Surgery in Barrett's oesophagus.

Surgical treatment for a columnar-lined oesophagus (CLO) may be considered in terms of indications, procedures and risks. Reflux may be an indication for surgery, but does not lead to reversal of epithelium, nor does it eliminate malignant potential. Whether it reduces such potential is not certain. Anti-reflux surgery is therefore based on reflux-related criteria, and not as a treatment for a CLO. Dysplasia is a possible indication for surgery, but as progression to carcinoma may be slow, must not be considered an absolute indication in an elderly, frail patient. Malignancy is a clear indicat

The interaction of pH, bile, and Helicobacter pylori may explain duodenal ulcer.

BACKGROUND: Inhibition of Helicobacter pylori growth by bile suggests that it should be difficult for H. pylori to colonize the duodenum and … al ulcer. These data also explain the disparate results of previous investigations of the effect of bile reflux in the stomach on the presence of H. pylori.

Anatomy of the choledochopancreatic duct junction in primary sclerosing cholangitis.

Primary sclerosing cholangitis (PSC) is a rare disease for which the etiology is unknown. Because abnormalities of the choledochopancreatic duct junction (CDPDJ) have been implicated in the pathogenesis of other rare biliary problems such as choledochal cysts and biliary atresia, we reviewed the cholangiographic anatomy of the CDPDJ in our patients with PSC. From 1955 through July 1983, 46 patients with PSC were seen at the University of California at Los Angeles Medical Center. In 20 of these 46 patients anatomy of the CDPDJ could be evaluated by review of endoscopic retrograde cholangiopancr

[Comparison of esophageal acid exposure characteristics between reflux oesophagitis and non-erosive reflux diseases].

OBJECTIVES: Comparing the esophageal acid exposure characteristics between reflux oesophagitis (RE) and non-erosive reflux disease (NERD) patients to identify the NERD subgroups. METHODS: One hundred and twenty-eight patients with heartburn and acid regurgitation were evaluated. Esophageal mucosa injury was assessed by gastroscopy. The extent of esophageal acid exposure was determined by ambulatory 24 h esophageal pH monitoring. DeMeester point >/= 15 was taken as positive. RESULTS: Thirty-seven patients were found to have RE (28.9%), 91 were diagnosed as NERD. There was no difference at DeM

Modified One Anastomosis Gastric Bypass Following Sleeve Gastrectomy for Severe Reflux and Delayed Gastric Emptying: A Prospective Trial with Clinical and Physiological Outcome Measures.

BACKGROUND: Gastro-esophageal reflux (GORD) following sleeve gastrectomy (SG) is a central challenge, and precise indications for revisional surgery or the physiology have not been precisely defined. We aimed to determine whether OAGB performed for reflux post-SG (1) accelerates gastric emptying half-time, (2) reduces the frequency and severity of reflux events, and (3) improves reflux symptoms. METHODS: We undertook a prospective trial (ACTRN12616001089426). There were 22 participants who underwent measurement before and after revisional surgery with 29 optimal SG (patients with optimal outc

[Cysts of the common bile duct in children].

From 1975 to 1989 operations were carried out in the clinic by various methods on 30 children with cysts of the common bile duct. Hepaticoor choledochojejunostomy after Roux is the operation of choice in cysts of the choledochus. However, recurrent cholangitis caused by jejunobiliary reflux was encountered in some patients in the postoperative period. The accumulated clinical experience has also shown that the Roux type operation does not guarantee a successful functional result. To prevent reflux of the intestinal contents into the biliary tract after Roux's anastomosis, the authors used an a

Long-term consequences of one anastomosis gastric bypass on esogastric mucosa in a preclinical rat model.

Although bariatric surgery is proven to sustain weight loss in morbidly obese patients, long-term adverse effects have yet to be fully characterized. This study compared the long-term consequences of two common forms of bariatric surgery: one-anastomosis gastric bypass (OAGB) and Roux-en-Y Gastric Bypass (RYGB) in a preclinical rat model. We evaluated the influence of biliopancreatic limb (BPL) length, malabsorption, and bile acid (BA) reflux on esogastric mucosa. After 30 weeks of follow-up, Wistar rats operated on RYGB, OAGB with a short BPL (15 cm, OAGB-15), or a long BPL (35 cm, OAGB-35),

Long-Term Evaluation of Biliary Reflux After Experimental One-Anastomosis Gastric Bypass in Rats.

BACKGROUND: Controversy remains regarding biliary reflux after one-anastomosis gastric bypass (OAGB). The aim of this "pilot" study was to analyze biliary reflux and its potential long-term consequences on esogastric mucosae in OAGB-operated rats. METHODS: Diet-induced obese rats were subjected to OAGB (n = 10) or sham (n = 4) surgery and followed up for 16 weeks. Evolution of weight and glucose tolerance was analyzed. Bile acid concentration measurement, histological and qRT-PCR analysis were performed in the esogastric segments. RESULTS: Weight loss and glucose tolerance were improved afte

Visualization of pancreatic juice movement using unenhanced MR imaging with spin labeling: preliminary results in normal and pathophysiologic conditions.

