge needle; no surgical instrumentation other than that needed for conventional cataract surgery is used. The technique is particularly appropriate for 3-piece IOLs with flexible haptics.

An infant with bilateral Peters' Anomaly, with corneal opacification in the right eye and an opacification with an axial defect in the left also had an associated hydrocephalus and cardiac anomalies that proved fatal; histopathology of the right eye showed retinal neovascularization in addition to findings consistent with a diagnosis of Peters' Anomaly; in the left eye, pathologic findings were suggestive of a corneal coloboma associated with aphakia.

Forty aphakic eyes, with secondary pupillary membranes, underwent pars plana membranectomy and YAG laser discission randomly. Visual improvement was similar in both the groups. IOP remained low for a week in pars-plana membranectomy while it transiently increased following YAG laser discission Complications like anterior chamber reaction, corneal edema and CME were more after pars plana membranectomy than in YAG laser discission. In membranes thicker than 1.2 mm, only pars plana membranectomy is recommended.

The purpose of this study was to create an intraocular lens (IOL) reliably fixed in the capsular sac and convenient for implantation through a tunnel incision. Our design consists from a biconcave lens 5 mm in diameter and 2 vis-a-vis C-shaped open haptic elements at an angle of 110 degrees to the optic component. The total diameter of IOL is 12 mm. The lens is made from rigid material (oligocarbonate methacrylate). IOLs of this design were implanted in 164 eyes. No complications were recorded.
