Progressive dilation of the ascending aorta in children with isolated bicuspid aortic valve.

Although patients with bicuspid aortic valves (BAVs) are predisposed to ascending aortic (AA) dilation, stenosis, and dissection, the development of aortic disease in children with BAVs is poorly described. The purposes of this study were to determine the rate of change of AA diameter in children with BAVs and to identify risk factors for the development of aortic dilation. The echocardiograms of 276 children aged<19 years (mean 8.5+/-5.3) with isolated BAVs were reviewed. Aortic measurements were normalized to z scores on the basis of body surface area. In a subset of 112 patients with serial…

Morphology and function of the bicuspid aortic valve with and without coarctation of the aorta in the young.

This study sought to determine the morphology and function of bicuspid aortic valves (BAVs) with and without coarctation of the aorta (CoA) in a young population. The transthoracic echocardiograms of 117 patients with BAVs and 62 patients with CoA were retrospectively reviewed and compared with normal transthoracic echocardiographic results. In each patient, the area subtended by the aortic cusps and valve and the diameters of the aortic root at different levels were measured, and additionally in each BAV, the type of cusp fusion and the presence and degree of aortic stenosis and/or regurgitat…

A patient with bicuspid aorta and intercoronary continuity: a rare and variant of coronary circulation.

Bidirectional flow in patients with normal coronary arteries is an indicator of intercoronary continuity, a rare variant of coronary circulation, distinct from collaterals. The case of an 18 year old Turkish male with bicuspid aorta and intercoronary artery is reported and different aspects of this interesting entity are emphasized.

[Surgical treatment of traumatic rupture of the bicuspid aortic valve; report of a case].

A 27-year-old man was injured during a motocross game. He was suffered from dyspnea, orthopnea, and hemoptysis. The to-and-fro murmur was noticed 3 days after the accident and then the patient was admitted to our hospital. Echocardiography revealed severe aortic regurgitation. Computerized tomography also showed severe pulmonary contusions. Seventeen days after the accident the aortic valve replacement was performed. The aortic valve was anterior-posterior type bicuspid valve (fusion of right coronary cusp and light coronary cusp) and the tear was detected in the anterior cusp. The postoperati…

Reconstruction of the pulmonary valve and outflow tract with bicuspid prosthesis in tetralogy of Fallot.

BACKGROUND: Right ventricular outflow tract and pulmonary valve implant in patients with tetralogy of Fallot may be required to avoid late postoperative right ventricular impairment. The use of porcine bicuspid prosthesis might be a technical alternative, although comprehensive studies on the long-term use of these prostheses are not available. METHODS: Sixty-three patients (aged 5 months to 34 years; mean, 6 years) with tetralogy of Fallot and pulmonary hypoplasia underwent surgical repair and enlargement of the right ventricular outflow using preserved porcine pulmonary bicuspid prostheses.…

Superiority of two-dimensional echocardiography in the diagnosis of congenitally bicuspid aortic valve.

This study compares the role of M-mode and two-dimensional echocardiography in the evaluation of the nonstenotic, congenitally bicuspid aortic valve. Ten asymptomatic healthy subjects (age 20-35 years) with a systolic ejection click and short systolic murmur were evaluated. M-mode echocardiograms were entirely normal. The aortic root, diastolic closure line, eccentricity index of the aortic valve, and left ventricular size and function were normal in every subject. The two-dimensional parasternal long axis view of the aortic root was also normal in all patients with bicuspid aortic valves; the…

High diagnostic yield of family echocardiographic screening of children with bicuspid aortic valve.

AIMS: Bicuspid aortic valve (BAV) is present in 0.8% of newborns, with associated aortopathy in 33%. While the prevalence of BAV and associated aortopathy in first-degree relatives of symptomatic BAV patients ranges from 5 to 10% and 5 to 32%, data from unselected probands are limited. Uniform family screening guidelines are lacking. We aimed to determine the prevalence of BAV and aortopathy in first-degree relatives of children diagnosed neonatally with BAV. METHODS AND RESULTS: In this case-control family study based on the Copenhagen Baby Heart Study (n > 25 000 newborns enrolled 2016-18),…

Valvulo-Arterial Impedance in Patients with Severe Aortic Stenosis and Bicuspid Aortic Valve.

BACKGROUND: Risk stratification in patients with bicuspid aortic valve (BAV) and severe aortic stenosis (AS) remains challenging. Valvulo-arterial impedance (Zva), an integrated marker of global left ventricular (LV) afterload, has shown prognostic value in tricuspid AS; however, data in BAV are limited. This study aimed to evaluate the association of Zva with LV remodeling, symptoms, and all-cause death in patients with BAV and severe AS. METHODS: In this retrospective, two-center cohort study, 147 patients with severe AS and BAV were included. Zva was calculated at the time of the first ech…

A pathobiologic classification of aortic stenosis: Moving beyond the degenerative-bicuspid-rheumatic paradigm.

