Point estimation is particularly important in predicting weight loss in individuals or small groups. In this analysis, a new health response function is based on a model of human response over time to estimate long-term health outcomes from a change point in short-term linear regression. This important estimation capability is addressed for small groups and single-subject designs in pilot studies for clinical trials, medical and therapeutic clinical practice. These estimations are based on a change point given by parameters derived from short-term participant data in ordinary least squares (OL

Cohen's kappa coefficient, which was introduced in 1960, serves as the most widely employed coefficient to assess inter-observer agreement for categorical outcomes. However, the original kappa can only be applied to cross-sectional binary measurements and, therefore, cannot be applied in the practical situation when the observers evaluate the same subjects at repeated time intervals. This study summarizes six methods of assessing agreement of repeated binary outcomes under different assumptions and discusses under which condition we should use the most appropriate method in practice. These app

A major biomedical goal associated with evaluating a candidate biomarker or developing a predictive model score for event-time outcomes is to accurately distinguish between incident cases from the controls surviving beyond t throughout the entire study period. Extensions of standard binary classification measures like time-dependent sensitivity, specificity, and receiver operating characteristic (ROC) curves have been developed in this context (Heagerty, P. J., and others, 2000. Time-dependent ROC curves for censored survival data and a diagnostic marker. Biometrics 56, 337-344). We propose a

The transmission dynamics of infectious diseases have been traditionally described through a time-inhomogeneous Poisson process, thus assuming exponentially distributed levels of disease tolerance following the Sellke construction. Here we focus on a generalization using Weibull individual tolerance thresholds under the susceptible-exposed-infectious-removed class of models which is widely employed in epidemics. Applications with experimental foot-and-mouth disease and historical smallpox data are discussed, and simulation results are presented. Inference is carried out using Markov chain Mont

A population average regression model is proposed to assess the marginal effects of covariates on the cumulative incidence function when there is dependence across individuals within a cluster in the competing risks setting. This method extends the Fine-Gray proportional hazards model for the subdistribution to situations, where individuals within a cluster may be correlated due to unobserved shared factors. Estimators of the regression parameters in the marginal model are developed under an independence working assumption where the correlation across individuals within a cluster is completely

Dropout is a common occurrence in longitudinal studies. Building upon the pattern-mixture modeling approach within the Bayesian paradigm, we propose a general framework of varying-coefficient models for longitudinal data with informative dropout, where measurement times can be irregular and dropout can occur at any point in continuous time (not just at observation times) together with administrative censoring. Specifically, we assume that the longitudinal outcome process depends on the dropout process through its model parameters. The unconditional distribution of the repeated measures is a mi

We propose a new class of semiparametric generalized linear models. As with existing models, these models are specified via a linear predictor and a link function for the mean of response Y as a function of predictors X. Here, however, the "baseline" distribution of Y at a given reference mean mu(0) is left unspecified and is estimated from the data. The response distribution when the mean differs from mu(0) is then generated via exponential tilting of the baseline distribution, yielding a response model that is a natural exponential family, with corresponding canonical link and variance funct

Retrospective case-control studies are more susceptible to selection bias than other epidemiologic studies as by design they require that both cases and controls are representative of the same population. However, as cases and control recruitment processes are often different, it is not always obvious that the necessary exchangeability conditions hold. Selection bias typically arises when the selection criteria are associated with the risk factor under investigation. We develop a method which produces bias-adjusted estimates for the odds ratio. Our method hinges on 2 conditions. The first is t

We propose a new statistics for the detection of differentially expressed genes when the genes are activated only in a subset of the samples. Statistics designed for this unconventional circumstance has proved to be valuable for most cancer studies, where oncogenes are activated for a small number of disease samples. Previous efforts made in this direction include cancer outlier profile analysis (Tomlins and others, 2005), outlier sum (Tibshirani and Hastie, 2007), and outlier robust t-statistics (Wu, 2007). We propose a new statistics called maximum ordered subset t-statistics (MOST) which se

