[Gallstone ileus, surgical management review].

BACKGROUND: Gallstone ileus (GI) represents a rare cause of mechanical intestinal occlusion, which is caused by the impaction of a gallstones at the gastrointestinal tract, being most frequently the terminal ileum; its etiology is due to the passage of a calculum through a biliary-enteric fistula. Due to its low incidence, diagnostic suspicion and adequate initial surgical treatment are essential for an adequate clinical evolution. OBJECTIVE: A bibliographic review on the current surgical management of GI was carried out and exemplified by the presentation a clinical case. CLINICAL CASE: 78-

Mirizzi syndrome in a Native American population.

BACKGROUND: Mirizzi syndrome is a rare disorder consisting of obstruction of the common hepatic duct resulting from gallstone impaction in the cystic duct or neck of the gallbladder. Formation of a biliobiliary fistula is common, and if unrecognized at the time of surgery, may result in major biliary ductal injury. PATIENTS AND METHODS: The severity of biliary tract disease among Native Americans led to this retrospective review of Mirizzi syndrome to determine complications and intraoperative management. A review of 623 biliary tract operations performed from October 1990 to December 1992 id…

Laparoscopic Management of Bouveret's Syndrome after Failed Endoscopic Approach.

Bouveret's syndrome is a complication of cholelithiasis that presents with gastric outlet obstruction due to an impacted gallstone in the duodenum following cholecystoduodenal fistula. This is a rare presentation of biliary-enteric fistula; therefore, there are no standardized guidelines for the management of this disease. We present a case of a patient with Bouveret's syndrome managed with laparoscopic surgery after an unsuccessful attempt of endoscopic removal.

Common operative problems in hepatobiliary surgery.

The pace of change in hepatobiliary surgery requires a sound foundation in basic surgical principles. Further reductions in morbidity and mortality rates and appropriate use of alternative therapies require careful attention to preoperative risk assessment and patient selection. To operate safely and successfully on the liver and bile ducts, the surgeon must be well versed in normal and variant hepatobiliary anatomy, understand the underlying disease and therapeutic alternatives, and known techniques of reoperative biliary surgery. Surgeons who operate on the gallbladder must be prepared to co

[Pre- and postoperative external biliary drainage in patients with obstructive jaundice caused by neoplasm of the pancreatic head].

The external biliary drainage in severe jaundice secondary to pancreatic carcinoma acts differently in pre, intra, and postoperative period. In the preoperative period the drainage enables to lower the direct bilirubinemia and the intrahepatic biliary stasis and consequently the complications linked to the impaired hepatic and renal function. During the operation the catheter acts as a guide in performing the biliodigestive anastomosis. In the postoperative period the external drainage of most of daily biliary secretion protects the biliodigestive anastomosis during the postoperative paralytic

Removal of a distal common bile duct stone through percutaneous transhepatic catheterization.

A residual distal common bile duct stone was successfully advanced into the duodenum after percutaneous transhepatic catheterization of the biliary tract. Nonsurgical retrieval of retained biliary duct stones through surgically created T-tube tracts has been shown to be a practical procedure with high reported success rates. Reoperation has been necessary in cases where no drainage tube was introduced at surgery or when stones have formed after the fistula closed. Transhepatic catheterization techniques may provide a way to manipulate residual biliary tract stones when no other access is avail

Role of endoscopic intervention in biliary complications of hepatic hydatid cyst disease.

OBJECTIVE: Biliary complications of hepatic hydatidosis are often difficult to detect and manage. The aim of this study was to present our experience on the effectiveness of endoscopic treatment modalities in cases of biliary complications of hepatic hydatid cysts. MATERIAL AND METHODS: Over the past 10 years, 15 patients diagnosed with hepatic hydatidosis and manifesting symptoms and signs indicative of biliary involvement were examined by means of endoscopic retrograde cholangiopancreatography (ERCP) in our Gastroenterology Endoscopic Unit; 7 patients had already been operated on for hepati

Sclerosing cholangitis induced by formaldehyde solution injected into the biliary tree of rats.

Sclerosing cholangitis has been reported after surgical treatment of hydatid disease of the liver and has been hypothetically attributed to the caustic effect of the parasiticide solution injected into the cyst and diffusing into the biliary tree through a cystic-biliary fistula. In this experimental study, we showed that, in rats, injection into the biliary tract of 20% hypertonic saline solution or 2% formaldehyde solution, the most commonly used scolicidal solutions, was followed by lesions of the biliary epithelium. As compared with 20% hypertonic saline solution, the 2% formaldehyde solut…

[Diagnosis and treatment of billiary ileus].