BackgroundAortic stenosis is traditionally categorised as degenerative calcific, congenital bicuspid, or rheumatic. Contemporary molecular biology, multimodality imaging, and transcatheter valve therapy demonstrate that aortic stenosis represents a spectrum of mechanistically distinct diseases converging upon a shared haemodynamic endpoint. A four-category pathobiologic classification is proposed linking disease mechanism to operative strategy and procedural risk stratification.MethodsA narrative mechanistic review was performed using contemporary international guidelines, foundational biologi…

Bicuspid Aortic Valve Leaflet Modification Before Transcatheter Aortic Valve Replacement: A New Paradigm.

BACKGROUND: Transcatheter aortic valve replacement (TAVR) deployment in bicuspid aortic valve (BAV) can result in deformed transcatheter heart valves (THVs) due to incomplete, irregular, and eccentric stent expansion. Adjunctive pre-TAVR leaflet modification (LM)/splitting techniques may potentially reduce the circumferential leaflet stress exerted by the BAV leaflets on the THV, reduce risk of immediate complications, and improve THV expansion. OBJECTIVES: The aim was to introduce a new paradigm of pre-TAVR BAV LM and a proof-of-concept for THV geometry improvement. METHODS: Computed tomogr…

Transcatheter aortic valve intervention in mixed Sievers type 1 bicuspid aortic valve disease.

INTRODUCTION AND OBJECTIVES: In patients with tricuspid aortic valve stenosis, the coexistence of aortic regurgitation (AR) at baseline has been associated with improved outcomes following transcatheter aortic valve implantation (TAVI). However, its prognostic impact in patients with bicuspid aortic valve (BAV) stenosis remains unknown. METHODS: We enrolled consecutive patients with severe Sievers type 1 BAV stenosis undergoing TAVI at 24 international centers. Patients were stratified into 2 groups according to baseline AR severity: pure BAV stenosis (ie, no or mild AR) and mixed BAV disease…

Epigenetic Biomarkers in Thoracic Aortic Aneurysm, Dissection, and Bicuspid Aortopathy: A Comprehensive Review.

Thoracic aortic disease (TAD) encompasses a spectrum of life-threatening conditions, including thoracic aortic aneurysm (TAA), acute type A aortic dissection (ATAAD), and bicuspid aortic valve (BAV)-associated aortopathy. While genetic mutations are well-documented contributors, emerging evidence highlights epigenetic mechanisms as critical regulators of TAD pathogenesis. This comprehensive review explores the role of epigenetic modifications-DNA methylation, histone modifications, and microRNA (miRNA) regulation-in vascular remodeling, extracellular matrix degradation, and endothelial dysfunc…

Bicuspid Aortic Valve: Old and Novel Gene Contribution to Disease Onset and Complications.

Background: Bicuspid aortic valve (BAV) is the most common congenital heart defect, and its complications (namely, dilatation of the thoracic ascending aorta) raise concerns regarding the proper timing of aortic surgery. The study aim is to unravel the genetic basis of BAV and its complications through a high-throughput sequencing (HTS) approach and segregation analysis if family members were available. Methods: Fifty-two Italian BAV patients were analyzed by HTS using the Illumina MiSeq platform. Targeted sequencing of 97 genes known to be or plausibly associated with connective tissue disord…

Aortic root replacement for bicuspid aortic valve dysfunction does not impair survival rates.

BACKGROUND: Patients with a bicuspid aortic valve carry an increased risk for developing an ascending aortic aneurysm due to intrinsic aortic wall alterations. A lower threshold for aortic surgery may therefore be considered in these patients, especially in those who require aortic valve surgery. This study aimed to compare the outcomes of an isolated aortic valve replacement with that of an aortic root replacement in bicuspid aortic valve patients with an indication for aortic valve surgery. METHODS: Patients were included in retrospect from a tertiary academic hospital. Included patients re…

Bicuspid aortic valve: The most frequent and not so benign congenital heart disease.

Bicuspid aortic valve (BAV) is the most frequent congenital heart disease, with an incidence of approximately 1%. It can be silent and associated with normal valve function. However, a series of complications, even catastrophic, may occur with time: valve incompetence, valve stenosis by dystrophic calcification, infective endocarditis, progressive dilatation of the ascending aorta, aortic dissection, sudden death. The problem of BAV is not just about the number of semilunar cusps, but also the aortic wall. Severe noninflammatory degenerative changes (elastic fiber fragmentation, smooth muscle …

Infective Endocarditis in Patients With Bicuspid Aortic Valves: Unique Clinical and Microbiologic Features.

OBJECTIVE: Patients with bicuspid aortic valves (BAV) are at increased risk of infective endocarditis (IE). Information of the clinical presentation and the microbiology of BAV-associated IE, however, is limited. Therefore, our study aimed to characterise the clinical features native valve endocarditis (NVE) in the setting of BAV and compared them to patients with prosthetic valve endocarditis (PVE) following BAV replacement. METHODS: Adult patients with BAV or history of BAV with aortic valve replacement (AVR) and a definite or possible IE diagnosis within the Mayo Clinic Enterprise (USA) fr…