Mortality rate ratios and the associated proportional hazards models have been used to summarize the effect of Alzheimer's disease on longevity. However, the mortality rate ratios vary by age and therefore do not provide a simple parsimonious summary of the effect of the disease on lifespan. Instead, we propose a new parameter that is defined by an additive multistate model. The proposed multistate model accounts for different stages of disease progression. The underlying assumption of the model is that the effect of disease on mortality is to add a constant amount to death rates once the dise

In a growth model, individuals move progressively through a series of states in which each state is indicative of developmental status. Interest lies in estimating the rate of progression through each state while incorporating covariates that might affect the transition rates. We develop a Bayesian discrete-time multistate growth model for inference from cross-sectional data with unknown initiation times. For each subject, data are collected at only one time point at which we observe the state as well as covariates that measure developmental progress. We link the developmental progress variabl

In this paper, we apply flexible Bayesian survival analysis methods to investigate the risk of lymphoma associated with kidney transplantation among patients with end-stage renal disease. Of key interest is the potentially time-varying effect of a time-dependent exposure: transplant status. Bayesian modeling of the baseline hazard and the effect of transplant requires consideration of 2 timescales: time since study start and time since transplantation, respectively. Previous related work has not dealt with the separation of multiple timescales. Using a hierarchical model for the hazard functio

Patients who have undergone renal transplantation are monitored longitudinally at irregular time intervals over 10 years or more. This yields a set of biochemical and physiological markers containing valuable information to anticipate a failure of the graft. A general linear, generalized linear, or nonlinear mixed model is used to describe the longitudinal profile of each marker. To account for the correlation between markers, the univariate mixed models are combined into a multivariate mixed model (MMM) by specifying a joint distribution for the random effects. Due to the high number of marke

The use of survival models involving a random effect or 'frailty' term is becoming more common. Usually the random effects are assumed to represent different clusters, and clusters are assumed to be independent. In this paper, we consider random effects corresponding to clusters that are spatially arranged, such as clinical sites or geographical regions. That is, we might suspect that random effects corresponding to strata in closer proximity to each other might also be similar in magnitude. Such spatial arrangement of the strata can be modeled in several ways, but we group these ways into two

We consider testing whether the nonparametric function in a semiparametric additive mixed model is a simple fixed degree polynomial, for example, a simple linear function. This test provides a goodness-of-fit test for checking parametric models against nonparametric models. It is based on the mixed-model representation of the smoothing spline estimator of the nonparametric function and the variance component score test by treating the inverse of the smoothing parameter as an extra variance component. We also consider testing the equivalence of two nonparametric functions in semiparametric addi

BACKGROUND: Preliminary retrospective data suggest it is possible to identify impalpable breast cancer in women presenting with a family history of breast cancer under the age of 50, by using regular mammography. In consequence, this service is offered in a number of centres in the UK. The effectiveness of such a service, however, has not been fully evaluated.
METHODS: We propose to perform such an evaluation in a cohort of 20000 women under the age of 50 with a significant family history of breast cancer, given regular mammographic surveillance over 5 years. Comparison of surgical and pathol

BACKGROUND: Epidemiological studies of the effects of hormone replacement therapy (HRT) often rely on exposure data and information on past health from self-administered questionnaires. The accuracy with which women report current use of HRT and the specific preparation in use is not known. This study aims to compare aspects of self-reported use of HRT and treatment for various conditions with data from general practice prescription records.
METHODS: Reported questionnaire data on use of HRT were compared with those on the general practice prescription record for 570 women participating in th

A stochastic epidemic model featuring fixed-length latent periods, gamma-distributed infectious periods and randomly varying heterogeneity among susceptibles is considered. A Markov chain Monte Carlo algorithm is developed for performing Bayesian inference for the parameters governing the infectious-period length and the hyper-parameters governing the heterogeneity of susceptibility. This method of analysis applies to a wider class of diseases than methods proposed previously. An application to smallpox data confirms results about heterogeneity suggested by an earlier analysis that relied on l