PURPOSE: To visualize the movement of pancreatic juice noninvasively by using an unenhanced magnetic resonance (MR) imaging technique. MATERIALS AND METHODS: Institutional review board approval and informed consent were obtained for this study. A flow-out technique of time-spatial labeling inversion pulse (SLIP) technique was applied using both nonselective and selective inversion pulses to label, or tag, pancreatic juice to move out of the region. Ten normal (eight men, two women; age range, 22-61 years; mean age, 35 years) and two pathologic conditions were examined on a commercially availa

[Anomalous choledocho-pancretic ductal junction in a choledochal cyst--a case report].

Choledochal cysts are rare congenital anomalies, mostly detected in adults. Pathogenesis of these cysts seems to be in anomalous junction between pancreatic and common bile duct, above the papillary sphincter and outside of the duodenal wall. The absence of the sphincter above the junction is followed by reflux of the pancreatic juice into the bile duct leading to dilatation and fibrous changes of bile duct wall. A 38-year-old female is presented in whom a choledochal cyst was found 11 years earlier, during the operation performed for obstructive jaundice, when cystojejunostomy with Roux-en Y

p53 gene mutation and p53 protein overexpression in a patient with simultaneous double cancer of the gallbladder and bile duct associated with pancreaticobiliary maljunction.

Pancreaticobiliary maljunction (PBM) is associated with the occurrence of biliary cancer due to pancreatobiliary reflux. We present a case of simultaneous double cancer of the gallbladder and bile duct. A 77-year-old woman who had jaundice, intra- and extra-hepatic biliary ductal dilatation and a space-occupying lesion in the gallbladder and lower bile duct underwent pancreatoduodenectomy. The gallbladder cancer showed papillary carcinoma without mutation of the K-ras gene and with p53 non-sense mutation of CCA (Pro) to CA (Stop) on codon 301 in exon 8. The bile duct cancer revealed a well-dif

Pancreas divisum in pancreaticobiliary maljunction.

BACKGROUND/AIMS: Pancreaticobiliary maljunction (PBM) and pancreas divisum (PD) are congenital anomalies that develop in the embryo at an early stage. They are possibly the result of bile and pancreatic duct misarrangement. We investigated the configuration of the pancreatic duct in patients with PBM and its clinical implications. METHODOLOGY: In 84 PBM patients, the configuration of the pancreatic duct and the presence of biliary cancer were documented. Patency of Santorini's duct was determined fluoroscopically or by dye-injection endoscopic retrograde pancreatography. Bile amylase levels w

Sphincter of Oddi motor activity in patients with anomalous pancreaticobiliary junction.

Sphincter of Oddi motor activity was investigated in seven patients with an anomalous pancreaticobiliary junction and in six controls by endoscopic manometry under fluoroscopy. Characteristic phasic wave activity was observed in the sphincter segment in both groups. Amplitude of the phasic contractions was significantly higher in the disease group than in controls (100.8 +/- 14.1 mm Hg vs. 55.2 +/- 4.3 mm Hg, p less than 0.02). Although those with the anomalous pancreaticobiliary junction had a longer common channel (15-30 mm), the length of the sphincter segment with the phasic wave activity

The choledocho-pancreatico-ductal junction in infantile obstructive jaundice diseases.

A histopathological study on how the common bile duct and main pancreatic duct open into the duodenum was performed on autopsied cases of infantile obstructive jaundice diseases, and control cases. The so-called "common channel" formation type (Type III) was divided into two types; the junction of the common bile duct and the pancreatic duct was situated in the mucosal or submucosal layer of the duodenum in type IIIa and the junction below the propria muscularis of the duodenum in type IIIb. Type IIIb was identified in 17 out of 28 cases in congenital biliary atresia and in 2 cases of congenit

Barrett's Esophagus.

Barrett's esophagus develops as a result of chronic acid and bile reflux and carries an increased risk of esophageal adenocarcinoma. Because it has no specific symptoms, many patients do not receive a diagnosis or they present with symptoms of gastroesophageal reflux disease and other related risk factors or complications. Diagnosis relies on endoscopic and histopathological findings, including a visible columnar-cell-lined segment measuring at least 1 cm long that contains intestinal metaplasia with goblet cells. Ongoing surveillance focuses on early detection of malignant progression, partic…

Technical feasibility should not outweigh long-term outcomes in biliary reconstruction for children.

The choice of biliary reconstruction after excision of choledochal malformations in children should prioritize long-term physiological outcomes over operative convenience. In response to the article by Kanojia et al. on the management of aberrant bile ducts at the porta hepatis during minimally invasive surgery, we wish to highlight that while hepaticoduodenostomy is technically attractive (particularly with laparoscopic and robotic platforms) evidence suggests it is associated with increased biliary reflux and reflux-related morbidity, especially in cases with complex hilar anatomy requiring

Vaterian diverticula as a cause of acute pancreatitis.

The association of duodenal diverticula and pancreatitis is rare. Various types of such diverticula are reviewed, especially intra- and extraluminal Vaterian diverticula in which common and pancreatic duct terminate. The pathogenesis of the pancreatitis in case of interposed Vaterian diverticula is thought to be mechanical by means of the creation of a closed Vaterian pouch in which higher pressures produce reflux of bile and pancreatic enzymes. Two patients with this particular type of duodenal diverticula are presented.
