Management of patients with obstructive sleep apnea.

Behavioral measures, such as weight loss, altered sleeping position, and avoidance of alcohol and sedatives, can benefit some patients with mild obstructive sleep apnea (OSA). The mainstay of therapy for OSA is administration of continuous positive airway pressure (CPAP). Oral appliances may benefit patients who are unable or unwilling to use CPAP or other forms of PAP therapy. The wakefulness-promoting agent modafinil is recommended for patients who experience residual sleepiness despite optimal CPAP therapy, provided CPAP compliance is closely monitored.

Differential diagnosis of apneas in preterm infants.

Clinically relevant apneas, which are common in preterm infants, may adversely affect later neuropsychological condition in this group of patients. Pharmacotherapy to stimulate respiratory functions may be unsuccessful. Polygraphic recording may help in the differential diagnosis of these clinically relevant events. Twenty-nine preterm neonates born before 36 weeks of gestational age were examined using polygraphic recording (respiration--two channels, perioral electromyography, oxygen saturation, heart rate, electroencephalography, electrocardiography, electrooculography). The examination was…

Supraglottoplasty for laryngomalacia with obstructive sleep apnea.

OBJECTIVES/HYPOTHESIS: To determine if supraglottoplasty (SGP) is effective in reversing abnormal respiratory parameters in children with laryngomalacia and obstructive sleep apnea (OSA). STUDY DESIGN: Retrospective case series at a tertiary referral children's hospital. METHODS: Ten patients with laryngomalacia and OSA as documented by polysomnography underwent SGP between 2005 and 2007. Data collected included age, findings on flexible and rigid endoscopy, type of procedure performed, and postoperative course. The postoperative polysomnographies were reviewed to identify changes in obstruc…

Sleep apnea in the elderly: a specific entity?

Sleep apnea is highly prevalent in subjects after age 60, and affects older men and women similarly. Central apneas are often observed in addition to obstructive and mixed events. Pathogenesis of obstructive and central events during sleep in the elderly can be attributed to an amplification of well-established causes of sleep-disordered breathing (SDB) in younger adults. As in middle-aged adults, sleep-related complaints, cardiovascular diseases, depression and traffic accidents should prompt an evaluation by a sleep specialist. However, secondary enuresis and nocturia, cognitive impairment, …

[Is pharmacologic treatment of sleep apnea efficient?].

High frequency of sleep disturbed breathing in adults is increasingly recognized. Obstructive sleep apnea, defined as the recurring obstruction of the upper airway is documented in 24% of adult males and 9% of females. The highest incidence is in the sixth decade. Snoring accompany 24% of middle aged man and 14% of females of the same age. Sleep apnea with the central origin, defined as the transient and recurrent cessation of breathing during sleep, is less frequent than obstructive sleep apnea. It can be identified in the wide spectrum of patients with medical, neurologic and neuromuscular d…

Sleepiness and relationships in obstructive sleep apnea.

This is a qualitative analysis of data from a multisite study of 156 participants with Obstructive Sleep Apnea (OSA). Participants completed a battery of tests, including the Functional Outcomes of Sleep Questionnaire (FOSQ) that contains an item assessing the impact of OSA on relationships. Approximately one third of participants wrote comments; they were predominately male, mean age 44.7, with severe OSA. Interpersonal themes expressed included work and marital problems and social life restriction. Intrapersonal themes included embarrassment and poor mood. This report adds specific details t…

Treatment of sleep apnea in heart failure.

Obstructive and central sleep apnea are common in heart failure, and may participate in its progression by exposing the heart to intermittent hypoxia, increased preload and afterload, sympathetic activation, and vascular endothelial dysfunction. Treatment of sleep apnea in patients with heart failure may reverse these detrimental effects, in addition to alleviating symptoms of sleep apnea. In patients with heart failure and obstructive sleep apnea, short-term randomized trials have demonstrated that continuous positive airway pressure (CPAP) improves cardiac function, and lowers sympathetic ac…

[Sleep apnea-hypopnea syndrome and the heart].

Cardiovascular and cerebrovascular diseases are the most common diseases in industrialized societies. The main objectives of this article were to summarize the physiological effects of sleep apnea on the circulatory system and to review how treatment of this condition influences cardiovascular disease. Acute sleep apnea has a number of hemodynamic consequences, such as pulmonary and systemic hypertension, increased ventricular afterload and reduced cardiac output, all of which result from sympathetic stimulation, arousal, alterations in intrathoracic pressure, hypoxia and hypercapnia. When chr…

Effect of sleep apnea on cognition and mood.

