[Giant cell arteritis - two cases with acute blindness].

Giant cell arteritis is characterized primarily by inflammation in certain large and medium-sized arteries. The major risk factors are age, female gender and Northern European descent. In this report we describe two cases of acute vision loss due to giant cell arteritis. In both cases the erythrocyte sedimentation rate (ESR) was below 50 mm/hr and the presenting complaint was foggy vision followed by acute blindness. The cases are to some extent different, for example in the former case the patient reported jaw claudication and ophthalmologic evaluation was consistent with anterior ischemic op…

Vitamin A responsive night blindness in Dent's disease.

Dent's disease is an X-linked renal tubular disorder characterized by low molecular weight proteinuria, hypercalciuria and nephrocalcinosis or nephrolithiasis. The disease is caused by mutations in a renal chloride channel gene, CLCN5. We report on three boys, of Indian origin, with Dent's disease that presented at an early age (1-4 years), with polyuria, polydipsia, salt craving, recurrent vitamin A-responsive night blindness, hypophosphataemic rickets, hypercalciuria and low molecular weight proteinuria. All these patients were found to have novel mutations in the CLCN5 gene.

Blindness, optic atrophy and sinusitis in the horse.

OBJECTIVE: The case histories described each presented with a visual deficit, varying from permanent total blindness with ophthalmoscopic evidence of optic atrophy to variable and transient visual disturbances, including occasional blindness, but with absence of ophthalmoscopic or any other ocular abnormality. ANIMALS STUDIED: Three horses of widely different age and type, but all with an original history of upper respiratory tract infection. PROCEDURE: All three cases were examined by a specialist veterinary ophthalmologist. In addition, magnetic resonance imaging (MRI) and, where possible,…

Nystagmus characteristics in congenital stationary night blindness (CSNB).

AIM: To analyse nystagmus characteristics in patients with congenital stationary night blindness (CSNB) for differentiation from other forms of early childhood nystagmus. METHODS: Horizontal and vertical eye movements of 10 patients (6-46 years, mean 17.1 years, median 12.5 years) with CSNB (eight with CSNB1, two with CSNB2) were recorded with the scleral magnetic search coil technique or by electro-oculography. Nystagmus characteristics such as the amplitude, frequency, conjugacy and intermittency were analysed. RESULTS: All patients had continuous, pendular, oblique and mostly dysconjugate…

Zinc therapy for night blindness in cystic fibrosis.

This is the first report of a supplemented CF patient presenting with clinical vitamin A deficiency to be successfully treated with zinc therapy alone. Therefore in addition to retinol supplementation, normalizing serum zinc levels may be important in maintaining the vitamin A status of CF patients. The interactions and synergistic effects between the two micronutrients are discussed.

Transient cortical blindness after cardiac catheterization with iobitridol.

The occurrence rate of transient cortical blindness after contrast media exposure has been reported to be as high as 1% to 4% after cerebral or vertebral angiography, but such blindness has been described in only a few cases of coronary angiography with modern, non-ionic, low-osmolality radio-contrast agents. In this study, we present a case of abrupt cortical blindness after exposure to contrast media during diagnostic coronary angiography; to our knowledge, this is the 1st report in the medical literature that describes transient cortical blindness after iobitridol use.

Simultanagnosia: effects of semantic category and repetition blindness.

When confronted with two identical stimuli in a very brief period of time subjects often fail to report the second stimulus, a phenomenon termed "repetition blindness". The "type-token" account attributes the phenomenon to a failure to individuate the exemplars. We report a subject, KE, who developed simultanagnosia (the inability to see more than one item in an array) as a consequence of bilateral parietal lobe infarctions. With presentation of two words, pictures or letters for an unlimited time, KE typically reported both stimuli on less than half of trials. Performance was significantly in…

[An old woman with sudden pareses and blindness].

We present a patient with Posterior Reversible Encephalopathy Syndrome (PRES). A 74-year-old woman was admitted with sepsis, which originated from erysipelas on her neck the following day. She developed respiratory obstruction due to oedema, septic shock, disseminated intravascular coagulation (DIC), acute renal failure and atrial fibrillation. She responded well to treatment and improved rapidly, despite of her serious condition. When she had almost fully recovered after 15 days, her general condition worsened, and she developed confusion, blindness and pareses. MRI showed vasogenic oedema in…

Modeling targeted ivermectin treatment for controlling river blindness.

There is considerable host heterogeneity in exposure to onchocerciasis. We incorporate this heterogeneity into a model of onchocerciasis transmission that we use to evaluate intervention strategies targeting specific portions of the human population for treatment with ivermectin. Our model predicts that targeted allocation of ivermectin in a highly heterogeneous population will reduce the public health burden of onchocerciasis using 20-25% of the doses of untargeted allocation. Targeted allocation therefore poses significantly lower risk of adverse effects, while potentially delaying the emerg

Changing pattern of childhood blindness in Maharashtra, India.

