A rare case of cervical spinal cord compression in diffuse idiopathic skeletal hyperostosis (DISH or Forestier's Disease) caused by a craniocervical mass of soft-tissue is reported. The objective is to describe an uncommon mechanism of spinal cord compression in DISH. Three weeks after a cardiac infarction a 69-year-old man slowly developed spastic tetraparesis. Magnetic resonance tomography showed a craniocervical tumor compressing the spinal cord and a massive DISH of the cervical spine. An extended mass of yellowish amorphous material was removed from between the dura, the posterior odontoi

The Condylus tertius is defined as a small bony hunch on the anterior surface of the clivus. Its presence means an enormous functional impairment of the upper head joint, looking at the 3-point-contact between the skull base and the upper cervical vertebrae. In 10 of the 2000 forensic examined bodies, analyses of neck vertebra + skull base revealed this feature. The origin of these findings is discussed, as stated in the literature of embryology, to be a suboccipital hypochordal plate. So in one of the cases the condylus was found at the hypochordal plate itself whereas the so-called socket wa

BACKGROUND: Airway management is crucial in clinical anesthesia. Many complications associated with airway management result from unexpected difficult airway, but predicting a difficult airway is a major challenge. We investigated the efficacy of a new combined model including radiological indicators to predict difficult airway in patients undergoing surgery for cervical spondylosis, a population with a high incidence of difficult airway.
METHODS: We randomly enrolled 303 patients scheduled for elective surgery for cervical spondylosis at Peking University Third Hospital between August 2012 a

BACKGROUND: Traumatic spondylolisthesis of the axis, also known as a hangman's fracture, is a well-described entity but is relatively uncommon. There are only two published reports of these fractures occurring in pediatric patients with osteogenesis imperfecta, a disorder that predisposes patients to long bone fractures.
CASE DESCRIPTION: We present a unique case of an adult woman with osteogenesis imperfecta who sustained an Effendi type II hangman's fracture.
CONCLUSION: Effendi type II fractures can be conservatively managed via rigid or nonrigid cervical orthosis.

Vertebroplasty of the axis is always a challenging procedure. We report the case of a young, HIV-positive patient suffering from an osteolytic metastasis of the axis. An open dorsal vertebroplasty was performed. A leakage of the cement formed a new cortical bone of the massa lateralis of C2, and stabilized the C1-C2 articulation by an arthrodesis-like effect. Durable pain relief and stabilization were obtained. The location of the cement, although atypical, had all desired effects of a conventional vertebroplasty. The intra-articular injection of cement into the facets for stabilization and pa

INTRODUCTION: In human, the cervical spine region is very susceptible to impact injury. The complex structures of C1 and C2 serve to transmit the weight of the cranium to the greatly similar structural cervical spine from C3 caudally. Application of sudden disruption forces will detach the junction of the cervico-cranium with the pars interarticularis of the neural arch of C2 from the lower cervical spine by fracturing, as in a "hangman's fracture". Severe falls or blows to the head from heavy objects will cause the fracture of the odontoid process of C2. Many biomechanical studies were conduc

Odontoid dysplasia in the absence or presence of atlanto-axial instability was found in all cases. In general, this correlated well with the overall clinical severity of the condition and illustrates the importance of careful follow-up of these patients. Symptoms or signs of cervical cord compression merit further investigation. Proven compromise of the upper cervical cord is an indication for posterior fusion of the upper cervical spine.

A simultaneous odontoid decompression and bilateral posterior atlanto-axial facetal distraction, C1-2 joint spacer/bone graft placement and stabilization may be performed utilizing the 'posterior-only' approach. This procedure may be performed utilizing a single posterior midline incision, a bilateral posterior approach to the C1-2 facet joints and a bilateral posterolateral approach to the odontoid process and C2 body. It may be carried out in situations where a C1-2 non-reduction/partial reduction using a 'posterior alone' procedure is anticipated due to the complex bony/soft tissue configur

This first description of percutaneous vertebroplasty (PVP) of the second cervical vertebra (C2) using an ascending oblique transdiscal approach is a case report of a 63-year-old woman who complained of neck pain that was resistant to painkillers and the result of an osteolytic metastatic lesion of C2. PVP was performed using an ascending oblique transdiscal approach, a percutaneous technique that had never been described before. It has since been used in more than 15 patients, resulting in pain relief and stabilization of C2 with good distribution of cement across the vertebral body and dens.

Patients with Klippel-Feil syndrome (KFS) have congenital fusions of at least 1 cervical motion segment, and often present with compensatory hypermobility or symptomatic stenosis of the cranio-vertebral junction which requires occipitocervical reconstruction and fusion. One subgroup of KFS patients in which this is particularly common is those with isolated C2-3 congenital fusion (C2-3 CF). The anatomic suitability for C2 pedicle and laminar screw placement had been analyzed in the general adult population, and guidelines for their techniques had been established. However, the feasibility and

The axis ring is a very important sign in the detection of low odontoid fractures. It can be the only radiological sign of a type III (low) odontoid fracture in a polytraumatised patient. As these are unstable fractures, their detection is important, even before a spiral CT is performed. Tracheal tube placement in patients with type III odontoid fractures should be performed very carefully to prevent displacement of the fracture. This sign, which has been published more than once in the radiological literature, seems to be not so well known by the physicians nor by a lot of radiologists. We ca

The authors report 9 cases of partial or total lysis of the odontoid peg in rheumatoid arthritis. Retrospective study of 100 cases of rheumatoid arthritis and the data from the literature demonstrate the rarity of these lesions, the last stage of the odontoid erosions which are much commoner. The therapeutic approach is usually nuero-surgical.

Tuberculous spondylodiscitis usually affects the dorso-lumbar spine, and its cervical location is a rare condition that can mimic other diseases and consequently cause treatment delays. We report a case of tuberculous spondylodiscitis of the lower cervical spine discovered under unusual circumstances in a patient with severe polytrauma involving a cranio-cerebral trauma, a non-displaced fracture of the two laminæ and the spinous process of the C6 vertebrae as well as fibular and tibial shaft fractures. The patient underwent static tibial nailing, and a collar with occipital and chin supports w

Dysphagia involving the occipital bone is a critical complication after posterior fusion surgery. However, to our knowledge, dysphagia after posterior fixation of the cervical spine C1-2 has not been reported. Herein, we report a patient with dysphagia after posterior fusion of the cervical spine C1-2. The patient was a man in his 80s. He was referred to our department with a chief complaint of dexterity difficulty. After a thorough examination, we diagnosed subluxation of the atlantoaxial vertebrae and performed C1-2 posterior fusion surgery. Dysphagia appeared the day after surgery, which wa

BACKGROUND: From a biomechanical point of view, pedicle screws (PS) are better than other kinds of screws for implantation in the seventh cervical vertebra (C7). However, the application of PS is limited because of the high risk of severe complications. It is essential to define the optimal entry point and trajectory. The aim of this study was to comprehensively analyze the starting point and trajectory for C7 PS insertion using three dimensional (3D) models.
METHODS: Overall, 60 subjects aged 18 to 67 years old were included. All CT images were used to construct 3D computer models of the C7