Obstructive sleep apnea syndrome (OSAS) is a common disorder in adults and children, which is characterized by repetitive transient reversible upper airway obstructions during sleep. Due to disrupted sleep architecture and intermittent hypoxemia, OSAS leads to impaired daytime functioning in various neuropsychological and affective domains. The most common abnormalities are executive dysfunction, impaired vigilance, depression, and possibly anxiety and, in children, hyperactivity. Optimal treatment of OSAS with continuous positive airway pressure may reverse the cognitive and affective dysfunc…

Obesity, sleep apnea, the airway and anesthesia.

PURPOSE OF REVIEW: Recent articles indicate that at the present time disastrous respiratory outcomes during the perioperative management of patients with obstructive sleep apnea are a major problem for the anesthesia community. RECENT FINDINGS: A recent review of the literature indicates that disastrous respiratory outcomes are due to intubation failure, respiratory obstruction soon after extubation, and respiratory arrest after narcotic and sedative medication. In obese patients, the basic problem is deposition of adipose tissue in the pharynx. The excessive adipose tissue in the pharynx cau…

Swallowing apnea as a function of airway closure.

The purpose of this research was to ascertain whether the existence of swallowing apnea (SA) was the sole result of glottic closure or the result of its own neural mechanism. Forty individuals served as participants. Groups 1 and 2 consisted of 10 individuals with laryngectomy less than one year and 10 individuals greater than one year, respectively. Groups 3 and 4 comprised 20 individuals who were age- and gender-matched to Groups 1 and 2. SA duration (SAD) was acquired during saliva swallows and 10-, 15-, 20-, and 25-ml water boluses. A repeated measures ANOVA was performed to determine mean…

Imaging for the snoring and sleep apnea patient.

Upper airway imaging techniques have significantly advanced the understanding of the pathogenesis of obstructive sleep apnea and the biomechanical mechanisms by which therapeutic interventions for this disorder exert their effects. Both static and dynamic imaging studies have been used to examine the structure and function of the upper airway during wakefulness and sleep. These studies have highlighted the importance of the lateral pharyngeal walls in addition to the tongue and soft palate in modulating changes in upper airway caliber. Upper airway imaging has also been used to understand the …

Current options in the management of apnea of prematurity.

Apnea of prematurity (AOP) is a common problem that affects premature infants and, to a lesser degree, term infants. Apnea of prematurity appears to be due to immaturity of the infant's neurologic and respiratory systems. Apnea of prematurity is a diagnosis of exclusion that can be made only when other possible infectious, cardiologic, physiologic, and metabolic causes of apnea have been ruled out. The fundamental principles for managing apnea of prematurity include monitoring the infant closely while instituting supportive care measures such as tactile stimulation, continuous positive airway …

The behavioral morbidity of obstructive sleep apnea.

The behavioral morbidity associated with obstructive sleep apnea (OSA) includes symptoms of excessive daytime sleepiness (EDS), neurocognitive deficits, psychological problems, and possibly an increased chance of accidents. EDS is among the most frequently reported symptoms in patients diagnosed with OSA. The available data suggest that the primary cause of EDS is sleep fragmentation. The subjective measures of sleepiness include the sleep wake activity inventory and the epworth sleepiness scale. Sleepiness can also be evaluated objectively in the sleep laboratory using the multiple sleep late…

Identifying depression as a symptom of sleep apnea.

Sleep apnea syndrome is a common, life-threatening disorder, causing significant physical, emotional, and social problems. The symptoms of obstructive sleep apnea syndrome can lead to an erroneous diagnosis of dysthymia. For patients with sleep apnea syndrome, standard antidepressant treatments may exacerbate the condition. DSM-IV's diagnostic model needs only a small change to allow consideration of sleep apnea syndrome as a cause of a mood disorder.

[Physical training of patients with sleep apnea].

PURPOSE: It is a common question of sleep apnoea patients in the sleep lab whether they stand a chance to decrease the symptoms and severity of their disease by physical exercise. As far as we know, there is no data about this specific question until now, even though this has been subject to speculation. A few studies, however, report on an improvement of the respiratory drive (and chemoreceptor sensitivity) after physical exercise in athletes. The aim of this study was to prove whether physical exercise in sleep apnoea patients could improve the symptoms of their disease in an open trial. ME