AIM: To determine the causes of severe visual impairment and blindness in children in schools for the blind in Maharashtra, India. METHODS: Children aged <16 years with a visual acuity of <6/60 in the better eye, attending 35 schools for the blind were examined between 2002 and 2005, and causes were classified using the World Health Organization's system. RESULTS: 1985 students were examined, 1778 of whom fulfilled the eligibility criteria. The major causes of visual loss were congenital anomalies (microphthalmos or anophthalmos; 735, 41.3%), corneal conditions (mainly scarring; 395, 22.2%),…

[Congenital stationary night blindness (CSNB)--a case report].

PURPOSE: To document occurrence of the congenital stationary night blindness type Schubert-Bornschein in some members of one family, to put together the family tree, and to evaluate the examination methods. Among others the electroretinogram (ERG) is substantial in the diagnosis of the disease and to distinguish different types of CSNB. In the affected family, only in men the disease is expressed, females are transmitters. This corresponds to the X-linked inheritance. Simultaneously with CSNB, in this family, high myopia occurs not only in men, but also in women. CONCLUSION: CSNB is an inheri…

Blindness due to retinal dysplasia in broiler chicks.

Thirty-three live 9-day-old broiler chicks were submitted for laboratory evaluation because of blindness. Blindness was observed in up to 1% of 25,000 birds from four different houses. All the chicks were from the same breeder source. Blindness was apparent when the chicks were 2-3 days old. Clinically, the blind chicks were smaller than their counterparts, were unable to find feed and water, and wandered aimlessly. Necropsy did not reveal any lesions. Two chicks examined clinically at 14 days of age lacked pupillary reflexes after light stimulation, and the anterior and posterior segments of …

Organic alexithymia: a study of acquired emotional blindness.

The phrase 'organic alexithymia' is introduced as a clinically and theoretically useful construct for furthering understanding of alexithymia and the occurrence of alexithymic symptoms in patients with acquired brain injury (ABI). The construct is illustrated by the case study of HR, a 21-year-old man seen at the State Head Injury Unit, Perth, for neuropsychological and clinical assessment 2 years following a motor vehicle accident. HR's case supports the hypotheses that a syndrome substantially similar to alexithymia can occur in patients with acquired brain injury, and that the acquired natu

Blindness in the Indian state of Andhra Pradesh.

PURPOSE: To determine the current prevalence and causes of blindness in the Indian state of Andhra Pradesh to assess if blindness has decreased since the last survey of 1986-1989. METHODS: A population-based epidemiology study, using a stratified, random, cluster, systematic sampling strategy, was conducted in the state of Andhra Pradesh in India. Participants of all ages (n = 10,293), 87.3% of the 11,786 eligible, from 94 clusters in one urban and three rural areas representative of the population of Andhra Pradesh, underwent interview and a detailed dilated ocular evaluation by trained prof…

Evaluation of patients with decreased vision or blindness.

A good ocular examination and careful evaluation of the pupillary light reflexes will localize most lesion(s) producing a complaint of blindness. Lesions of the clear media severe enough to produce blindness will prohibit fundus examination. Retinal, optic nerve, and complete chiasmal lesions will produce dilated, slowly reactive pupils. Retinal blindness is usually associated with dramatic ophthalmoscopic lesions of diffuse altered reflectivity and vascular attenuation, the major exceptions being the sudden acquired retinal degeneration syndrome and some patients with glaucoma. Optic nerve le…

Blindness in Africa: present situation and future needs.

AIM: To review the prevalence and causes of blindness in sub-Saharan Africa, the existing services and limitations, and the Vision 2020 goals for the future. METHODS: Methodologically sound population based surveys published in the past 20 years are reviewed and results for prevalence and causes of blindness are tabulated. The current resources and needs according to recent publications and international working groups are described. CONCLUSIONS: Blindness prevalence rates vary widely but the evidence suggests that approximately 1% of Africans are blind. The major cause is cataract; trachoma…

Row blindness in Gestalt grouping and developmental dyslexia.

A classic Gestalt figure is a 4 x 4 array of items grouped by similarity into either rows or columns. We found that some people do not see rows ("row blindness"). Furthermore, row blindness correlates with difficulties processing written language--more than half of the college-age dyslexics tested were row blind. Reading difficulties are probably not a cause of row blindness and forming rows by grouping is a part of reading, suggesting that row blindness might be one source of problems processing written language.

Reversible blindness associated with alcoholic ketoacidosis: pseudomethanol intoxication.

We report a case of reversible blindness associated with severe acidosis. A 49-year-old female presented with sudden onset of bilateral blindness. A work up for possible etiologic factors including computed tomography failed to reveal any abnormalities except for ethanol induced severe ketoacidosis. Methanol was not detectable in the serum despite an ethanol level of 14.76 mmol/L, effectively ruling out the diagnosis of methanol intoxication. Treatment of the acidosis led to rapid resolution of her blindness. Review of the literature revealed two case reports of reversible blindness associated…