INTRODUCTION: Biliary ileus represents only 14% of mechanical obstructions of the gastrointestinal tract. However, the rate of non-strangulated small bowel obstructions reaches as much as 25% in patients over 65 years of age. Usually, a pressure necrosis is created by a large gallstone that passes through the developed biliodigestive fistula, subsequently obturating the gastrointestinal tract. CASE REPORTS: 35 patients underwent a surgical procedure at the Department of Surgery, Teaching Hospital Pilsen for biliary ileus from January 1, 2000 to January 31, 2015. Mean age was 79 years (median

Successful Management of a Post-Choledochal Cyst Excision Pancreatic Fistula in an Adult Patient: A Case Report and Literature Review on Risk Factors.

Introduction  Choledochal cyst is a premalignant condition and surgical excision with biliary enteric anastomosis is the standard of care. Surgical treatment in adults may be difficult due to associated biliary pathology and high incidence of postoperative complications is reported. Postoperative pancreatic fistula (POPF) is a rare early complication following choledochal cyst excision. Material and Methods  A 23-year-old male patient was operated for a Todani type IV-A choledochal cyst with anomalous pancreaticobiliary junction. Cyst excision with hepaticojejunostomy was performed. [Low obturation ileus caused by gallstone incarceration in sigmoid colon].

Authors presents two cases of low obturation ileus caused by gallstone which had obstructed sigmoid colon. They pay attention to diagnostic problems and usefulness of sonography and X-ray techniques also in case of biliary ileus. Authors shows difficulties in surgical procedures, prefering enterotomy and evacuation of concrement, without simultaneous internal biliary fistula liquidation, especially in cases of increased risk and in old age patients.

Subcutaneous herniation of gall-bladder with spontaneous cholecystocutaneous fistula.

Spontaneous cholecystocutaneous fistula is now a clinical rarity due to improvements in the treatment of biliary tract diseases. This case report describes a 67-year-old woman in whom subcutaneous herniation of a pathologically dilated gall-bladder was followed by spontaneous discharge of pus and mucus with formation of a cholecystocutaneous fistula. The use of serial computed tomography examinations enabled this unusual diagnosis to be made.

A Rare Spontaneous Gastrobiliary Fistula.

We report the case of a 69-year-old man with a spontaneous gastrobiliary fistula. Internal biliary fistulas are usually the result of longstanding, untreated choledocholithiasis, cholecystolithiasis, peptic ulcers or rarely neoplasia. This patient's unspecific clinical picture led to a late diagnosis, which was made during surgery. How to cite this article: Chwiesko A, Jurkowska G, Kedra B, Okulczyk B, Kamocki Z, Dabrowski A. A Rare Spontaneous Gastrobiliary Fistula. Euroasian J Hepato-Gastroenterol 2014;4(2):101-103.

[Endoscopic interventions of the biliary tract in postoperative complications after cholecystectomy for preventing relaparotomy].

The postoperative complication rate of laparoscopic cholecystectomy (LC) is about 5-6%. The most frequent complications are residual gallstones in the common bile duct, biliary leckage, biliary fistula and duct stenosis. In the period between 01.11.94 and 01.04.96 we performed 1620 endoscopic retrograde cholangio pancreatographies including 410 papillotomies at the second Department of Medicine in the Clinic of Suhl. Thereby in 2.1% (34 cases) of patients a complication after laparosopic cholecystectomy was seen and endoscopically controlled. Residual bile duct stones were removed without any …

Diagnosis and Operative Management of a Cholecystopleural Fistula Following Iatrogenic Injury.

A cholecystopleural fistula is an abnormal connection between the gallbladder and the pleural space. Potential causes of this rare fistula include malignancy, trauma, iatrogenic injury, and more. Failure to recognize a cholecystopleural fistula can lead to various serious complications. We present the case of a patient who underwent an aborted percutaneous cholecystostomy tube placement for recurrent acute cholecystitis, followed by percutaneous transhepatic biliary tube placement with a clinical course that was complicated by the development of an empyema. The patient was managed operatively

Anesthetic management of a patient with acquired bronchobiliary fistula: a case report.

BACKGROUND: Bronchobiliary Fistula (BBF), a rare abnormal connection between the biliary tract and bronchial tree, typically arises from hepatobiliary surgery, infection, or trauma. Perioperative anesthetic management presents substantial challenges due to risks of pulmonary aspiration, cross-contamination, and respiratory compromise. CASE PRESENTATION: A 61-year-old female with a history of intrahepatic cholangiocarcinoma resection developed cough, fever, and bilious sputum. Imaging confirmed BBF with bilateral pneumonia. She underwent re-exploration and fistula repair. Anesthetic management …
